EROSION AND SEDIMENTATION CONTROL NOTES

1. SOIL EROSION AND SEDIMENTATION CONTROLS SHALL BE PROVIDED IN ACCORDANCE WITH THE "MASSACHUSETTS SOIL EROSION ANDSEDIMENT CONTROL HANDBOOK" THE NPDES GENERAL PERMIT FOR STORM WATER DISCHARGE FROM CONSTRUCTION ACTMTY (SEPT. 2003),AND THE NOTES AND DETAILS SHOWN IN THIS PLAN SET.

2. DUST SHALL BE CONlROLl.ED BY WATERING OR OTHER APPROVED METHODS AS NECESSARY, OR AS DIRECTED BY lHE OWNER OR ENGINEER.

3. THE CONTRACTOR SHALL SOiEDULE HIS/HER WORK TO All.OW THE FINISHED SUB GRADE ELEVATIONS TO DRAIN PROPERLY WITHOUTPONDING. SPECIFICALLY, ALLOW WATER TO ESCAPE WHERE PROPOSED CURB MAY RETAIN RUNOFF PRIOR TO APPLICATION OF SURFACEPAVING. PROVIDE TEMPORARY POSITNE DRAINAGE, AS REQUIRED, TO STABILIZED DISCHARGE POINTS. CONlRACTOR IS RESPONSIBLE FORALL TEMPORARY UTILITY CONNECTIONS.

4. THE CONTRACTOR SHALL MAINTAIN A SUFFIOENT RESERVE OF VARIOUS EROSION CONTROL MATERIALS ONSITE AT ALL TIMES FOREMERGENCY PURPOSES.

5. ANY DEWATERING WASTE WATERS PUMPED FROM EXCAVATIONS SHAU. BE CONVEYED BY HOSE TO AN UPLAND AREA AND DISCHARGED INTOHAY BALE ENCLOSURES OR SEDIMENTATION BAGS OUTSIDE ALL WETLAND AND WETLAND BUFFER AREAS AS REQUIRED.

6. CONSTRUCTION SITE WASTE MATERIALS SHALL BE PROPERLY CONTAINED ONSITE AND DISPOSED OFF SITE IN ACCORDANCE WITH ALLAPPLICABLE LOCAL AND STATE REGULATIONS.

7. ALL DISTURBED AREAS SHALL BE STABILIZED WITHIN 14 DAYS OF COMPLETION OF WORK IN THAT AREA.

B. ALL DRAINAGE STRUCTURES SHALL BE CLEARED OF ACCUMULATED SEDIMENT PRIOR TO ACCEPTANCE OF FINAL PROJECT.

9. THE CONTRACTOR SHALL MAINTAIN SURFACE DRAINAGE DURING CONSTRUCTION. STORMWATER SHALL BE MAINTAINED AWAY FROM WORKSITES WHILE PREVENTING AREAS OF EROSION.

10. CONTRACTOR IS RESPONSIBLE FOR INSTALLING AND MAINTAINING NECESSARY TEMPORARY EROSION CONTROL MEASURES.

11. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ESTABLISHING OR INSTALLING ALL TEMPORARY SEDIMENT AND EROSION CONTROLS ASSHOWN ON THESE PLANS AND SHALL MAINTAIN ALL EROSION CONTROL MEA.5URES AS NECESSARY DURING THE ENTIRE CONSTRUcnONPERIOD.

12. TEMPORARY DIVERSION SWALES SHALL BE PROVIDED AS NECESSARY TO DIRECT RUNOFF TO THE SEDIMENT BASINS DURING CONSTRUCTION.

13. ANTI-TRACKING PADS AND WHEEL WASH STATIONS SHALL BE PROVIDED AT ALL POINTS OF EGRESS OR INGRESS AND SHALL BE MAINTAINEDTO LIMIT lRACKING OR FLOWING OF SEDIMENT ONTO PUBLIC ROADS AND INTO THE REST OF THE SITE.

14. EROSION CONTROL BARRIERS SHALL BE INSTALLED AS SHOWN ON THE EROSION CONTROL PLAN PRIOR TO COMMENCEMENT OFCONSlRUCTION OPERATIONS.

15. SOIL EROSION AND SEDIMENTATION CONTROL MEASURES SHALL BE INSPECTED AND MAINTAINED ON A WEEKLY BASIS AND AFTER EACHSTORM EVENT OF 0.25 INCH OR GREATER DURING CONSTRUCTION TO INSURE THAT CHANNELS, DITCHES AND PIPES ARE CLEAR OF DEBRISAND THAT THE EROSION CONTROL BARRIERS ARE INTACT. IDENTIFIED DEFIOENCIES SHALL BE CORRECTED IMMEDIATELY.

16. THE CONTRACTOR SHALL CLEAN AND MAINTAIN EROSION CONTROL BARRIER WHEN SEDIMENT ACCUMULATES TO ONE HALF THE HEIGHT OFTHE HAYBALES OR ONE lli!RD THE HEIGHT OF SILT FENCE. MATERIAL COLLECTED FROM THE SILTATION BARRIERS SHALL BE REMOVED ASNECESSARY AND DISPOSED IN AN UPLAND AREA.

17. INSTALLATION OF THE EROSION CONTROL BARRIERS AS ILWSTRATED IS INTENDED TO REPRESENT THE MINIMUM SEDIMENTATION CONTROLFACILITIES NECESSARY TO MEET ANTICIPATED SITE CONDillONS. ADDilIONAL EROSION CONTROL MEASURES SHALL BE IMPLEMENTED ASCONDmONS WARRANT OR AS DIRECTED BY THE OWNER OR OWNER'S REPRESENTATIVE.

16. REQUIRED SEDIMENTATION CONTROL FACTL11IES MUST BE PROPERLY ESTABLISHED, CLEARLY VISIBLE AND IN OPERATION PRIOR TOINITIATING ANY LAND CLEARING ACTIVITY AND/OR OTHER CONSTRUCTION RELATED WORK. SUCH FACTL11IES SHALL REPRESENT THE LIMITOF WORK. WORKERS SHALL BE INFORMED THAT NO CONSTRUCITON ACTIVITY IS TO OCCUR BEYOND THE LIMIT OF WORK AT ANY TIMETHROUGHOUT THE CONSTRUCTION PERIOD.

19. CATCH BASINS AND STORM DRAIN INUETS SHALL BE PROTECTED WITH INLET PROTECTION.

20. THE CONTRACTOR SHALL NOT REMOVE ANY HAY BALES, SILT FENCE OR OlliER EROSION CONTROLS UNTIL THE CONTRIBUTING DRAINAGEAREA HAS BEEN PERMANENTLY STABILIZED.

21. RIP-RAP OR OTHER ENERGY DISSIPATERS SHALL BE USED WHERE NECESSARY TO PREVENT SCOUR.

GRADING AND UTILITY NOTES

1. UNDERGROUND UTILITIES ARE APPROXIMATE AND ASSUMED. BEFORE COMMENONG SITE WORK IN ANY AREA, CONTACT "DIG SAFE" AT1-686-DIG-SAFE, AND ALL UTILITY COMPANIES NOT COVERED BY "DIG SAFE", TO ACCURATELY LOCATE UNDERGROUND UTILITIES. ANYDAMAGE TO EXISTING UTILITIES OR STRUCTURES AS SHOWN ON THE PLANS SHALL BE THE CONTRACTOR'S RESPONSIBILITY. COSTS OF SUCHDAMAGE SHALL BE THE CONTRACTOR'S RESPONSimLITY. NO EXCAVATION SHALL BE DONE UNTIL UTILITY COMPANIES ARE PROPERLYNOTIFIED.

2. ALL EXISTING CONDillONS SHOWN SHALL BE CONSIDERED APPROXIMATE AND ARE BASED ON THE BEST AVAILABLE INFORMATION. THE CONTRACTOR SHALL BE RESPONSIBLE FOR VERIFYING THAT THE PROPOSED IMPROVEMENTS SHOWN ON THE PLANS DO NOT CONFLICT WITH ANY KNOWN EXISTING OR OTHER PROPOSED IMPROVEMENTS. IF CONFLICTS ARE DISCOVERED, THE CONTRACTOR SHALL NOTIFY THE OWNERAND THE ENGINEER PRIOR TO INSTALLATION OF ANY PORTION OF THE SITT WORK WHICH WOULD BE AFFECTED. NO FIELD ADJUSTMENTS INTHE LOCATION OF SITE ELEMENTS SHALL BE MADE WITHOUT THE ENGINEERS APPROVAL

3. ALL WORK PERFORMED AND ALL MATERIALS FURNISHED SHALL CONFORM Willi THE LINES AND GRADES ON lliE PLANS AND SITE WORKSPECIFICATIONS, UNLESS OTHERWISE DIRECTED.

4. AT ALL LOCATIONS WHERE EXISTING CURBING OR PAVEMENT ABUT NEW CONSTRUCTION, THE EDGE OF THE EXISTING CURB OR PAVEMENTSHALL BE SAW CUT TO A CLEAN, SMOOTH EDGE. BLEND NEW PAVEMENT AND CURBS SMOOTHLY INTO EXISTING BY MATCHING LINES, GRADESAND JOINTS.

5. ALL EXISTING AND PROPOSED UTILITY COVERS, GRATES, ETC. SHALL BE ADJUSTED TO BE FLUSH WITH THE SURROUNDING SURFACE ORPAVEMENT FINISH GRADE OF THIS CONTRACT. RIM ELEVATIONS OF STRUCTURES AND MANHOLES ARE APPROXIMATE. FINAL ELEVATIONS ARETO BE SET FLUSH AND CONSISTENT Willi THE PROPOSED FINAL GRADES.

6. WHERE AN EXISTING UTILITY IS FOUND TO CONFLICT WITH THE PROPOSED WORK, THE LOCATION, ELEVATION AND SIZE OF THE UTILITYSHALL BE ACCURATELY DETERMINED WITHOUT DELAY BY THE CONTRACTOR AND lliE INFORMATION FURNISHED TO THE ENGINEER FORRESOLUTION.

7. THE CONTRACTOR SHALL PROTECT ALL UNDERGROUND DRAINAGE, SEWER AND UTILITY FACTLITIES FROM EXCESSIVE VEHIOJLAR LOADSDURING CONSTRUCTION. ANY DAMAGE TO THESE FACIL11IES RESULTING FROM CONSTRUCTION LOADS WILL BE RESTORED TO ORIGINALCONDillON AT NO COST TO THE OWNER.

8. DURING CONSTRUCTION OPERATIONS, THE CONTRACTOR SHALL PROTECT EXISTING UTILillES BY PROVIDING TEMPORARY SUPPORTS ORSHEETING AS REQUIRED AT NO ADDillONAL COST TO THE OWNER.

9. EXCAVATION REQUIRED WITHIN THE PROXIMITY OF EXISTING UTILITY LINES SHALL BE DONE BY HAND. CONTRACTOR SHALL REPAIR ANYDAMAGE TO EXISTING UTILITY LINES OR SlRUCTURES INOJRRED DURING CONSTRUCITON OPERATIONS AT NO COST TO THE OWNER.

IO. PITCH EVENLY BETWEEN SPOT GRADES. ALL PAVED AREAS MUST PITCH TO DRAIN AT A MIN. OF 1/8" PER FOOT UNLESS SPECTFIED.

11. SITE GRADES SHALL CONFORM WITH ADA AND MAAB REQUIREMENTS.

12. CONTRACTOR IS REQUIRED TO APPLY FOR. OBTAIN, AND PAY ALL FEES ASSOCIATED WITH CONSTRUCTION PHASE PERMITS.

STORMWATER MAINTENANCE

1. PREVENTATNE MAINTENANCE SHOULD BE PERFORMED AT LEAST TWICE A YEAR, AND IDEALLY SEDIMENT SHOULD BE REMOVED FROM RAINGARDEN AFTER EVERY MAJOR STORM EVENT. THE SYSTEMS SHOULD BE INSPECTED AFTER lliE FIRST SEVERAL RAINFALL EVENTS, AFTER ALLMAJOR STORMS (2 YEAR OR GREATER), AND ON REGULAR Bl-ANNUAL SCHEDULED DATES.

2. RAIN GRADEN SHOULD BE INSPECTED AND CLEANED DURING THE REGULAR SCHEDULED INSPECTIONS. WATER QUALITY INLETS SHOULD BECLEARED OF ACCUMULATED SEDIMENT, LEAVES, AND DEBRIS AT EACH REGULAR INSPECTION, AND MORE FREQUENTLY WHERE APPROPRIATE.INLET AND OUTLET PIPES SHOULD BE CHECKED FOR CLOGGING.
