The SQF Quality Achievement Awards Program recognizes the outstanding commitment, support and performance of individuals and organizations who contribute to endorsing and improving the SQF program and mission around the globe.

ruleS & eliGiBilityn All nominators and nominees must
be affiliated with the SQF program. Examples of appropriate affiliations include, but are not limited to registered auditors, consultants or trainers, licensed certification bodies, registered and certified suppliers, licensed training centers, etc.
n All nominees must have shown ongoing and dedicated support for the Safe Quality Food Program during the 2014 and/or 2015 calendar year.
n The entry form must accompany your nomination and all sections must be filled out legibly.
n Narratives must be 1,000 words or less.n Nominators must obtain permission
from the nominee prior to submitting a nomination.
n Self-nominations will be accepted.n Specific evidence and supporting
documentation must be included for all comments and accolades given by the nominator on behalf of the nominee. Examples of supporting documents include published articles, industry awards, news articles and 3rd party testimonials. These materials will not be returned to you. No videos will be accepted.
JudGinG criterian Only complete, legible entries will be
considered.
n Entries will be scored and judged by a panel consisting of a manufacturer/distributor/primary producer, past SQF Award winner, and an executive from a food retailer.
n Final award decisions will be made by members of Safe Quality Food Institute for each specific award category.
n Finalists will be notified on or before September 8, 2015. All finalists will be reviewed to ensure they are in good standing with SQF and/or their customers.
ScOrinG criteriaEntries will be scored on overall content and the nominee’s demonstrated achievements in:
n Contributing significantly to achieving the SQF Mission.
n Communicating the goals, mission and benefits of the SQF program to both internal and external stakeholders.
n Proving his/her/their understanding and commitment to the SQF program.
n Demonstrating an ongoing personal commitment to improving the awareness of food safety within the global community.
entry SuBmiSSiOnAll nominations must be submitted in the requested format no later than 12:00 AM EST (USA), July 1, 2015. Email is the preferred method of receipt. Send to The SQF Institute at [email protected]. All nominations must include the evidence as outlined. Incomplete submissions or those that do not follow the submission rules will not be evaluated.
PrizeSn We will honor the top finalists from each
category during the 2015 SQF International Conference, where award winners (one in each category) will receive a crystal award.
n The award winners will receive complimentary registration to the 2016 SQF Conference.
n All finalists will receive a reduced rate registration to the 2015 SQF Conference.
n The award winners will be recognized through a variety of SQFI marketing and advertising vehicles the entire year following.

SQFI AudItor oF the YeAr
Awarded to an individual who has demonstrated outstanding performance as a credentialed SQF auditor.
The nomination submission for this award must include:
•Completedapplicationform.
•Confirmationthatthenomineeisacurrent,registeredauditoringoodstandingwithSQFI.
•Anexecutivesummary(1000wordmaximum) that illustrates how the nominee demonstrates leadership, acts as a mentor and generally strives to improve the auditing credential and promotion of auditors.
•Evidenceoftrainingactivitiestopromoteprofessionaldevelopmentanddemonstratecontinuous improvement within the industry.
•Examplesofhowthenomineeworkswithinaswellasoutsidetheorganizationtodrivefood safety and quality.
•Maximum two letters of reference with one reference from a certification body. All references shall include the contact name, address and phone number for verification purposes.
Other considerations may include:
•ActivitiesoutsideSQFpromotingfoodsafety,suchasworkwithGFSI,regulatorybodiesor other stakeholders.

Awarded to a registered SQF Trainer who has contributed significantly to promoting the SQF mission and supporting SQF stakeholders in implementing robust food safety and quality practices.
The nomination submission for either this award must include:
•Completedapplicationform.
•Anexecutivesummary(1000wordmaximum) that describes how the nominee demonstrates professionalism, integrity, food safety knowledge and the promotion of SQF.
•ConfirmationthatthetrainernomineeiscurrentlyregisteredasanSQFTrainerandingoodstanding and associated with a training center that is also in good standing.
•PerformancemetricsastohowthenomineemeetsandexceedstheexpectationsofanSQF Trainer that includes the number of implementing and/ or Advanced Practitioner training classes taught by the candidate in relationship to the training classes offered by the training center.
•Examplesastohowthenomineeworkswiththeindustrytodrivefoodsafetyandqualityin or along the supply chain.
•Evidenceoftrainingactivitiesfocusingonimplementingfoodsafetyandqualityalongthesupply chain.
•Maximum two letters of reference with one letter from an SQF Training Center and the other from an SQF certified facility, consultant or other stakeholders in the system.
•Allreferencesshallincludethecontactname,addressandphonenumberforverificationpurposes.

SQFI MAnuFActurer/dIStrIbutor/PrIMArY Producer oF the YeAr
Awarded to an SQF certified manufacturer, distributor or primary producer who has shown a commitment to excellence through the SQF program.
The nomination submission for thia award must include:
•Completedapplicationform.
•Anexecutivesummary(1000wordmaximum) that that outlines the practices, culture and mission of the organization and the contributions to SQF. The executive summary must be from the organization’s leader such as the owner, CEO, President, or the Senior Operations Manager and include information exemplifying continuous improvement and management commitment within the organization.
•Evidencethatthesupplier/producerhasmaintainedSQFcertificationforaminimum of 3 years.
•Examplesofhowthenomineeworkswithintheorganizationtodrivefoodsafety and quality.
•Maximum three letters of reference with at least two letters from a buyer. All references shall include the contact name, address and phone number for verification purposes.
Other considerations may include:
•ActivitiesoutsideSQFpromotingfoodsafety,suchasworkwithGFSI,regulatorybodies,or other stakeholders.

SuPPlIer AchIeveMent/ SQFI PrActItIoner oF the YeAr
This award is given to an SQF Practitioner or an individual within an SQF Certified facility or company, who has demonstrated accomplishments in driving continuous growth and has taken their facility or company above and beyond the SQF program.
The nomination submission must include:
•Completedapplicationform.
•Anexecutivesummary(1000wordmaximum) that describes activities that the nominee has accomplished to drive continuous improvement within the organization beyond the SQF Code.
•Examplesofhowthenomineeworkedwithintheirorganizationactingasateacher,trainer,or mentor in support of food safety and the SQF system.
•Maximum two letters of support from stakeholders in the SQF system. All references shall include the contact name, address and phone number for verification purposes.
Other considerations may include:
•ActivitiesoutsideSQFpromotingfoodsafety,suchasworkwithGFSI,regulatorybodies,or other stakeholders.
