Why should I consider ACOG Group Term Life Insurance over another life insurance policy?ACOG cares for life, and that’s why we offer Group Term Life Insurance. It can be an ideal choice for individuals who have short-term financial obligations or young families on a tight budget. It provides straightforward, cost-effective coverage with rates that start low and increase in five-year age bands as you get older—allowing you to help protect your loved ones at a competitive rate!

Who is eligible for coverage?All ACOG members* and their lawful spouses who are under the age of 60 who are residents of the U.S. or Canada (except for residents of Quebec) may apply for this coverage. Your spouse may apply, even if you don’t!

How much coverage is available?You and your spouse may apply for $100,000 to $2,000,000 in coverage (in $10,000 multiples). The coverage amount in force at age 59 reduces to 80% at age 60, 60% at age 65, 40% at age 70. Premiums do not reduce.

How much coverage can I apply for?While every family’s life insurance needs are unique, many primary breadwinners should be insured for at least 10 times their annual income.* To help you figure out how much life insurance you and your family may need, we have provided you with our Life Insurance Needs Calculator at acogplans.com/TL. Remember, life insurance not only helps cover immediate expenses; it can also help safeguard long-term plans such as paying off the mortgage or sending your children to college.

Terminal Illness (Accelerated) Benefit. If you or your spouse are under age 75 and are diagnosed with a terminal illness with a life expectancy of 24 months or less, the terminal illness (accelerated) benefit would allow you to request an advanced payment in a lump sum of up to 75% of your in-force life insurance amount, and not more than $200,000.

All money received under this benefit is yours to spend as you see fit, whether you choose to pay for medical bills, take care of financial obligations, or simply maintain your quality of living. However, full premiums will continue to be payable and the amount received at the time of the insured’s death will be reduced by any payment made under this benefit. For additional details and limitations, please see the Certificate of Insurance. Please note that receipt of terminal illness (accelerated) benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult your personal tax advisor. Proof of the terminal illness must be certified by a licensed physician and in a form that is satisfactory to New York Life.

Details of this coverage30-Day Free Look. If you are not completely satisfied with the terms of your Certificate of Insurance, you may return it, without claim, within 30 days. Your coverage will be invalidated and you will receive a full refund—no questions asked!

Effective Date. Coverage will begin on the first day of the month following the date your application is approved by New York Life, provided the premium is paid when due. Insureds must be able to perform the normal duties of a person of like age in good health on the date insurance is to take effect. If these conditions are not met, such insurance will take effect on the date the person resumes such normal activities.

Beneficiary. You may select any person, persons, trust, or other legal entity as your beneficiary and as your spouse’s beneficiary. You may change your beneficiary, unless irrevocable, at any time, by submitting a written request on a form satisfactory to New York Life’s home office. Once the change is made to the record, it will take effect as of the date your written request is made.

Non-Tobacco User Discount. You may qualify for lower non-tobacco user rates, provided you have not used any tobacco or nicotine products during the last 24 months.

Limitations. If death is the result of suicide or an intentionally self-inflicted injury during the first two years of coverage, while sane or insane, benefits are limited to return of premiums paid. (Subject to state variations.)

Termination of Coverage. Your ACOG Group Term Life Insurance will continue until you reach age 75 or earlier if: 1) the policy ends; 2) the policy is amended to end insurance for your class; 3) at the end of the grace period for which the last premium has been paid; 4) the day before your amount of coverage, less any Accelerated Death Benefit paid, equals zero or less; or 5) you request to end coverage.

Conversion Option. Your Group Term Life Insurance coverage may be exchanged for an equal amount of individual life insurance, without any evidence of insurability required, under certain instances of involuntary termination of coverage, as described in your Certificate of Insurance. The option must be exercised within 31 days of the date your coverage terminates.
