Mounting research suggests that westerners harbor a less diverse distal gut microbiota than rural

and hunter-gatherer populations. Ongoing research among the hunter-gatherers in Tanzania sug-

gest that while modern diet and lifestyle choices in the industrialized world are ratcheting down

microbial diversity among these populations, it’s the destruction of regional (microbial) species

pool that may be playing a bigger role in human health than previously expected. When consid-

ered against studies like the American Gut project, managing environmental filters may be an

opportunity to reposition our global conversation about prebiotics and probiotics and provide an

on-ramp for a new generation of micro-environmentalist.

Jeff is the Founder ofthe Human FoodProject and co-founderof America Gut, theworld’s largest, open-sourcemicrobiome project. He has spent thelast few years living and working amongvarious groups in Africa characterizingthe microbiome of rural and hunter-gatherer groups that “might” hold theclues to the ecological disaster known asthe modern human gut. David Keller

David began withGaneden as aconsultant in 2003 andhas been with thecompany for more than 14years. To date, 25 clinical trials andreview papers have been published.David was able to obtain FDA GRAS(Generally Regarded as Safe) certificationfor Ganeden’s patented strain ofprobiotic bacteria, GanedenBC30™(Bacillus coagulans GBI-30, 6086) for bothfoods and infant formula, and authoredthe first probiotic specific USPmonograph. David chaired the workinggroup for these guidelines.

As challenges to the food industry continue to arise from both regulatory and consumer groups,the International Probiotics Association (IPA) and Council for Responsible Nutrition joined forcesto create a proactive best practices guideline for probiotics. This voluntary guideline coversboth dietary supplements and functional foods. The goal is to increase transparency to allow allparties to better understand what is contained and being sold in probiotic products. In addition,the IPA has filed a citizens’ petition with the Office of Dietary Supplement Products, formallyrequesting that the unit of measure for probiotics should be changed from milligram to colonyforming units.

From 2010 to 2014 he was a BauerFellow in the FAS Center for SystemsBiology at Harvard University, where heestablished an independent researchgroup prior to starting his faculty positionat UCSF. Notable honors include theKipnis Award in Biomedical Sciences, theNeedleman Pharmacology Prize, theDamon Runyon-Rachleff InnovationAward, and the Searle Scholars Award.He also serves on the Scientific AdvisoryBoard for Seres Therapeutics andWholeBiome.

Food, drugs, and bugs: The role of the gut microbiome in nutrition andpharmacology Dr Peter Turnbaugh, Department of Microbiology & Immunology, University of California, San Francisco

One of the most important limitations to modern medicine is the variation between patients intheir response to dietary and therapeutic interventions. Mutations in the human genome, inparticular the enzymes and transporters expressed in the intestine and liver, have a major impacton drug levels in circulation. But these studies ignore our ‘second genome’ – that of the trillionsof microorganisms that thrive in and on the human body (the microbiome). The Turnbaugh laband colleagues are pursuing a more comprehensive view of pharmacology that includes thestructure and activity of our resident microbial communities and a deeper understanding of theirinteractions with each other, with their host habitat, and with the nutritional milieu of thegastrointestinal tract. Peter’s lecture will include a detailed discussion of the following key areas:

• Humans are home to trillions of microbes that shape host health and disease• Metagenomics (culture-independent methods for microbial ecology) and gnotobiotics (germ-

free and intentionally-colonized mice) provide a powerful approach to understand host-microbiome interactions

• Gut bacteria influence the treatment of human disease by directly metabolizing therapeutics and through their interactions with host tissues

• The gut microbiome is a useful source for novel pre- and pro-biotics or even medications

Keith Barrington

Keith completed hismedical training at theUniversity of Liverpool inthe UK before moving toCanada to take up aneonatal fellowship in Edmonton Albertain 2008. Since then he has held severaluniversity posts and has served as Chairof the Fetus and Newborn Committee ofthe Canadian Pediatric Society and of theSociety of Neonatologists of Quebec.His research interests include apnea andrespiratory control, cardiovascularadaptations and circulatory support, andethical decision making. He has recentlybeen investigating the development ofthe neonatal microbiome and the role ofprobiotics in preventing serious intestinaldiseases. He has published extensively,is an active member of the Neonatalreview group of the Cochranecollaboration and a busy blogger atneonatalresearch.org.

Manipulating the microbiome in micro-premature babies Dr Keith Barrington, Neonatologist and clinical researcher, Sainte Justine University HealthCenter, and Professor of Pediatrics, University of Montreal

When babies are born prematurely the normal colonization of their intestines is deranged. Thisleads to serious complications, including infections and necrotizing enterocolitis. Routinesupplementation with probiotics has been proven to reduce these serious and life-threateningcomplications. Based on clinical experience and recent research, Keith will explain:

• Cause and effect – how premature babies are affected by their abnormal intestinal mircobiomes and how they can be treated with probiotics

• Lessons learned – experiences with the routine use of probiotics in a Canadian hospital’s neonatal intensive care unit

• Regulatory hurdles – what stands in the way of the use of probiotics in America’s high-risk hospital population

Thomas Tompkins

Thomas has beenResearch Director atLallemand since 1999where he has establisheda strong scientific programto evaluate the pre-clinical and clinicaleffects of probiotics. His internal researchteams evaluate the safety and action ofprobiotics and their effect on immunity inhuman cell systems, and novel in vivomodels. His current focus is the role ofthe microbiota, and its associatedresistome, in gastrointestinal andneurological disorders. In particular, he isinterested in identifying the impact ofprobiotics in modulating the functionalityof the brain-gut-microbiome axis. Hecollaborates with a number of externalinvestigators to evaluate the clinicalopportunities for the use of microbes inthe alleviation of symptoms associatedwith gastrointestinal and other mucosa-related disorders.

Jürgen received his MDfrom the University ofWürzburg andcompleted his habilitationthesis on dietary fiber at theGutenberg University Mainz. His primarycareer focus has been on internalmedicine, gastroenterology,endocrinology and diabetology andnutritional medicine. He led the Instituteof Physiology and Biochemistry ofNutrition of the MRI and has won severalscientific awards. Presently he works forthe Clinical Research Center Kiel.

Probiotic lactobacilli – effects on vaginal microbiota and bacterialvaginosisDr Jürgen Schrezenmeir, Professor of Internal Medicine at the Johannes-Gutenberg-University

The normal vaginal milieu of adult women is maintained by the vaginal microbiotapredominantly consisting of lactobacilli. Bacterial vaginosis (BV), the most commonmicrobiological disorder of the vaginal environment in adult women, is characterized by animbalance of vaginal microbiota: a depletion of lactobacilli (particularly of L. crispatus and L.jensenii) in favour of a wide overgrowth of the vaginal epithelium with a high diversity ofanaerobic bacteria. It is associated with increased risk for ascending urogenital infections andabortion. Although antibiotic treatment of BV is recommended, therapy is still a matter ofdebate. This presentation reviews the current rationale and evidence for the use of oralprobiotics for maintaining normal vaginal microbiota and for managing BV.

Chris Callewaert

Chris received his PhDfrom Ghent University inBelgium. He is now apostdoctoral researchfellow in the Knight lab atthe University of California, San Diego.He investigates body odor and thecharacteristics of the bacteria living in thearmpits, skin, clothes, washing machinesand homes. He is the first to undertakebacterial transplants to solve underarmbody odour.

Underarm body odor induces strong psychological and social discomforts . The axillarymicrobiome of 189 suffers has been analysed in relation to the odour and psychological impact.It was found that body odor formation was largely determined by the microbiome and is stronglylinked to phylogeny. Nutrition, gender, body mass index, age and shaving also have an impact.Bacterial transplants were carried out on 18 patients, where the malodoros microbiome wasreplaced by a non-odoros microbiome of a family member. Immediate improvements in axillaryhedonic value were seen, and a shift to staphylococcus dominance was evident within onemonth of treatment.

Dan Merenstein

Dan is an AssociateProfessor of FamilyMedicine atGeorgetown University,where he also directs FamilyMedicine research. The primary goal ofDan’s research is to provide answers tocommon clinical questions that lackevidence and improve patient care. Danis a clinical trialist who has recruited over1,700 participants for eight probiotictrials since 2006. He has been funded bythe NIH, USDA, Foundations andIndustry. He is an expert on probiotics,antibiotic stewardship in outpatientsettings and also conducts HIV researchin a large women’s cohort.

Lessons learned from Bayer: The impact on companies, providers and patientsDr Dan Merenstein, Associate Professor of Family Medicine, Georgetown University

Research into probiotics and the consumption of probiotics has exploded in the last few years.Unfortunately research, regulatory laws and usage are not synchronized. The U.S. governmentbelieved that the Bayer Corporation did not have data to support a probiotic and brought a caseagainst the company. There are many lessons to be learned from this case, from thegovernment’s approach, that of Bayer’s lawyers and the final verdict. Additionally, as newknowledge has been gained about probiotics, there are questions about when species data isenough to provide evidence of efficacy.
