individual and the impact of these factors on the global COPD epidemic.• Identify ways to increase awareness of the worldwide COPD epidemic and available COPD treatments for the general

public and patient populations.• Explain the diagnostic tools and evidence-based treatment protocols for COPD.• Define patient empowerment and identify three patient-centric approaches to equipping patients, their families and

their communities to address the COPD epidemic.• Evaluate the latest research in COPD (e.g., data related to diagnosis, pharmacologic and non-pharmacologic treatment,

diagnosis, pharmacological advances, new biomarkers and pulmonary rehabilitation) and its translational impact on COPD care delivery.

• Identify the most prevalent COPD comorbidities and strategies for addressing these via a multidisciplinary team and individualized treatment plan approach.

• Identify proactive COPD care practices – including improving care team coordination and communication, patient education (including self-management) and follow-up strategies – and opportunities for their implementation in your delivery of care.

• Identify essential elements of a readmissions reduction program for COPD patients and how they can be implemented in your health care organization.

• Explain the COPD care community’s progress and challenges within the current public policy and legislative climate.• Identify promising innovations in the care of COPD and its comorbidities.
