Extensor digitorum longus
Action Extend the 2nd through 5th
toes, dorsiflex the ankle, evert the foot
Origin Proximal anterior shaft of
fibula and interosseous membrane
Insertion Middle and distal
phalanges of 2nd through 5th toes
Innervation Deep peroneal (L5-S1)
Extensor digitorum longus Supine
Palpating the distal tendon
Ask pt. to extend his toe
Follow the tendons toward ankle (converge into single bundle underneath extensor retinaculum)
Explore the belly b/w tibialis anterior and peroneal muscles
Test for extensor digitorum longus & brevis
Test for extensor digitorum longus & brevis
• Patient:Patient: Supine or sitting
• Fixation:Fixation: The examiner stabilize the foot in slight plantar flexion
• Test:Test: Extension of all joint of the 2nd through 5th digits
• Pressure:Pressure: Against the dorsal surface of the toes, in the direction of flexion
Extensor hallucis longus
Action Extend the 1st toe,
dorsiflex the ankle, invert the foot
Origin Middle anterior surface
of fibula and interosseus membrane
Insertion Distal phalange of 1st toe
Innervation Deep peroneal (L5-S1)

Ask pt. to extend his first toe
Follow the tendon toward the ankle
Test for extensor hallucis longus & brevis
Test for extensor hallucis longus & brevis
• Patient:Patient: supine or sitting
• Fixation:Fixation: The examiner stabilizes the foot in slight plantar flexion
• Test:Test: Extension of the metatarsophalangeal and interphalangeal joint of the great toe
• Pressure:Pressure: Against the dorsal surface of the distal and proximal phalanges of the great toe in the direction of flexion

Locate medial malleolus
Slide off the malleouls posteriorly and proximally into the space b/w the shaft of tibia and calcaneal tendon
Difficult isolate specific tendon

Test for flexor hallucis longus
• Patient:Patient: Supine or sitting
• Fixation:Fixation: The examiner stabilizes the metatarsophalangeal joint in a neutral position and maintains the ankle joint approximately midway between dorsal and plantar flexion
• Test:Test: Flexion of the interphalangeal joint of the great toe
• Pressure:Pressure: Against the plantar surface of the distal phalanx, in the direction of extension
