What are the essentials of life for you? Day to day, week after week, month

into month, year by year… what are the things that you most treasure

about life? What are the elements that, if you could not continue to be

engaged in them, would raise questions about quality of life for you?

e each answer these questions differently — some of us more rooted in relationships, some of us more rooted in activities. All of us, at some point,

depend on help for the care of our loved ones, or care of our own self. Of course, when we are asked to answer these questions from vulnerability — who and what kind of support do we need in such tender times?

Each fall, at our annual meeting, we pause together to celebrate and give thanks for what has been in the last year, and to learn a little more together over food and conversation. We ask, implicitly, “What are the essentials for health-care at home?”

With the advent of new leadership and staff, and the ever-changing face of health care at large, it has been a year of change! Invitation to think about essentials again. The agency has had a strong year, financially and organizationally, stepping into some new ideas, and shoring up continuing ones. Lisa’s energy and dedication have been a joy to welcome!

For myself, it has always been an honor to volunteer with this agency — on the board, in people’s homes, and in circles of grief sharing and ritual.

Central New Hampshire VNA & Hospice tries to keep the essentials of our patients, families, and wider community at the heart of who we are. Meeting the needs of folks, even as they evolve and change. Sometimes simply sitting with the questions, and asking them together.

FROM OUR CHIEF EXECUTIVE OFFICER

s the ancient Greek Philosopher, Heraclitus once said “change is the only constant in life.”

During the past year at Central New Hampshire VNA & Hospice, this proverb has rung true. Externally, the rapidly changing regulatory environment is a challenge; Medicare has implemented new Conditions of Participation, which was the first amendment of its kind by the Center for Medicare and Medicaid in over 10 years. As a result, we needed to revise our practices and policies to meet the regulations while continuing to provide care to over 2,200 patients. As part of these efforts, we needed to develop detailed emergency preparedness plans for the agency, our patients, our staff, as well as coordination within the wider community.

All of these adjustments were being worked on during the internal change of leadership. I came on board as the Chief Executive Office in December, three-quarters of the way through the fiscal year. I would like to acknowledge the leadership team for their work during this time. Championed by Chris Long, Interim CEO, it was indeed, a job well done. During this year, there were many other staffing changes including the filling of many positions. Even with all these competing priorities and challenges, the agency had a very good year. This success can be credited to the dedication of all the staff who worked tirelessly in our commitment to our patients and the communities we service.

Relationship and community are the foundations of change. We are planning for a coming year of building. Our plans for the coming year include: more staff education and hiring, refreshing our logo, and a new look for our website. We will be strengthening our relationships with our community and healthcare provider partners. We will be continuing our commitment to our patients and referral sources, through collaborative relationship building. Partnerships are key, as we all need to work together in the healthcare industry, for a better outcome with the patient at the heart of system.

With our foundations shored-up, we can now go on to make Central New Hampshire VNA & Hospice the provider and employer of choice. As we move forward, I am certain Central New Hampshire VNA & Hospice will continue to adapt to the healthcare changes that we face. With our strong leadership team and staff and the support of our Board of Trustees and the wider community, Central will remain one of the top agencies in the state and the New England region. We are positioned with our longevity and stability, to be able to say; “we have been here, we will be here.”

MAX’S STORY

This is a story about one little patient you have helped.

ust imagine you bring your 4 year old to the pediatrician because his

color is poor, his appetite is off and he is more tired than you think he should be. The diagnoses comes back and your worst fears are realized – your son has leukemia.

This is a true story. Shortly after his fourth birthday, Max needed chemo.

Pediatric cancer is doubly and even triply difficult because it affects the family in so many ways. In addition to the huge emotional toll, other issues compete for attention. Who’s going to drive Max to Boston twice a week? Who will stay home with his older sister? How much school will they miss? How will Mom and Dad keep their jobs?

Central VNA is the only agency north of Concord that provides healthcare AT HOME to children birth – 18 years of age. “Mr. Max,” as he is known around here, needed weekly blood work and weekly chemo for two full years. That is over 200 appointments. But because

Central VNA could bring the treatment to Max, the impact to his family was greatly reduced. And Max could be seen in his home environment and not a sterile room with a plastic chair, hooked up to a pump by strangers.

When we arrived for his final treatment, the neighbors were there to greet us. Drums banging, hands clapping, cheering and singing ensured. Their thanks goes to you. Because you were there for Max, he is cancer free, back in school full time, and living the life that a six year old is meant to live. Mom and Dad kept their jobs. His sister lost no school days and has moved into the third grade on schedule. And the family is intact.

Your donations helped make this outcome possible. Mr. Max is now a member of your family – the family of patients that Central VNA has treated. Sadly, there are 3 more children receiving chemo at home, and their costs are exorbitant. Your continued support is very much appreciated.
