I, the undersigned, being duly sworn and under oath, state that the following facts known by me or told to me by the other

reliable persons form my belief that jackie D. Smith the defendant committed the crime charged.

On Monday ; May 26, 2014 at approximately 2250 I was dispatched to the residence of 623 S . 9 th Street for a complaint of anintoxicated individual causing , a disturbance . Whi le approaching the residence, I observed two individuals, one male and one femalestandinqon in the street at 9 & Scha ller Street engaging in a physical altercation. I parked and exited my vehicle near them and couldhear both subjects yelling at one another loudly . As I approached the subjects on foot , they turned and began walking south. I stoppedthem on the sidewalk in the 600 block of south 9 th Street . The female identif ied herself as Jackie Smith of Lake City . She was placedunder arrest and transported to the Sac County Law Enforcement Center where she was charged with Disorderly Conduct and jailed .

She was placed under arrest andtransported to the Sac County Law Enforcement Center . At the jail she consented to a breath test and the result was .159 BAC . Sheshe was charged with Public Intoxication and jailed .
