Achievements and Performance for the Year End March 2016

We are pleased to report that over the year: we provided day care services to a total of 1,517 Asian elders, with 77 frail Asian elders on individual

budgets receiving 19,250 hours of day care support 130 Asian elders benefited from over 700 hours of befriending support, provided at home and/or in

hospital, through our daycare and preventative work 45 regular health and well-being activities and 6 community events took place, benefiting both Asian

elders and older people from mixed communities, through our preventative work project, 100 Asian elders and carers received casework and advocacy support through our daycare,

preventative work and Carers’ projects through our intergenerational work, 20 Asian elders engaged with 45 young people at a Pumpkin

Event, a photo and video workshop, and volunteering, while 60 participants (children, Asian elders, parents, staff) celebrated Diwali together at a local primary school

95 Asian elders were given information and supported to give their views through language-based workshops at a consultation event in February 2016

through our dedicated older Carers’ work, we ran health information sessions and outreach events, support group meetings, and provided 1:1 advocacy and befriending support sessions benefiting 31 older Asian carers

other Carers’ work benefited 110 isolated Asian carers through health and well-being activities, peer support and intensive 1:1 support

23 events and trips took place including theatre outings, seaside trips, a railway outing, celebrations and Information Days

Recent achievements include securing 2 major grants for 3 and 5 years respectively. The former to work with older Asian Carers at critical transition points, the latter to work on encouraging Asian elders to “Join In and Keep Healthy” as well as breaking down barriers amongst different communities via partnership working with elders and community organisations. We have continued to review internal support and infrastructure systems in line with the changing needs of the people we work with. Developing robust in house training and support for staff.

We continued to update IT and systems, starting initial work on the website, and supporting Asian elders to use laptops and tablets e.g. for researching health information, for booking hospital and doctors’ appointments We also undertook a review of transport arrangements in light of the needs of very frail service users.

An average of 445 frail and isolated Asian elders benefited each week from Subco’s services.

Subco Trust provides a range of benefits and activities to the public as it seeks to improve the lives of people from diverse Asian communities who are residing in the UK and are experiencing need, hardship or distress. Objectives and Activities Subco was founded in 1993 to address the unmet needs of vulnerable Asian elders in Newham, one of the poorest areas of the UK. Since that date, we have pioneered innovative, culturally and linguistically appropriate services to meet their health needs.

Subco provides an integrated, flexible day opportunities programme, with an exciting and stimulating range of activities and support. This includes:

dementia services information, advice and advocacy intergenerational work health and well-being workshops events and trips support for family carers

In all these areas of work we seek to put Asian elders at the heart of our organisation and ensure their continuing involvement in policy development and delivery of services. The Asian elders we work with have a range of long-term health conditions, for example, diabetes, heart disease, stroke, memory loss and dementia, hearing loss, arthritis, depression, cancer, and often several of these conditions. With its friendly, experienced and qualified staff team, Subco now has a strong track record and many years’ experience of providing services to Asian elders with these complex long-term conditions and disabilities.

Subco has many working partnerships and referral routes in and out of all its services. We work closely with the First Avenue Resource Centre, Active Newham, West Ham Foundation Trust, Stay Well partnership, East London NHS Foundation Trust, Bart’s Health, Newham Clinical Commissioning Group, receive referrals from GP’s and health practitioners as well as Adult Social Care Services, London Borough of Newham, Compassionate Funerals, the Forum for Health & Wellbeing, The Challenge. Joint work undertaken includes making and receiving referrals, signposting, case assessments and reviews, strategic involvement. These partnerships and joint working help us to open up services for Asian elders, both enhancing our own services and improving access to others.

Future Developments

The focus for the year 2016-17 will be:

Through consultation, continuing to respond to the changing needs of service users and

their families to ensure appropriate services are developed

Continuing to incorporate Personalisation good practice into all our work

Through a programme of preventative work, delivering health and well-being activities as

a way of meeting the unmet needs of ‘younger Asian elders’

Working with other organisations, running health and well-being activities for older people

from mixed communities to break down barriers and challenge stereotypes

Developing intergenerational work through building on new and existing partnership work

with local organisations

Exploring funding streams to sustain and consolidate work with primary, secondary and

tertiary carers

Enabling SubCo to develop expertise in end-of-life care work through further exploring

partnership working with specialist agencies

Exploring ways of sustaining Newham’s Stay Well Partnership after funding from Newham

ended in March 2015.

Continuing to invest in staff training, developing our in house training programmes, as well

as accessing accredited external training for staff

Reviewing our current transport fleet in response to the changing needs of service users

Securing premises by renegotiating a long term lease with Newham Council
