General notes on the use of Forms Leaders should complete the Kairos Information on the forms prior to distributing the forms.
Each region will have their own unique requirements for the information they collect. A process on how the forms are used and information provided for the database should be determined by each region. This should meet the KPMA requirements.
The use of forms should be incorporated into the Kairos training program. This will enable the team members to better understand what and how information is used.
During team training, the leader should work through the form with the team member completing the form. This provides an opportunity to clarify and discuss any ambiguities.

ThankQ DataBase: The Kairos Volunteer Management System Overview To aid the collection and responsible use of information collected through the various arms of Kairos (Kairos Outside, Kairos Inside, and Torch) a review was done on all the Kairos forms used to capture team and participants information. This document:

SECTION 1
1.1 Guidelines on the management of forms and information
Contact the Leader / Committee for the information. Not all the required information is on one form. Some information like team positions, talks etc are part of the records the Leader creates.
It is best to go through the form explaining it while it is being completed. Discuss any uncertainties. Check with regional committee as to procedures and information needed.
Pre-populate the Kairos (orange boxes) information before handing out the form for others to complete
Most of the Team information is recorded on the “Team Member Information” form.
Most of the participant / guest information is recorded in the “Privacy Policy & Database information” form. Note that these forms include a signature as agreement to have the information recorded in the database. These forms are scanned and stored in the Database.
Treat information confidentially and respect the contact and personal information that you have been intrusted to.
Store forms in a safe and secure place (don’t leave it lying around for others to see. Don’t lose it)
Enter or revise the information as soon as possible in the database. The sooner the better.
Destroy the forms responsibly once the information has been entered and you have checked that it is correct. Inform the Committee when this is done and the process complete.
In cases where a person is active in more than one region; a request can be made to Kairos Support Office to make arrangements for the person’s details to be available to the other region/s by proving them access to this persons records in the database. For cross-regional visibility to be set up, both/all the regions have to agree that that the data can be shared. This procedure only relates to team members and not guests/participants. Refer to the: ‘Procedure to enable contact information to be shared across regions’
Comply to the KPMA Privacy Policy: Kairos Prison Ministry Australia will comply with the requirements of the Privacy Laws. The information provided by you will be held in a secure database under the control of Kairos Prison Ministry Australia (KPMA). This information will be used for the primary purpose of informing you of programs organised by KPMA and associated administrative functions. It will also be used to inform you of activities, events and general information about KPMA. Except as required by law, your information will not be disclosed to any other person or organisation. General Kairos Forms (used across all arms of Kairos)
KGEN-4001 Clergy/Chaplain’s Expression of Interest This form is designed to capture information relating to persons who are interested in becoming involved with Kairos through being clergy or chaplains on teams. It is only required for potential clergy/chaplains and should not be completed by persons who have previously filled such positions on team. The following extracts are from the respective Manuals: Kairos Outside for Women Manual Page 60:
KOW – head chaplain needs to be ordained (or denominational equivalent)
Certified by an approved theological college and endorsed by the Regional Committee
Other Chaplains can be: Roman Catholic Nun / hold a formal position that requires theological training / be licensed/certified and active in pastoral counselling / have at least one year of full time theological training with a focus on pastoral care of counselling and current relevant experience.
Kairos Inside – Red Manual – KPMA 9/12 II-25 to II-28 Persons who serve as clergy on Kairos teams should meet the following qualifications: 1. a. Have received formal training by their respective denominations and be officially ordained or
licensed, or b. Be recognised as ministers of good standing, having approved theological training and being
actively engaged in, or retired from, full or part-time involvement in Christian ministry, and 2. Be able to work effectively in an ecumenical environment, and 3. Personally accept and affirm the Kairos Statement of Faith (viz. the Nicene Creed.) The Kairos Short Course Manual provides more detail in Section II
KGEN-4002 Contact Information & Agreement The purpose of the Contact Information & Agreement is to capture general information of any person who may be interested in Kairos Prison Ministry Australia, whether that is as a potential guest, team member, and general volunteer or to receive further information. If a person has completed an insert to a brochure then that can be either attached to the form or the information transferred to it. The form can be used to assist with entry of the information into the KPMA Database as well as the tracking of what has happened since the enquiry was received. The Contact Information & Agreement Form can be handed out at any Kairos event or promotional opportunity to enable interested people to be contacted. The signature is to ensure that the person is aware of the recording of their information on the KPMA database. Note that where a person completes an insert to a brochure then this could be taken as their giving approval for their details, as provided, to be recorded within the KPMA Database.
