1st Accident
Date: 1st June 2006 at 9.00 hrsTotal Hours of interruption: 648 Location : at Adchini junction Affected section of pipe line : Sarojini Nagar flyover to Laddo Sarai CNG StationAffected Length: 6.8 kmDiameter: 6’’Reason : 3rd party Damage( by D.K Enterprises on (contractor of BSES) during laying of cable).
Cause : Negligence of BSES, Reamer of HDD Machine hit steel line due to trenchless activities.

Cause : pipe line get punctured due to piling work, negligent effect of DDA authorities, securities had made aware about location prior to 4 days of damage.

3rd Accident
Cause : Due to Dredging work during cleaning of drain by JCB.
There was gas leakage. No. of CNG Stations affected:1 No. of DTC Depot affected :0 No. of R.O affected :0 No. of FRS affected :0 Remedies : The damaged portion was being
at the depth of approx 7 m inside the flowing drain. Repair was not possible at all.
For this an alternate line was passed.

Cause : Wrong offsets in drawing and locator couldn’t locate because of its depth(13 m)
according to drawing, offset of line was 11 m, clearance was provided to the contractor 17 m, piling activity was done at 23 m.

Following are the reasons for the Damages:
3rd Party damage due to excavation by DTL, DDA, PWD, DJB etc.
Operational Errors like improper working of locator, not to check drawings etc.
Un controlled errors / time independent like puncture of dielectric material, due to electric surge.

Reasons of Damages
To Rectify operational errors Calibration of the equipments to be done on
timely basis.( not followed till now) Check the certificates of HDD operator and
Tracker. All pipeline fittings and accessories should
be checked and operational life time of all should be decided.
Check drawings and locator both during the construction work.
Suggestions
Drawings should be properly checked by zonal in charge after laying of steel line by contractor.
The hydrostatically tested pipes should be retested by nitrogen gas before charging the line.
Protective measures from Electric installation( Neoprene sheets, RCC blocks, Minimum distance should be maintained).
Proper communication with BSES and other agencies.
Uncontrolled/Time independent error
Proper patrolling of pipeline by patrolling guard. Reporting by patrolling guard is very important,
train and test patrolling guard. Intimation should be given to the various
authorities like DJB , DTL, BSES, MCD, DDA, etc prior to construction work.
Depute IGL representative on 3rd party work specifically on HDD REAMER,PILLING, JCB, BORING.
Markers should be properly maintained near steel gas line with contact no. provided so that 3rd party could contact before excavation.
