Introducción
En los pacientes con cáncer existe una diseminación sistémica precoz de células tumorales, bien en órganos a distancia como células tumorales diseminadas (CTD), como células tumorales circulantes (CTC) o partículas subcelulares “tumorales” en sangre o en forma de ácidos nucleicos circulantes.
La presencia de esta enfermedad subclínica, no detectable con los estudios de extensión habituales, puede conducir a la aparición clínica de metástasis, servir como factor indicador de un mayor potencial invasivo y un peor pronóstico y ayudar en la selección del tto.
Comprender desde el punto de vista molecular y biológico el proceso de diseminación tumoral es de utilidad en la “medicina de precisión”.

Un poco de historia
La presencia de células tumorales circulantes en sangre en pacientes con neoplasias sólidas avanzadas ya
fue descrita por T.R. Ashworth en 1869 [1].
Los primeros trabajos con inmunohistoquímica e inmunocitoquímica se llevaron a cabo en la década de los
1980 [2] empleando estos autores por primera vez el término de “micrometástasis”
El término “biopsia líquida” del inglés “liquid biopsy” fue empleado inicialmente para la detección de CTC
en sangre [3, 4]
Actualmente se prefiere para definir el análisis de ácidos nucleicos en sangre, en las CTC aisladas y
también en plasma o suero [5, 6] incluyendo el estudio de potenciales dianas terapéuticas moleculares y
alteraciones genéticas, especialmente mutaciones, relacionadas con la eficacia de los fármacos [7-9].

For 13 patients who were treated with trastuzumab, 12 were HER2 positive by routine methods, and seven patients showed plasma HER2 amplification. Interestingly, a more effective response to trastuzumab (partial response) was found in all seven patients with plasma HER2 amplification, whereas a less effective response to this treatment (stable disease or progressive disease) was found for four of six patients (three stable disease patients and one progressive disease patient) without plasma HER2 amplification (P = 0.0210; Fisher’s

CTC detection: CellSearch system (Veridex) 52 patients with AGC were enrolled into a prospective study. The chemotherapy regimen was an S-1-based regimen (S-1 with or without cisplatin) or paclitaxel. CTCs of whole blood at baseline, 2 weeks, and 4 weeks after initiation of chemotherapy, were isolated and enu- merated using immunomagnetics.

To generate survival curves, we converted continuous miR-200c expression levels to dichotomous using its mean levels of expression. miR-200c was overexpressed in the blood of 53.8% (28/52) of patients. The percentage of patients with miR-200c overexpression tended to increase with TNM stage: 33.3% (3/9) in stage I-II patients, 50% in stage III patients (6/12) and 61.3% (19/31) in stage IV patients (p = 0.076).
