Navigating the Future Course of Case Management has implications for the role of care coordination regardless of the setting in which you practice. In the six years of transformation since the Affordable Care Act legislation, long overdue emphasis on care coordination has occurred. More attention is focused on improving patient transitions from one care setting to another, reducing avoidable hospital readmissions, improving communities and focusing on greater patient and family engagement with the care team. We also have encountered many issues that get care coordination “off course.” These challenges include silos in providing care, electronic record variability and lack of connectivity, lack of communication across levels of care, increasing numbers of individuals providing “case management” services, and inconsistencies in carrying out the case management model of care. Policy and regulations continue to impact healthcare and the role of care coordination for case managers. How will case managers navigate the future to assure favorable outcomes for patients transitioning from one clinical environment to another or to the home? How are the standards of case management practice carried out for individuals with the title of case manager? What is in store in the future for case management professionals knowing the Center for Medicare and Medicaid requirements for value and outcome based payments? How will you navigate the future of case management and “stay on course”? At the 2016 Case Management Conference healthcare professionals will learn strategies to guide the future course of case management from national, regional, and local experts. Leading experts will present current evidence based findings and practice strategies to manage chronic conditions in older adults. Presentations will focus attention on building capacity for emotional resilience useful personally and professionally. Ethical issues in case management will be explored with specific strategies to reduce fragmentation of care that result when family values and beliefs conflict with professional ethics and evidence based care. Strategies to improve medication adherence in patients with dual diagnoses of behavioral and physical health needs will be addressed. Participants will engage in strategies for care coordination to address the epidemic of opiate overuse in our communities. Cheri Lattimer, RN, BSN is the Executive Director of the Case Management Society of America (CMSA), Secretary and Director of Utilization review Accreditation Committee (UrAC), President and owner of Integrity Consulting, and President of Case Management, Inc., the association management firm and education provider for CMSA. She currently leads the national

Transitions of Care Coalition (nAToCC). With over 30 years of experience in the healthcare industry she has held positions at various levels of care delivery. In 2005 she led CMSA in developing its extensive online Educational resource Library to support case managers with tools ranging from basic entry level to master’s level preparation. She serves on a variety of health care boards and is a national figure in the advancement of the care management integration movement.

Mark A. Weiner, MD works at Pain recovery Solutions in Ypsilanti, Michigan practicing full time Addiction Medicine, Internal Medicine and Pain Management since 2008. He is the Section Chief of Addiction Medicine at St Joseph Mercy Hospital Ann Arbor. He is the co-chair of the planning committee of the American Society of Addiction Medicine course, “Pain and

Addiction: Common Threads.” He is currently a member of the planning committee for the American Society of Addiction Medicine Annual Meeting. He is an editor of the upcoming ASAM Pain and Addiction Handbook planned for publication in 2016. He is a member of the Board of Trustees of Dawn farm Treatment Centers as vice chair and is the chairman of the Treatment Committee. Dr Weiner received his undergraduate degree (B.S. Psychology) at the University of Michigan, Ann Arbor. He earned his medical degree at Cornell University Medical College in new York City. He completed his internship and residency in Internal Medicine at northwestern University Mcgaw Medical Campus in Chicago, Illinois. He is board certified in Internal Medicine and Addiction Medicine.

Dr. Weiner has been a member of the American Society of Addiction Medicine since 2003 and has been active in the Michigan chapter (MISAM).

interprofessional collaboration and community resources useful to assist older adults in self-managing difficult chronic health conditions to maximize quality of life. Application of the principles learned will be completed through case study analysis.

This session will explore the human capacity of emotional resiliency and its usefulness as an application for case manager’s personal health and as a tool to optimize emotional health of patients in practice.

C: Choppy Waters: Ethical icebergs in Case Management Thomas L. Moore, MS, MA, LMSW, LLP, CAADC, MAC, CCS

This session will encompass several ethical issues inherent in case management practice, using case studies to apply ethical practice standards and principles and avoid potential problems of fragmented care while delivering client centered care.

D: Medication Adherence in Patients with Dual Diagnoses Mark Lowis, MSW, LMSW

This session will focus on the steps useful in applying strength-based strategies and motivational interventions to positively affect medication adherence in patients with dual diagnoses (behavioral health and physical health conditions).
