    Arthritis

    Arthritis is a disorder that causes pain, stiffness andswelling in or around joints. Cartilage is a proteinsubstance that serves as a "cushion" between the bonesof the joints. Arthritis is caused by the breakdown and

    eventual loss of the cartilage of one or more joints.

    Causes

    There are many causes of arthritis. Heredity plays a majorrole. Some of these includes bacterial and viralinfections, traumatic injuries etc.

    Osteoarthritis

    There are many different types of arthritis. Themost common type is called Osteoarthritis.

    Osteoarthritis causes excessive strain and the

    wearing away of cartilage in the joints of thefoot.

    Movement becomes very difficult and painful.

    The pain and swelling worsens while standingor walking, and stiffness usually occurs afterperiods of rest.

    Rheumatoid arthritis

    Another type of arthritis is Rheumatoid arthritis.

    It can cause severe deformities of the joints with

    associated fatigue of the entire body.

    It can develop at any age and there is no known

    cause for this condition.

    Rheumatoid arthritis is the most crippling (severe

    and disabling damage) form of the disease that canaffect people of all ages.

    Alkylosing Spondilitis

    Ankylosing spondylitis is an inflammatorydisease that can cause some of the vertebrae inyour spine to fuse together.

    This fusing makes the spine less flexible and canresult in a hunched-forward posture.

    Mechanism of Action

    From the phospholipasesof cell membrane . Arachidonic acid

    is released. This intermediate arachidonic acid is created

    from phospholipase-A2. This arachidonic acid goes by the

    cyclooxygenase pathway and produces prostaglandin,

    thromboxaneand prostacyclin.

    Painkil block the cyclooxygenase pathway and stoparachidonic acid to produce prostaglandin. It has more

    selectivity in Cox-2 than Cox-1.

    Dosage and Administration

    Adults: The recommended dose is 100 mg twicedaily.

    Children: There are no clinical data on the use ofAceclofenac in children under 18 years of age.

    Elderly: The pharmacokinetics of Aceclofenac isnot altered in elderly patients, therefore it is notconsidered necessary to modify the dose or dosefrequency.

    Terminology

    Cartilage is a flexible connective tissue found in many areas in the bodies

    of humans and other animals, including the joints between bones, the rib

    cage, the ear, the nose, the bronchial tubesand the intervertebral discs. It

    is not as hard and rigid as bonebut is stiffer and less flexible than muscle

    Prostaglandins are found in most tissues and organs. prostaglandins are

    produced at the site of inflammation where it is believed that they

    mediate many of symptoms of inflammation such as oedema and pain.

    Prostacyclin (also called prostaglandin I2 or PGI2)is prostaglandin member of the family. It inhibits platelet activationand is also an effective vasodilator.

    Thromboxaneis a vasoconstrictorand a potent hypertensive agent, and it

    facilitates platelet aggregation.
