AIRLINE CAREERS BEGIN HERE

HARLINGEN, TEX-AS – Elena Marin, M.D., Chief Executive Officer for Su Clinica, today an-nounced that Su Clinica’s Brownsville and Ray-mondville clinics have received recognition from the National Committee for Quality Assurance (NCQA) Patient Cen-tered Medical Home 2011 (PCMH 2011) program for using evidence-based, patient-centered processes that focus on highly coor-dinated care and long-term relationships.

“I am so proud and honored to receive Rec-ognition from NCQA,” stated Dr. Marin. “The NCQA Patient-Centered Medical Home 2011 stan-dards emphasize the use of systematic, patient-centered, coordinated care that supports access, com-munication, and patient involvement. What I have learned is that the quest to optimize health for our patients and for our com-munity is a continuous

effort that never ends. This recognition underscores our commitment to that effort,” she added.

The patient-centered medical home is a model of care emphasizing care coordination and com-munication to transform primary care into what patients want it to be. Re-search shows that medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ reported experience with care. The NCQA PCMH Recogni-tion program identifies practices that promote partnerships between in-dividual patients and their personal clinicians, rather than treating patient care as a sum of episodic office visits. Each patient’s care is delivered by clinician-led care teams that provide comprehensive health care needs and coordinate treat-ments across the health care system. Medical home clinicians demonstrate the benchmarks of patient-

centered care, including open scheduling, expanded hours, and appropriate use of proven health informa-tion systems.

“The patient-centered medical home raises the bar in defining high-qual-ity care by emphasizing access, health information technology and partner-ships between clinicians and patients,” said NCQA President Margaret E. O’Kane. “PCMH Recogni-tion shows that Su Clinica has the tools, systems and resources to provide their patients with the right care at the right time.”

To receive recognition, which is valid for three years, Su Clinica demon-strated the ability to meet the program’s key ele-ments, embodying char-acteristics of the medical home. Standards align with the joint principles of the patient-centered medical home established with the American College of Physicians, the Ameri-can Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.
