1. Four Year Plan • Student completes with parent/guardian and teachers
using Course Selection Guide
2. Course Selection Form • Student completes form based on Four Year Plan• Student takes completed form to subject area teachers for
recommendation signatures • Student and parent/guardian discuss and sign• Return to Tiger Time teacher on or before February 16

Four Year Plan
• Rough draft of high school career
• Refer to the Course Selection Guide for detailed course and graduation requirements
• Due on or before February 16
Course Selection Form• Courses listed on front are required
for graduation
• Courses with an * require a teacher recommendation
• Elective choices are listed on back
• Alternate elective choices • Will be used as substitute if
original elective choice is unavailable
• List in order of preference
• If auditioning for a music program, list it as an elective – if not selected, your first alternate will be substituted
Course Placement Override
• If you disagree with an academic recommendation for your student, you may submit a Course Placement Override Form to override the teacher’s recommendation
• Course Placement Override Forms result in a binding agreement for the level requested
• We encourage you to speak with Mrs. Shafer before submitting the form
• Course Placement Override Forms are available from Mrs. Shafer
• The form should be returned with the Course Request Form

Transition of Math Course Names
● State of Ohio reorganized the required math standards

Physical Education Waiver
• Students may satisfy the requirements for 1/2 credit of Physical Education by participating and completing two seasons of school-recognized interscholastic athletics, marching band, or cheerleading
• Students will select an additional elective• Students will take Online Health
• During 10th grade• Done on own time – not during the school day
• The Physical Education Waiver/Substitution Form is available from Mr. Halcomb, Mr. Chmiel or the LMS circle desk
• The form should be returned with the Course Request Form
Summer PE
• Students may satisfy the requirements for 1/2 credit of Physical Education by participating in Summer PE
• Offered at LHS - sign up is available prior to last day of current school year
• Fee to participate
• Must still indicate PE/Health on Course Selection Form • Upon completion, an alternate class will be substituted (in order
of preference)

Ohio Department of Education State Testing Requirements
Students must reach at least one of the following pathways:
Option 1:Earn a cumulative score of 18 on the seven End of Course exams (AIR tests) in Math I and II or Algebra and Geometry, English 9 and 10, Biology, American History and American Government - students earn a score of 1-5
• Students need to score a minimum of four points in math, four points in English and six points across science and social studies
• Students will take the test near the end of the course• Students who take the course as an AP course their AP
score will be translated into graduation points• Students already enrolled in Math I or II as a middle
schooler will take these tests / have taken these tests and their score will count towards their graduation points
• If students do not reach the minimum score they may ret-ake any of these tests and their best score will count (there are 3 testing timelines each school year)
Options 2:
Students earn “remediation-free” scores on the ACT
All students will take a state-funded ACT in Spring of junior year
“Remediation-Free” score ODE sets the minimum*subject to change by ODE
Options 3:
Earn an approved industry credential and achieve a workforce readiness score on WorKKeys (i.e. Great Oaks Program)
