Case Vessel agroundGeneral cargo vessel grounded in a fjord in Norway

• The vessel ran aground during her voyage from Norway to Germany • The chief officer fell asleep and failed to change the steering course • The lookout was not present on the bridge, because of a safety

round Human Error?

Page 10: Loss Prevention

Vessel agroundContributing factors

• Tight voyage schedule, frequent port calls

• Loading involving the entire crew

• Rest hour regulations not met (but filled out as required)

• Chief officer was alone on the bridge during voyage

• Signals that the officer was very tired were ignored

Accidents are seldom caused by a single person, but need to be seen in the context of the whole ship and company organization!
