Asper the Operating Schedule which has been completed at Box M on the application, thefollowing conditions will beendorsed onto the premises licence so thatthey are clear and unambiguous for the licensee, members of the public, Police and Councillors. 1- A registerofall refusedsalesshallbekeptatthepremisesandarecordofeveryrefusedsaleshal be entered into the register showing the date and time of refusal, details of the product and the individual refused andthe name of the person who refused it. Theregister shall be kepton the premises at all times and made available for inspection by Officers of the Licensing Authority or Trading Standards. 2. Allpersons engaged in the sale of alcohol, before being allowed to make sales of alcohol and at not more than 6 month intervals, shall undergo training in: a. The procedures used forcheckingthat purchasers ofalcoholare overthe age of18. Thisshall include the use of nationally approved proofofage schemes such as the Portman Group Pass card or theCitizen Card. b. The legal requirements under the Licensing Acl2003 relating to sales of alcohol to persons under the age of 18 including sales by proxy. c. The operation of a Challenge 21 policy and keeping of a refusal register book A written record of such training shall be maintained bythe licensee and shall bemade available promptly on request to Officers ofthe Licensing Authority or Trading Standards. Such records shallinclude details of the names of staff trained, date of trainin8 and a summary ofthe matters covered in ihe training. 3. A Challenge 21 Policy shall be implemented at the premises and willinclude the display of signs at point of sale, in addition to the above stafftraining. 4. The premises shall ensure that glassware or open containers do not leave the premises or any outside Sealrng area, Furlhermore, I have been made aware fromPC Steve Pellowe that the area at Leisure West does have some problems associated withASB, however, the conditions offered in the Operating Schedule (as administered above) do promote the Tice nsing objectives and therefore ldo not have any recommendations at this stage otherthan for your client to give consideration to the hours for thesale of alcoholand the instailation of a CCTV System. I have recently visited the above premises location and I am satisf ied that there is cornplia.ce with the requirementfor a Public Notice.
