Based on the assessment of prior learning, a broad written plan for the residency program shall be developed, setting forth goals, as well as a schedule of activities for achieving those goals. This plan should build on the resident’s strengths and address the areas for improvement.”

Standard 3.1 – Provide Direct Patient Care as a Member of Interprofessional TeamsThe resident shall be proficient in providing evidence-based direct patient care as a member of interprofessional teams

RangeResidents are expected to collaboratively manage simple drug related problems in patients with uncomplicated medical problems or psychosocial needs with minimal supervision from preceptors. For more complex drug related problems, or for patients with more complex medical problems or psychosocial needs, residents are expected to self-assess and iden-tify what supports are required to assist the resident in provision of patient care. Residents work collaboratively within in-terprofessional teams and recognize their roles, limitations, and responsibilities.

Standard 3.2 – Manage and Improve Medication Use SystemsThe resident shall demonstrate a working knowledge of medication use system(s) as well as pharmacy and other care provider roles within the system, in order to manage and improve medication use for individual patients and groups of pa-tients.

Residents are expected to understand and be able to effectively explain all aspects of the drug distribution and medication use systems in order to be able to optimize patient safety and patient care. Accordingly, residents apply their knowledge of medication management tools (such as formularies, automatic substitution policies, medical directives, etc) and thera-peutic strategies (such as therapeutic drug monitoring, drug utilization review, etc.) to ensure safe and effective use of medications within the health care organization. Residents utilize reporting systems (such as adverse drug reports or medication incident reporting) as vehicles to improve the quality of medication use within the health care organization or system.

1. The resident shall be able to relate the advantages and limitations of key components of the medication use system with respect to the patient, the de-partment and the organization, including but not limited to unit dose, tradi-tional system, computerized medication administration records, computerized physician order entry, clinical decision-support tools, bedside barcode admin-istration, and intravenous and/or oncology admixture services.

Residents are expected to apply their understanding of management principles (such as human resource management, continuous quality improvement, change management, and budget management) to ensure self-efficacy and effective team-based dynamics to optimize patient care. Residents are expected to demon-strate leadership skills within the residency program, the department, the organization, and/or the profession. Residents adhere to professional and ethical standards in working through complex situations.

RangeResidents are expected to accurately self-assess and respond to practice demands and practice-related learning needs, to ensure they are able to remain current throughout their professional careers. Residents are expected to demonstrate effective time- and resource-management skills to allow them to balance multiple competing demands while achieving agreed upon objectives. Residents are expected to apply normative standards to self- evaluate their contributions to pa-tient care and the profession. Residents work collaboratively and respectfully as part of pharmacy and interprofessional teams and adapt to changing/evolving circumstances of practice.

The resident shall use effective project management skills to undertake, conduct, and successfully complete a project re-lated to pharmacy practice.

RangeDuring the residency program, residents are expected to identify a practice-based problem or issue, generate a research proposal to address it, develop appropriate research methodologies, and demonstrate effective data gathering and analy-sis skills. The resident is expected to complete a written report in a format suitable for publication, and must be able to de-fend all elements of the project, from conception to execution to interpretation.

Based on reflection of your prior learning and your knowledge of the goals, objectives, and re-quired competencies of the residency program, identify a number of personal GOALS, OBJEC-TIVES, and/or FOCUS AREAS for your residency year. These should focus on areas you think require the most development and/or are of greatest interest to you.

Be as specific as possible with each goal/objective. Remember that ideally, these should be out-come-oriented and measurable. Consider HOW each will be achieved.

This list can be included in this document, or ideally, on a Page of your ePortfolio. It is open for continuous revision, progress reporting, and reflection throughout your residency year.

SAMPLEGOAL/OBJECTIVE: I will be able to consistently and efficiently (eg, within 2 hours) be able to de-velop a thorough (eg, including all the relevant drug-related issues) pharmaceutical care plan for a general medicine patient.

HOW TO REACH/ACHIEVE IT: Through practice, practice, practice on my clinical orientation and medicine rotations, with refinement on my other DPC rotations; through discussion with my resident colleagues, mentor(s), program coordinator, preceptors about “best practices”.
