to a paperless system has improved its delivery. For instance, health outcomes have been improved through the use of automated alerts, which promote the adoption of recommended practices and prevent possible adverse events. Clinicians can also access reference information and evidence at the bed-side, thus resulting in more informed, quicker decisions. Technology is not just recording care; it is being used to improve it.

Full digitalisation has also improved patient safety. By tackling the duplica-tion of data, we have reduced errors caused by inconsistencies. The automa-tion of the complete medication loop, from prescription to administration, has rationalised our use of medicine and reduced dispensing mistakes.

Paperless skeptics often point to the cost of implementing technology. It is not an unreasonable concern. In dif-ficult economic times, hospital man-agers across Europe face constrained budgets and pressure to do more with less. However, by making the paperless vision a reality, we have delivered ef-ficiency savings to reinvest in care. For instance, by rationalising workflows we have freed up nearly 8,000 physician and nursing hours a year. Millennium has also streamlined bed management, thus reducing average stays by 10%and increasing occupancy rates. At a time of constrained budgets, these are savings we cannot afford not to make. Austerity does not preclude investing in technology, it necessitates it.

The transition to HIMSS Stage 7 is an ambitious task. However, the experi-ence of the Hospital Marina Salud de Dénia in Spain should encourage col-leagues across Europe. Rapid digitili-sation is possible. Success hinges on viewing technology not as the end ob-jective, but as the tool for transform-ing the delivery of care. It should be seen as the heart of efforts to improve outcomes, strengthen safety and sav-ings. Achieving paperless status may be difficult, but our experience proves it is worth striving for.

Too often digitalisation projects begin with a timetable. Important strategies and decisions end up being driven by deadlines. However, at the Hospital Ma-rina Salud de Dénia, our starting point was not a delivery deadline, but an understanding that technology could improve the quality and affordability of our care. Therefore, digitalisation was not an objective in our strategy for improvement; it was the mechanism underpinning it.

When faced with the long path towards digitalisation, my colleagues across Eu-rope frequently confess to being appre-hensive about the scale of the task. This is understandable; hospitals are com-plex organisations and transforming their operations whilst maintaining high standards is a difficult challenge. Doing so to a tight timetable only increases the pressure. However, our experience in Spain proves that it is possible. Our Cerner Millennium electronic health record system went live in January 2009, just fourteen months after we began the digitalisation process. We then completed the journey to HIMSS Stage 7 in 2012. Now information is not only digitally accessible across our 209-bed hospital, but throughout our broader network of medical centres and practices, which cover 150,000 pa-tients—and double that amount during the peak tourist season.

Each hospital is different, and every journey to becoming paperless brings unique challenges. However, there are valuable lessons from our experience that can be applied to different health sectors across Europe. The first is to make the whys and wherefores the cen-

tre of the strategy in order to entrench a cultural understanding that tech-nology can help improve outcomes, improve patient safety and deliver ef-ficiencies that can be reinvested in care. This can only be achieved if clinicians are at the heart of the project, helping design a system that supports their workflow rather than obstructs it.

Large-scale change requires wide-spread support. Clinicians and patients are the best ambassadors for technol-ogy, but will only become so if they un-derstand what is being done and how it will benefit them. A relentless focus on getting the basics right is essential to building that support. Documents should be simple, training should be timely and attempts to add function-ality should never compromise basic system performance. Such work is not glamorous, but it is essential to building confidence in the system.

No matter how well planned the process, projects of this scale always encounter unforeseen problems. Marina Salud was not immune to this; our train-ing manuals were too complex, some new staff received training too late and the lack of remuneration for training limited engagement. Far from derail-ing the process, these issues merely underlined how vital it is to have staff support for the project. If clinicians and patients understand where they are go-ing and why, they are more resilient to teething problems along the way.

Some might question whether the outcome justifies the effort. Our experi-ence shows that far from distracting cli-nicians from delivering care, transition

Opportunities Uncovered for Driving CPOE Adoption in Germany

In addition to tracking IT adoption associated with over 50 software ap-plications throughout Europe, HIMSS Analytics Europe asks chief informa-tion officers and other IT profession-als at hospitals if they would recom-mend an installed product to a friend or colleague. The Net Promoter Score is their response to this question. The scale used for the response is 0 - 10, 0 representing “not at all likely” and 10 “extremely likely.”

When software application IT adop-tion rates were compared to product Net Promoter Scores throughout the countries in Europe, a correlation was uncovered. In Germany, where the Net Promoter Score for CPOE is low, CPOE market adoption is also low at less than 50% which suggests word of mouth negatively impacts CPOE adoption. In Spain, where the Net Promoter Score is high, CPOE adop-tion is also high at over 75% which suggests word of mouth has a posi-tive impact on CPOE adoption.

“These findings are exciting because they further illuminate drivers for IT adoption, and opportunities for sup-pliers to develop the necessary strat-egies to improve IT implementation and care delivery” says Uwe Buddrus, Managing Director of HIMSS Ana-

A low Net Promoter Score in Germany and its correlation to IT adoption suggests vendors de-velop marketing, sales, and product strategies that counter negative word of mouth to drive CPOE adoption. Given CPOE is a critical component of the HIMSS Analytics EMRAM, improved IT adoption in Germany has the potential to have a significant impact on its clinical outcomes.

Hospital Marina Salud de Dénia is truly paperless with no medical records on the nursing units. All physicians enter orders online and this is supported by an intelligent clini-cal decision support system. Staff document their clinical notes online where the latest results and impressions are instantly available for all care givers. The medical imaging department is completely digital, making all images across the hospital network available online. One of the key fac-tors which contributed to Hospital Marina Salud de Dénia’s Stage 7 achievement was its very sophisticated clinical and business intelligence program to analyze care quality and efficiency, making it a “data driven” organization.

Hospital Comarcal d’Inca is a secondary public hospital founded in 2007 and located on the island of Mallorca (Balearic Islands). With 165 beds and nearly 760 profes-sionals attending to roughly 117,000 patients, it handles 135,000 outpatients and 7,500 inpatients (discharges) each year. Hospital Comarcal d’Inca’s entire staff has been committed to the search for efficiency and productivity through new technologies and information systems. Most significant of these has been the implementation of a EMR that integrates all the hospital’s existing systems and devices; an alert system with automated warnings (which helps the medical staff in decision making), e-prescribing, potential drug interactions, automatic medicine dispensa-tion, bedside nurse help for medicine administration and a full PACS with all modalities.

The func-tional structure is based on multidisciplinary functional units, each case is studied by the corresponding functional unit and offer a treatment plan for each patient. Outstanding IT practices at Onkologikoa include self-developed closed loop medica-tion administration using Automated Dispensing Machines (ADM)s in pharmacy and at the ward, as well as bar coding, with strong clinical decision support especially for che-motherapy: there are several protocols for chemotherapy, so called ‘super sets’ for chemotherapy medication order-ing, including adjuvant meds, and alerts based on clinical documentation, e.g., duplicate checking for chemotherapy protocol related orders, some interaction with lab results to adjust drug dosage.
