Life for a blind child is a dreary one and a nightmarefor the parents.

He can feel the warmth and hugs of family membersbut when he steps out into the unfriendly world, themocking laughter, the jokes and puns of childrenunderline his plight. He can feel the joy of a morningbreeze but the blowing wind has no answer to hisquestions of what will become of him when his parentsare gone. It is a tearful condition but happily not now.

Blindness is assuming scary proportions around the

world. In Pakistan, 11% of our population suffers

from blindness or impaired vision.

Since LRBT was conceived as an NGO, in 1984, it hastreated 16.5 million people both young and old inPakistan. Despite that, today, the figure of child blindnessis heartbreaking – 2.6 million children in Pakistan areeither blind or suffer from impaired vision.

Restoring vision in children is our sacred mission.

May God bless the two visionaries, GrahamLayton and Zaka Rahmatulla, whose dreamforesaw the shifting sands of time in Pakistan.

Growing up blind in the slums of Karachi is amiserable existence.

Ali Hamza’s case is a miraculous wonder ofmedical science, and the advancement of highquality eye-care which is available at LRBT,Pakistan.

When Ali was six, he started to lose vision in botheyes. When taken for a checkup to a local doctor,he was advised immediate surgery. Thedesperate father went to a local hospital whereLo and behold!, the cost of surgery wasRs.16,000 (US$200) – three times the salary ofhis father.

Sometimes, fate changes so fast, it isoverwhelming. Ali was brought to LRBT’s KorangiHospital in Karachi where surgery wasperformed in both eyes, one at a time withamazing results. After opening the bandages, Alicould see. For him, a miracle of advancedscience and technology had brought him a freegift of restored eyesight

2.6 million children in Pakistan are either blind or suffer fromimpaired vision. For some, their visual impairmentis so severe that they are unable to do very muchwithout assistance.

Paediatric Clinicsand Programmes

LRBT has established specialized paediatric eye-care clinics in 5 of our hospitals where skilleddoctors and sophisticated equipment is available:

I Korangi (Karachi), Sindh

I Lahore (Punjab)

I Lar (Punjab)

I Mandra (Punajb)

I Quetta (Balochistan)

LRBT is the largest provider of paediatric eye care in the country. According to anindependent estimate, it performs 32% ofpaediatric eye surgeries in Pakistan.

LRBT also runs school-screening programmes inKarachi, Quetta and Lahore to identify childrenwith eye problems and then refer them fortreatment to its nearest hospitals for appropriatetreatment. We have also embarked on our firstrural school-screening programme in Tehsil Sohawa, District Jehlum in northern Punjabwhich will be extended resources permitting.

There was great rejoicing in the Hakeem familywhen Abdul Rehman was born. He was going tobe their support in their old age, their insurancepolicy.

But the joy soon turned to sorrow : AbdulRehman was blind.

Abdul Hakeem took him to a doctor in Quettawho diagnosed congenital cataract andprescribed surgery at the cost of Rs. 20,000.

This was 5 times Abdul Hakeem's salary which heearned working at a shanty hotel in village KillyBadini, District Noshki (Balochistan).

Three years later a visiting friend advisedHakeem to take the child to LRBT's Eye Hospitalin Quetta.

The child was operated in one eye. Uponopening the bandage, Hakeem showed the boy a piece of roti (bread) which he grabbed.

A miracle had happened. Their only sonhad emerged from a life of perpetualdarkness. The sun was shining again.

A kind word, sentences of comfort comespontaneously. They are not rehearsed becausethey come straight from the heart. Such is thedegree of care.

I 31,354 students were screened under school-screening programmes, out of which,1,173 were refracted and 1,681 referred for detailed examination/surgeries.

I We crossed 2 milestones in this year treating our:

● 15th millionth patient Abdul Rahman, a small child from aremote village in Balochistan in September 2008

● 16 millionth patient: Ali Hamza, a young boy from theslums of Karachi in April 2009.

I 1.75 million Patients were treated in the Out PatientsDepartments (OPD) of our 16 hospitals and 39 clinics; agrowth of 11 % over last year. This is 36% or more than 1 out of every 3 eye patients visiting OPD in Pakistan

I 185,631 major and minor eye surgeries were performed, agrowth of 4% over last year. This is 27.4 % or more than 1out of every 4 eye surgeries in Pakistan

I 134 corneas were transplanted.

I LRBT continued to surpass the WHO/ICEH standards forgood quality cataract surgery

I Two new Secondary hospitals at Arifwala (Pakpattan) in thePunjab and Mansehra (NWFP) were established.

I 2 new Primary Eyecare Centers (PECs) in Punjab and AzadKashmir were opened.

I The capacity of Tando Bago (Sindh) hospital has beendoubled...

I LRBT was invited to participate in the Clinton Global Initiative(CGI) Asia meeting held at Hong Kong in December 2008.

I LRBT was nominated for 2009 Antonio Champalimaud award.The Champalimaud Foundation, based in Lisbon, Portugal,was created at the bequest of the late Portugueseindustrialist and entrepreneur, António de SommerChampalimaud. Leonor Beleza, a former Portuguese Ministerof Health, is the Foundation's President

I LRBT was also nominated for 2009 Gates Award for GlobalHealth. The Global Health Council is the world's largestmembership alliance dedicated to saving lives by improvinghealth throughout the world

I LRBT has been certified by Pakistan Centre for Philanthropy(PCP) for the period ending 30th October 2011. We havebeen awarded 892 marks (89.2%) as compared to 843(84.3%) in 2003. PCP is a body notified by Government ofPakistan to certify NPOs to be eligible for tax exemption.

I Obsolete and irreparable equipment was replaced in somehospitals. More sophisticated equipment was provided tosome. Equipment was also provided to our new hospitals.

Chairman’s Report We are very grateful to our aforementioned donors who have made this unprecedentedexpansion possible and which has now enabled virtually every Pakistani to be within a 3hour bus ride of an LRBT facility.

Sight Savers International (SSI) who have been our partners since we began our missionto eradicate curable blindness in Pakistan 24 years ago were our largest donor followedby the Graham Layton Trust in the UK and LRBT America. Punjab Welfare Trust Boardapart from contributing to the operating expenses of our hospitals located in Punjab havealso given us a grant for new equipment and expansion.

Standard Chartered Bank (SCB) continued their donation of US $ 100,000 as a part oftheir global ≈Seeing is Believing∆ campaign. They have also raised donations for us fromtheir customers in Pakistan..

Pakistan Bait-ul- Mal's (PBM) grants in aid declined to less than half over the previousyear. Central Zakat Administration (CZA) & Infaq Foundation maintained their donations atlast year's level. Infaq also donated an additional Rs 500,000 for our work amongst theIDP's. CZA have advised us that they will double their donation to Rs 12 million next year

The creative brilliance of Saadia Rauf, Imran Khan and Irfan Aamir at O2 Communications wasonce again on display in our highly acclaimed integrated Zakat campaign and we must thankthe various TV channels and print media on running our campaign free. Tannaz Minwala,Mannan Hatim Ali and Azhar Ahmad Khan at Creative Unit have with the help of JamshydMasud's insightful photography once again designed and produced this very impactful Review.All of them have given us their valuable time and creative flair gratis because of their belief inour mission. We owe them a debt of gratitude as we do to all our donors.

Finally I would like to thank our doctors, paramedics and other staff who have performedover and above the call of duty. This has made it possible to treat 1.76 million patientsthis year. I would like in particular to thank Doctor Afzal Khattak and his team at Kalakalay/Barikot hospital and Dr Saeed Khan and his team at Quetta hospital who have maintainedour patient services under very difficult conditions

The next year will be extremely challenging. The 4 new hospitals we have opened inthe past 18 months as well as the 2 expansions will result in a significant increase inpatients. We expect 18.2 million patients to visit our facilities and need to raise Rs326 million to be able to deliver quality eye care to them. In addition we require Rs25 million for Capital expenditure. We hope that donors will recognize the criticalityof our services to the poor people of Pakistan and help us to meet these targets.

Sight is God's greatest gift to Man. Help us give back this gift to those who cannotafford the treatment.

With patients visiting our hospitals in ever increasing numbers we treated our 15thmillion patient in September 2008 and our 16th million patient in April 2009. Both werechildren: Abdul Rehman from a village in Balochistan and Ali Hamza from the slums ofKarachi. Abdul Rehman was blind from birth whilst Ali Hamza began losing his visionfrom age 6. Both were operated for cataract and with their vision restored can look moreconfidently at the future.

LRBT is the largest provider of eye care in Pakistan treating 36 % of patients visiting OPD,27 % of eye surgeries and 32 % of Paedratic cataract surgeries and it is totally free. Wehave been described by Sight Savers International as ≈the one national safety net that theunderprivileged have to access equitable and quality eye care in Pakistan∆.

TTHHEE IINNCCRREEAASSIINNGG VVOOLLUUMMEE OOFF PPAATTIIEENNTTSS HHAASS LLEEDD UUSS TTOOEEXXPPAANNDD OOUURR NNEETTWWOORRKK:: I Arifwala hospital which opened in December 2008 will serve the eye care needs of

approximately 8 million people of Pakpattan, Sahiwal, Vehari, Okara and BahawalnagarDistricts. The land and major part of construction cost was funded by Mr & Mrs. RajaAftab Saeed. Other key donors were Mrs Fayeeza Naqvi from Dubai and Munir Gondalfrom Saudi Arabia. The equipment and furniture was funded by LRBT America who hasalso agreed to fund the operating expenditure of the hospital.

I The new Mansehra hospital will serve a population of approximately 4 million residing inMansehra, Batgram, Muzzafarabad (Azad Jammu & Kashmir), Abbotabad, Daggar andHaripur areas. The land for this building was donated by Mian Usman Paracha. Majordonors for construction of building were Shell and SSI. Other donors were Deutsche BankSiftung, our Riyadh supporters including Mr Shaheen Amin, Friends of LRBT Islamabad,President of Pakistan PTCL, Uch Power and Mobilink. Major donor for equipment andfurniture was OPEC Fund for International Development (OFID) based in Austria.

IINN AADDDDIITTIIOONN TTOO TTHHEE AABBOOVVEE::I We have doubled the size of our Tando Bago hospital with the help of an anonymous

donor in the UK.

I The first phase of the expansion of Khanewal hospital was completed in January 2009funded mainly by Shell, Dr Fawad Zafar USA and an anonymous donor. The secondphase being funded by Mr Nasir Mehmood in Dubai commenced in February 2009and is expected to be completed by end 2010.

I Construction of the OPD complex at Korangi commenced in June 2009 and will becompleted in 1 year. It will cost Rs 17.54 million and is being funded by Ms. KhurshidBhaimia & her brothers.

OOPPDD VVIISSIITTSS // SSUURRGGEERRIIEESSDuring the year ending June 2009, 1,759,098 patients visited the Out Patient Departments(OPD) in our 16 hospitals and 39 primary eyecare / outreach clinics, a growth of 11%over the previous year.

During the same period 185,631 major & minor Surgeries were performed, an increase of4%. The overwhelming bulk of the major surgery was cataract as this is the biggest cause ofcurable blindness .Other surgeries such as Vitreo Retina, Glaucoma, DCR, Squint etc werealso conducted. We tripled our capacity for Vitreo-Retina for which there is a long waitinglist. We are the biggest provider of vitreo-retinal surgery for the poor people in the country.

134 Corneas were transplanted at Lahore and Korangi. In Lahore 91 corneas were donatedby Begum Zahra Manzoor Ilahi, President, Binai Eye Bank Pakistan, who obtained themfrom Sri Lanka. In Korangi 43 corneas were donated mainly by the Lions ClubInternational District 305 (S1 & S3) as well as the Ontario Eye Bank in Canada - courtesyof Syed Muslim Hasany

AAVVAASSTTIINN:: A number of elderly people (over 70 years of age) experience loss of central vision due toAge Related Macular Degeneration (ARMD) resulting in difficulty in daily activities likereading or driving. ARMD is the second most common cause of legal blindness in thewest and the incidence is increasing in this part of the world. ARMD occurs in two formsDry form which accounts for 90% of the cases and is less vision threatening and Wet formwhich is present in 10% of the cases but accounts for 90% of the severe visual loss.

At present there is no cure for ARMD however the more vision devastating wet form canbe treated up to some extent with in two different ways:

a. Laser therapy (PDT- Photodynamic Therapy) is a method by which an injectable Dye isused in conjunction with laser application. Usually a minimum of three sessions arerequired and a single session costs around Rs. 100,000/- Due to its higher cost PDT isnow being used less frequently. LRBT did not offer this treatment.

b. Use of Intravitreal Injection of Anti - VEGF (Vascular Endothelial Growth Factor) hasnow replaced PDT. Two types of Drugs are presently used for this purpose. Lucentis(Novartis Pharma) an FDA approved drug which costs Rs. 50,000/- per session.(minimum of 3 sessions is recommended). Avastin (Roche Pharma) a drug used in cancer 27

of the large intestine, not yet FDA approved for use in eye, is also currently being usedand costs around Rs. 400/- per dose and is equally effective.

LRBT started the Avastin therapy 2 years ago in our Korangi and Lahore Tertiary hospitalswith very good results. In 2008/9 we treated 585 patients.All treatment and surgery was absolutely free

EEXXPPEENNSSEESSOur Operating Expenses during the year were Rs 341 million being 25% above last yeardue to two additional hospitals and a growth of 11 % in patient visits to OPD and 6% inmajor surgeries. Capital Expenditure including Work in Progress and equipment in transitat Rs 135 million was 0.3% lower than last year. Our overhead expenses were 5.2 % ofOperating Expenditure and are among the lowest in the world.

EEXXPPAANNSSIIOONN OOFF CCAAPPAACCIITTYYNEW HOSPITAL AT ARIFWALA (PAKPATTAN):Construction of this 40 bed Secondary hospital which started on 7th July 2007 wascompleted in November 2008. The hospital started functioning in December 2008 but wasformally inaugurated on 18th January 2009 by Mrs. Ismat Aftab. Each day an average ofover 280 outpatients visited our OPD and over 22 major and minor surgeries wereperformed free of cost.

NEW HOSPITAL AT MANSEHRA (NWFP): Construction of LRBT's Secondary eye hospital at Mansehra (40 beds) which started on10th August 2007 was completed in February 2009. We began operating from the newpremises from 2nd March 2009 when the temporary hospital set up after the earthquake inNovember 2005 was closed. The new hospital building was formally inaugurated by MianMuhammad Usman Paracha on 21st April 2009. Each day on an average over 190 patientsvisited our OPD and over 25 major and minor surgeries were performed free of cost.

NEW PRIMARY EYECARE CENTER :Two new Primary Eye Care Centers (PEC) one at Toba Tek Singh District Faialabad and theother at Dadyal (Kashmir) were established in association with Mariyam Eye Hospital andIftikhar Memorial Trust respectively.

PROGRESS - NEW OPD COMPLEX KORANGI (KARACHI) :Construction of OPD wing and patients waiting area on the adjacent vacant plot, owned byus, was started in mid June 2009. The construction cost, estimated to be Rs.17.54 millionis being funded by Ms. Khurshid Bhaimia and her brothers. The OPD complex is expectedto be completed by end June 2010.The Korangi hospital serves a population of approximately 13 million residing in thedistricts of Karachi East, Karachi West, Karachi South, Karachi Central & Malir. Each day

on an average over 830 outpatients visited our OPD and over 135 major and minor surgerieswere performed free of cost.

EXPANSION OF CENTRAL OFFICEConstruction of second and third floor of Central Office, which started on 14th December2007, was completed in March 2009. Whilst the second floor is required for increased staffneeded to manage our expanded operations, the third floor will be leased.

EXPANSION OF KHANEWAL HOSPITAL:Work on the 1st phase of expansion, which included a larger waiting shed, two screeningrooms, larger operation theatre (to accommodate 4 operation tables) and sterilization roomstarted on 1st February 2008 was completed in January 2009.

Work on 2nd phase of expansion which includes male and female wards of 20 beds each isexpected to be completed by the end of 2010.

This hospital is providing services to a population of approximately 3 million residing in theareas of Khanewal, Vehari and Jhang. Each day on an average over 295 outpatients visited ourOPD and over 40 major and minor surgeries were performed free of cost.

EXPANSION OF TANDO BAGO: Construction started on 31st March 2008 and the work on the major project, which includesexpansion of Operation Theatre, Waiting Area with Screening and Consultants rooms, a wardand residence for a married doctor has been completed. This project, including additionalequipment and furniture, has been funded by an Anonymous donor.

This hospital serves a population of approximately 4 million residing in districts of Badin,Tharparkar & Thatta. Each day on an average over 225 outpatients visited our OPD and over40 major and minor surgeries were performed free of cost.

SSWWAATT::We had to close down our Kalakalay hospital in Swat on 28th July 2008 due to the securitysituation. We then opened a temporary facility in Barikot at the Government Tehsil HeadQuarter hospital in December 2008 but had to close it down in early May 2009 when the armyoperation was launched.

Consequent to the army Operation upto 3 million persons were displaced out of which about 15% were lodged in camps. We formed a team comprising two doctors, paramedics and driver.This team visited various camps in rotation, carried out examination, medicated, refracted andissued glasses. Patients requiring more complex treatment or surgery were transported to ourAkora Khattak hospital where they were provided boarding/lodging and the requisite treatmentby LRBT. 8,864 patients were treated, 1,633 glasses were provided and 85 surgeries wereperformed. All absolutely free of cost to the patients.

With most of the IDPs returning home and the security situation improving, we have reopenedthe Barikot facility on 3rd August 2009.

EEAARRTTHHQQUUAAKKEE IINN BBAALLOOCCHHIISSTTAANNOn the morning of 29th October 8 an earthquake measuring 6.2 on the Richter scale hit Ziarat(60 Km northeast of Quetta) and its surroundings. A number of precious lives were lost,considerably more injured and scores rendered homeless.

LRBT formed 2 mobile medical emergency teams who concentrated on the worst affectedarea. These teams provided general treatment and relief material.

Training Courses :● As a consequence of the decision to start Refresher Courses for all paramedic staff

During the 12 months under review, the board of the Trust has been pleased to welcomeKhawar Mann & Miss Sarah Brown as Trustees upon election on19th September. Khawaris a partner at Apax Partners, the international private equity firm, specialising inhealthcare investments and Sarah, who is the grand-daughter of the late Edna Padmore,is a Solicitor and a lecturer at The College of Law.

During the same period, Yaseen Anwar, Dr Yasmine Hilton, Samina Hand and SuneelShivDasani have resigned from the board of the Trust. All four Trustees have taken thisstep reluctantly but have needed to devote more time to their careers and/or professionalactivities. The Graham Layton Trust has been greatly indebted to the contributions madeby each of these four persons during the time that they were on the board and lookforward to their ongoing support as 'Friends of GLT'.

LRBT America has had a very active 2009. We have organized and hosted a range of fund-raising activities thisyear to broaden our donor base.

LRBT America hosted its 11th Annual Gala at the Plaza Hotel, New York on April 25, 2009. With over 300 peoplein attendance, the event was a sold out success

Continuing their spirit of volunteerism and dedication, the LRBT America team held a fundraising event for theSwat IDPs in July at Pacha NYC. The event targeted at a younger audience attracted and added a whole newdemographic, adding over 300 donors to our base of support.

Our 4th Annual Iftar was held at the Residence of Ambassador Hussain Haroon, Pakistan's PermanentRepresentative to the United Nations. Ambassador Haroon generously opened his residence for us and made itpossible to minimize our cost. The Iftar venue was a major hit with the 200 plus attendees who thoroughlyenjoyed the evening and helped raise substantial funds for our Ramadan campaign.

In addition, we are actively working to broaden sources of contributions for LRBT. We attended an Augustinformational session for Pakistani-American NGOs hosted by USAID in Washington, DC. We will also bepresenting LRBT's activities to a broad audience on October 20th at a Capitol Hill event organized by Pakistani-American political advocacy groups.

LRBT America would like to thank all their generous donors and volunteers who have contributed so generouslyand given their time in making this another successful fundraising year. In particular, we thank the volunteer NewYork Committee, each of whom have dedicated countless hours to event organization, fundraising, spreadingawareness of LRBT's activities, and other activities. This year's Committee members include: Ayesha Ahmad,Naushab Ahmad, Noni Durrani, Shama Haider, Sara Haq, Subika Jafri, Shaan Kandawalla, Sehr Karim, Aliya Khan,Asma Khan, Aasim Khawaja, Christine McGinnis, John McIntyre, Lubna Qureshi, Rebecca Ramsey, Kashif Riaz,and Nasreen Rizvi.

LRB Foundation CanadaThis is the second year of our establishment. Our tax exempt application is in process awaiting approval.

In order to raise awareness of LRBT's activities we organized a fashion show in Toronto by a leading Pakistanifashion designer which was covered by the ethnic mainstream media.

A second cataract surgery project has been approved for C$ 25,062 by International Development & ReliefFoundation (IDRF). This is being funded by donations raised by us supplemented by financial support from IDRF.

because he was very unwell during the early months of 2008, he felt unable tocontinue with the increasing workload of this post. All Trustees recognized and havethanked him for the very considerable contribution that he had made over the pastfour years, which has been exemplified by the very considerable increase inincoming resources that has been achieved under his stewardship. Khawar Manntook over the position of Managing Trustee from Michael Pelham as from 19thFebruary 2009.

During these twelve month, Trustees have considered how best to meet the challengeof meeting the ever increasing commitment to LRBT to fund, on a best endeavoursbasis, the requirement for inter ocular lens (IOL) at all of LRBT hospitals. It wasagreed that the method used to operate the day to day governance of the Trust which,until this time, had relied exclusively on volunteer input, was no longer be adequate.Accordingly the Trust appointed Ms Vivienne Hahn as a salaried Project Co-ordinatorof the Trust with effect from 17th November 2008, and has become responsible foradministration and co-ordinating fund-raising activities.

The Trust donated a total of $ 646,794 to LRBT including $ 339,009 from ananonymous corporate donor for specific projects at the LRBT Quetta and Tando BagoHospitals. This pleasing outcome was the result of a wide range of generousdonations and an increasing number of fund raising events, notably the 'A VenetianMasquerade' Dinner Dance held at the Dorchester Hotel, a Spring Appeal to previousdonors, a film show in Edinburgh, and a Sponsored London marathon run by HelenHayward. The success of these fund raising activities is due entirely to the unstintingeffort and work of the Trustees and all of the volunteers who give their time sogenerously and without question, particularly the energetic and enthusiastic Ballcommittee, who raised nearly £90,000. Generous Donors and Sponsors haveprovided continued support from the UK, Saudi Arabia and Bahrain. GLT and itsbeneficiaries are deeply grateful to them all.

As a consequence GLT was able to meet its commitment in respect of funding (on a best endeavours basis) the supply of 'free' IOL's to LRBT's hospitals in 2008/9.The £226,155 cost of this commitment was in line with expectations, in spite of thedeterioration of the sterling/US dollar exchange rate during and after the internationalfinancial crisis in September 2008. It is likely that financial constraints caused bythis crisis will continue in 2009/10, during which period it seems probable that thesterling/US dollar exchange rate will deteriorate further whilst the year on yeardemand by LRBT for 'free' IOL's has been forecast to increase by 14%. Thereforecost of this commitment next year is likely to rise to approximately £300,000. Raisingthis sum will prove to be a significant challenge for GLT.

Finally, the Trustees would like to record their thanks to all of the many donors andvolunteers who have, according to their means and time available, contributed sogenerously to GLT and to its performance in this period.
