EXECUTIVE SUMMARY

The United States Agency for International Development (USAID)-funded Leadership, Management and Governance/Haiti (LMG/Haiti) field support project, implemented by Management Sciences for Health (MSH) in partnership with the World Bank and under the guidance of the Ministère de la Santé Publique et de la Population (MSPP), has been working in Haiti since August 2012 to generate inspired leadership, sound management, and transparent governance for stronger health teams, services, organizations, and healthier Haitians. The project’s four objectives are to:

1. Strengthen the capacity of the MSPP contracting function to manage all sources of funding (including United States Government [USG]) to improve the quality of and access to health services

2. Strengthen the MSPP's capacity to better regulate, manage, and monitor the health system 3. Strengthen the leadership, management, and governance (L+M+G) capacity of the

Country Coordinating Mechanism (CCM) to improve financial oversight and accountability as well as improve performance and coordination

4. Strengthen the strategic communications capacity of the MSPP and local Haitian journalists, and support USAID/Haiti to engage, inform, and elevate awareness of the Haitian public, diaspora, and US-based policymakers on key health issues for the country

This report describes LMG/Haiti achievements from July-September 2017, the final quarter of the LMG/Haiti project. The majority of the technical activities planned were completed, including (1) conducting one final results-based financing (RBF) monitoring visit, (2) printing key MSPP documents developed with assistance from LMG/Haiti (health facility authorization and accreditation manual, Unité de Coordination des Programmes [UCP] coordination manual, the MSPP Emergency Communication Plan, and the Guide for Internal Communication), (3) assisted the CCM Haiti and the Principal Recipient (PR) to complete the first phase of the 2018-2020 HIV/TB grant making cycle, and (4) developing 2018-2020 action plans for the CCM civil society sector. Project staff focused on completing close out activities, including conducting the end-of-project event, and finalizing technical deliverables.

I. PROJECT PERFORMANCE

During the fourth quarter of project year five (PY5), LMG/Haiti continued to strengthen Haiti’s health sector by making progress towards achieving several targets in the USAID-approved PY5 performance monitoring plan (PMP), which is in Annex A. Below is a summary of select achievements. Objective 1: Strengthen the capacity of the MSPP contracting function to manage all sources of funding (including US Government [USG]) to improve the quality of and access to health services Indicator 6: Number of RBF monitoring visits conducted The Unité de Contractualisation (UC) conducted one monitoring visit to evaluate RBF implementation progress in the Nord department from August 7-8, 2017. The project exceeded the target number of visits conducted (17 out of the 16 planned visits, a 106% achievement rate). Objective 2: Strengthen the MSPP's capacity to better regulate, manage, and monitor the health system Indicator 25: Number of organizational capacity assessment action plans implemented by central directorates The main expected outcome in the UCP’s organizational capacity assessment action plan to reinforce the collaboration between the three programs (HIV, TB, and malaria) was to develop and print the UCP coordination manual. LMG/Haiti finalized the printing of this document during the quarter, enabling the UCP to fully implement its action plan. The project achieved 50% of its final target (three actions plans implemented out of six). The project was not able to achieve its final target because the UCP and the three priority health programs had competing priorities (such as participating in the development of the HIV/TB and malaria concept notes for the Global Fund).

Transition project activities to the Health Finance and Governance (HFG) project

Completed LMG/Haiti facilitated the transition of select project activities to the HFG project by sharing information and conducting technical meetings with HFG technical advisors. Conducted the final

Completed On August 29, 2017, LMG/Haiti technical team leaders and the Project Director held a final technical meeting with USAID to present a summary of the results over the last five years and the project’s sustainability plan.

LMG/Haiti end of project event

Completed LMG/Haiti organized an end-of-project event on September 13, 2017, to present key achievements from the project. This event was attended by almost 80 invitees to celebrate the five years of implementation of LMG/Haiti. Presenters provided strong evidence of their satisfaction with the assistance provided by USAID/Haiti through the project. Representatives from each partner (MSPP, CCM, Réseau Haïtien des Journalistes en Santé [RHJS]) spoke eloquently about the positive results achieved in the health system and gratefully acknowledged the financial and technical support from USAID, through LMG/Haiti, to an audience that included USAID, non-governmental organizations (NGOs), international organizations, MSPP, and CCM representatives.

III. CURRENT PROJECT ACTIVITIES

LMG/Haiti continued to work closely with the MSPP at the central level, the CCM, and other partners to finalize the project’s deliverables and complete close-out activities. Table 2 summarizes the status of accomplishments during the quarter. Table 2: Key project activities

During the visit, the team held a meeting with department-level staff to discuss the issue of health facilities that do not have bank accounts. The UC will identify all RBF health facilities in departments implementing RBF that are facing the same problem and follow up with the Ministère de l'Économie et des Finances (MEF) to resolve the issue.

KEY ACHIEVEMENTS

In addition to the activities mentioned in the table above, LMG/Haiti completed the following activities during the quarter: Objective 2: Strengthen the MSPP's capacity to better regulate, manage, and monitor the health system

Provided technical assistance to the DOSS to continue PES implementation by initiating the process of estimating pilot costs in the three departments and identifying equipment needs within these networks.

Provided technical assistance to the HIV/TB committee by participating in technical meetings and providing tools to help them better manage the programs, such as templates for field visit reports.

LMG/Haiti assisted the UCP Executive Directorate to draft the preface for the coordination manual and obtain the Minister’s signature. The project printed 200 copies of the finalized manual and gave them to the UCP for distribution and use with their staff and partners.

Objective 3: Strengthen the L+M+G capacity of the CCM to improve financial oversight and accountability as well as improve performance and coordination

Program continuation cycle for the 2018-2020 HIV/TB and malaria grants: LMG/Haiti supported the CCM-Haiti and the Principal Recipient (PR) to complete the first phase of the 2018-2020 HIV/TB grant making cycle by providing technical assistance to develop all programmatic and budgetary details for the funding request, respond to requests

for clarification from the Global Fund Secretariat, and reach agreement with the Global Fund Secretariat on the final grant budget. This first phase of the grant making was terminated on July 20, 2017. The CCM and the PR left for Geneva on August 20, 2017, to finalize the grant making process at the Global Fund headquarters.

2018-2020 action plans for the CCM civil society sector: The CCM requested LMG/Haiti’s technical and financial assistance in developing the 2018-2020 civil society action plan during a two-day workshop held July 26-27, 2017, in Port-au-Prince. Seven civil society constituencies participated to the workshop (representatives from people living with HIV [PLWH], women, youth, people with malaria, lesbian, gay, bisexual, transgender, and intersex [LGBTI], the press, and trade unions) along with technical experts from UNAIDS and the Pan American Health Organization (PAHO)/World Health Organization (WHO). LMG/Haiti advisors facilitated the workshop and assisted working groups.

Indicator 9: Percentage of health facilities that have updated data available on the RBF database each quarter

Definition/Description: The MSPP has developed the RBF invoicing database. LMG/Haiti, as a member of the RBF Technical Working Group (TWG), will work with the TWG to upload the RBF data each quarter to the invoicing database. RBF data will be available each quarter after the invoicing database is established.

•Numerator= number of health facilities that provided updated RBF data each quarter

•Denominator= total number of health facilities implementing the RBF strategy and for which data have been verified.

Final project target achieved. Eighty-one (100%) health facilities that are implementing the RBF strategy (seven pilot sites + 74 new sites) have available data that was verified during this quarter and uploaded to the RBF database. The UC sent all payment requests to both the Direction de l'Administration et du Budget (for USAID-supported sites) and to the Unité de Gestion des Projets (for World Bank and Global Fund

supported sites).

Indicator 7: Number of department-level directorates implementing the RBF strategy according to established procedures

Definition/Description: LMG/Haiti supports the UC to build the capacity of the department level directorates to implement the RBF strategy according to established procedures. This indicator will be measured during monitoring visits by assessing progress made towards achieving the targets in department level directorate business plans and all other relevant RBF tools according to the RBF operational manual. However, the indicator is

dependent upon USAID and World Bank scale up of RBF to other departments.

Final project target achieved. A total of seven departmental directorates (Nord, Nord-Est, Nord-Ouest, Sud, Centre, Nippes, and Grande-Anse) are implementing the RBF strategy.

Indicator 8: Number of health facilities implementing the RBF strategy according to established procedures

Definition/Description: LMG/Haiti supports the UC to build the capacity of the seven pilot health facilities to implement the RBF strategy according to established procedures. Definition/Description: LMG/Haiti collaborated with the World Bank and UC to conduct the RBF baseline assessment in PY4. In PY5, LMG/Haiti will provide technical and financial support to the UC to produce the RBF baseline assessment final report, organize a workshop to share the key findings, and print hard copies of the report for dissemination during the workshop. Workshop attendees include MSPP authorities and staff from all

departments, health system representatives, and RBF stakeholders in Haiti (WB, USAID, WHO, Canadian cooperation, UNICEF, Global Fund, etc.).

Definition/Description: The RBF impact evaluation is expected in FY17. LMG/Haiti will provide support to UC to prepare key documents for final evaluation (research protocol, consultant scope of work and contract template, timeline, budget)

Final project target not achieved. The UC/MSPP and the principal investigator informed LMG/Haiti during PY5Q3 that they decided to lead the RBF impact evaluation after October 2018 and will review the key documents for the final evaluation shortly before they are used. However, during the RBF baseline assessment dissemination workshop, participants provided inputs and observations which the UC noted and will

take into account when reviewing the final evaluation tools.

Indicator 21: Number of meetings conducted by the UCP to monitor programmatic indicators

Definition/Description: LMG/Haiti will provide financial and technical support to the UCP M&E and technical teams to conduct quarterly meetings to monitor and evaluate the programs results and report findings using the dashboards that will be developed.

The project achieved 67% of its final target (2 meetings conducted out of 3). The project was not able to meet its final target because all three programs had conflicting schedules and competing priorities (in particular, the development of the HIV/TB and malaria concept notes for the Global Fund), and it was challenging to schedule meetings when all three programs were available. However, both HIV and TB

programs were able to simultaneously present their programmatic indicator results during the first national HIV/TB workshop conducted in May 2017.

Definition/Description: LMG/Haiti will conduct organizational capacity assessments using the Management and Organizational Sustainability Tool (MOST) tool to identify and strengthen gaps in the management capacity of organizations. The process increases the MSPP staff's ownership of their directorate's mission, objectives and values through a participatory approach.

Indicator 23: Number of coordination meetings conducted by the UCP with the three priority health programs

Definition/Description: LMG/Haiti will support the UCP to conduct monthly coordination meetings with the leadership of the three national programs to monitor regular activities and discuss strategies for improving the integrated management of the programs.

The project achieved 80% of its final target (12 meetings conducted out of 15). Coordination meetings require the participation of a coordinator from each priority health program and the UCP Executive Director. During the period of transition in the Haitian Government (February 2016), the UCP Executive Director played a key role within the Minister's Cabinet and was not always available to participate in these

meetings. Additionally, the priority health program coordinators had limited availability due to their participation in developing the Global Fund concept notes.

While the project had initially achieved its final target during PY3, in PY5 LMG/Haiti provided technical assistance and logistical support to the CCM to develop the 2018-2020 malaria program continuation funding request for the Global Fund. Targets for this indicator were not included in the approved PY5 PMP, as the CCM only requested LMG/Haiti’s assistance in December 2016. The

application was submitted to the Global Fund on March 27, 2017, and approved on May 16, 2017. The project achieved 59% (67 out of 114) of its final target. The project was not able to achieve its final target. Most of stories to be posted on the MSPP website were supposed to be written by the Réseau Haitian des Journalists de Santé (RHJS) journalists enrolled in the training session organized by LMG/Haiti during PY4. The journalists developed these stories, but they were posted through various media sources and

not published on the MSPP's website. Moreover, starting in July 2016, the LMG/Haiti Communication Advisor was no longer embedded in the MSPP to develop additional stories to post on the MSPP website.
