Medical Certification of Cause of Death Scheme contributes to Health
System by providing scientific, authentic and basic information on causes of
mortality. This data profile will be definitely helpful for better health planning
and to design reliable and result yielding public health intervention
strategies.
The MCCD 2009 report includes data on deaths due to
Communicable & Non-communicable diseases, vaccine preventable
diseases, water borne and arthropod diseases as well as important
indicator like maternal deaths, infant deaths, due to malnutrition, etc. The
mortality data has been analyzed as Million plus Corporations & District.
The area wise disease and mortality pattern provides ready interpretation
for health planners.
The outcome of MCCD report will definitely provide valuable
reference data to medical professionals and researchers. The efforts of
Dr. H. H. Chavan, Deputy Director of Health Services, (HIVS), pune and his
team to bring out this report are appreciable.

Preface
Medical Certification of Cause of Death Scheme (MCCD) has been
implemented in most of the urban areas. However to obtain the data from
rural area, it was extended to Rural / Sub district Hospitals in the State. I
am happy to note that substantial number of MCCD certificates, possessing
good quality data has been received from these hospitals. It has created
awareness about proper identification of underlying cause of death and to
code it as per ICD – 10th revision through MCCD training to all districts.
MCCD certificates from reporting units year after year increased. We
had taken effort for that, we mostly concentrate on Corporation area. This
report will provide very essential statistical information to health personnel
for further studies & drafting future policies. I am happy to record
appreciation of efforts taken by Dr. Vijay Doiphode, Medical Officer, Shri
Sanjay.T. Jadhav, Statistical Assistant and his team for preparation of this
report.
I am deeply obligued to Shri. J. K. Banthia, Additional Chief

1.1 BACKGROUND:
Mortality statistics is the key health indicator to assess quality of
life. However reporting of deaths has been the challenge for Public
Health Organisations, Health Planners & Programmers. It is essential to
identify the disease burden through good documentation of data on
deaths. New types of killer diseases like AIDS, Cancer, Heart diseases,
Juvenile diabetes and other non-communicable diseases are raising
their ugly heads and ringing warning bells. Medical research is facing
stiff challenges from the spread of a variety of ailments. Due to lack of
authentic data; it becomes difficult to tackle these issues. Good
documentation of information on causes of deaths makes the way to
achieve the solutions.
Medical Certification of Cause of Death Scheme is an important
tool of obtaining authentic and scientific information regarding causes of
mortality. International Classification of Diseases is customised to the
Indian conditions under the MCCD scheme. This report is based on the
tenth revision of International Classification of Diseases. The prescribed
Form no. 4 / 4A (Annexure) to be carefully filled by the physician who
examines / attends / treats the diseased during the period of his/ her last
illness. This report is totally based on the cause of death written on it.
1.2 USEFULLNESS OF MCCD SCHEME:
Mortality statistics is essential for better health planning and
management of programmes and to undertake control measures in an
effective & planned manner. It provides scientific & basic information for
medical research and gives mortality profile for medical professionals. It
is useful to know the impact of health services and helps to understand
the trend and changing mortality pattern of various diseases & also to
find out the magnitude of newly emerged diseases like HIV/AIDS.
1.3 LEGAL PROVISION:
Registration of Births and Deaths Act, 1969 has a legal provision
for MCCD under section 10(2), 10(3) and 17(1) (b). Govt. of Maharashtra
has framed the Rules based on the provisions of RBD Act, 1969, with
the approval of Central Govt. These rules came into force from 1st May
2000. Under the Rule No. 7 of “Maharashtra Registration of Births and
Deaths Rules, 2000”, it is obligatory to submit MCCD to the local
registrar. MCCD notification has been published by the Public Health
Department, Govt. of Maharashtra, in the gazette dated, 8th Feb 2006.
By this, it has been made compulsory to send each and every MCCD
certificate, in Form 4/4A, by all Medical Practitioners and Medical
Institutions, in Govt. and Private sector, both in rural and urban areas.
1.4 REPORTING UNITS OF MCCD:
MCCD Certificates from all hospitals and private medical
practitioners are collected at the office of local Registrars and are sent to
office of Dy. Chief Registrar of Births & Deaths & DDHS, SBHI & VS
(Maharashtra State) Pune. MCCD certificates are collected centrally at
state headquarters from following reporting units:

2.1.1 State Highlights-
Maharashtra State is the first state among major states
reporting highest no. of medically certified deaths
against the total registered deaths.
Maharashtra state reports above 2, 00,000 (Two lakh
medically certified deaths every year.
Out of the Total Registered deaths 33.53 % deaths are
medically certified. (Table No.2)
Overall Percentage of medically certified deaths to
total registered urban deaths is 65.38 %. (Table No.2)
Out of total medically certified deaths reported, male
deaths are approx. 61.3 % while female deaths are
38.7% in 2009 ( Table No 4 )
Other Forms of Heart Diseases (I30, I51) has emerged
as a highest principal individual cause of death for year
2009 followed by Acute Myocardial Infarction (I21-I22) (
Table No 5 )
Out of 20 major groups, diseases of the circulatory
system major group tops ( I00 – I99) with maximum
medically certified deaths followed by certain infectious
& parasitic diseases (A00-B99) major group(Table No
6 )
Top ten major groups‟ accounts for almost 93.15% of
the total medically certified deaths. (Table No 6)
Non communicable disease burden found to be more
than communicable disease burden as maximum
deaths occurred due to it.(Table no. 27 & 28)
Infants & children-under five deaths, show foetal
prematurely, foetal malnutrition as topmost cause,
followed by hypoxia, birth asphyxia & congenital
malformation of the circulatory system.(Table no. 21)
Leading causes of maternal deaths observed were
other complications of pregnancy and delivery (O67-
O75, O80-O84),oedema, proteinuria and hypertensive
disorders in pregnancy, childbirth and puerperium
(O10- O16) & Other Pregnancies with abortive
outcome (O00-O02, O05-O08)(Table no. 20)
Out of total Registered deaths in the state, Mumbai
reported maximum medically certified(100%) deaths
followed by Thane(52.4%) & Pune(43.9%)(Table no. 2)
Amongst the seven million plus city corporations in
Maharashtra; Mumbai, Pune and Thane corporations
reported maximum deaths.
2.1.2 Distribution of Deaths by Age & Sex-
Total no. of Medically Certified deaths reported in year 2009 is
212335. Out of the total deaths reported in 2009, 130078 (61.3%) deaths
are male deaths & 82257 (38.7%) are female deaths. Maximum deaths
are reported in 70+ years age group. i.e. 27.7% . Total 8% infant‟s
deaths are reported in 2009. (Table no. 4)
2.1.3 Principal Individual Death Causes-
Other Forms of Heart Diseases (I30-I51) with11% is top most cause of death followed by Acute Myocardial Infarction I21-I22 with 7.9% deaths in year 2009, Septicemia (A40-41) with 7.4%, Respiratory Tuberculosis with 6.5% deaths. Deaths reported as Senility (R54) is 3.6%. 3.1%. due to pneumonia and 2.7% Deaths are due to diabetes. (Table no. 6)
2.1.4 Deaths by Major Cause Groups-
Circulatory system major cause group leads with 28.9% deaths
followed by infectious & parasitic diseases with 17.7% deaths and
diseases of respiratory system with 10.5% deaths. (Table no. 6)
2.1.5 Medically Certified Causes of Deaths by age & sex-
In all causes of deaths, maximum deaths are reported in age
group of 70+ years i.e. 27.7%. While in age groups of 35 to 69 years
total deaths is 43.9%. Among all age groups, lowest proportion of
deaths is observed in 1 to 4 years age groups. Female proportion of
deaths in all age groups is significantly less, (38.7% than that of
male proportion (61.3). (Table no. 4)
2.1.6 Observations in leading Major Groups
1) “Diseases of the Circulatory System” is a topmost major
cause group for year 2009. It is observed that as age group advances,
the number of deaths increased gradually, which is more marked after
35-44 years age group. (Table no. 9)
2) “Certain infectious and parasitic diseases” is a major cause
group, accounting 17.7%. Maximum deaths are observed due to this
cause group between 25 to 64 years age group, Female proportion is
similar in age groups of 0 to 14 years. (Table no.10)
Top five causes of deaths due to Infectious & Parasitic Diseases
Maximum death occurred due to Septicemia ie 7.4%. Deaths due
to respiratory tuberculosis accounts for 6.5% of the total reported deaths.
Maximum deaths due to respiratory tuberculosis occurred in 35 to 54
years age group. Total deaths reported due to HIV are 1.1% . Maximum
deaths due to HIV in the age group of 25 to 54 . Diarrhoea & Gastro
enteritis accounts for 0.3%. Death due to diarrhoea occurred more in
terminal age groups of 0-1 year & 70 above years.
3) “Diseases of the respiratory system” is a major cause group,
accounting 10.5 %. Male deaths are double to female deaths in 25-69
years of age. (Table no. 12)
Top five causes of death due to Respiratory System
Deaths due to other diseases of the respiratory system is 3.9 and
deaths occurred due to pneumonia is 3.1% of total reported deaths.
Maximum deaths were observed in 70+ and above age group.
4) “Injury, poisoning & certain other consequences of external
causes” is a major cause group, accounting 8.8 %. Total 77.9 % deaths
were reported in the age groups of 15 to 64 years, which is economically
productive age group. People in these age groups were involved in
various activities for earning their livelihood & for other reasons, so they
may be exposed more to these external causes than other age groups.

Top five causes of death due to Injury, poisoning & certain
other consequences of external causes
Deaths reported due to this cause group accounts for 8.8% of the
total deaths. Other injuries of specified, unspecified and Multiple body
region is a leading cause of death in this group, accounting for 3.0 % of
the total deaths followed by Burns & Corrosions is 1.9 % Female deaths
observed significantly more than males. Maximum deaths due to Burns
& Corrosions are reported in the age group of 15-34 years. Female‟s
deaths are more than two times to male deaths in this age group.
Top five causes of death due to Digestive System
Deaths reported due to this cause group accounts for 3.8% of the
total deaths. Diseases of Liver, tops higher with 3.2% of the total deaths.
Maximum deaths occurred in age group of in 45 to 64 years age
group.(Table no. 16)
Top five causes of death due Neoplasms
Deaths reported due to this cause group accounts for 5.0% of the
total deaths. Malignant neoplasm of trachea bronchus and lung tops
higher in this cause group accounting to 0.5 % the total deaths in the
state. In other causes of deaths due to malignant neoplasm male deaths
are found more than the female deaths.
2.1.7 Causes of deaths in Communicable & Non
communicable diseases-
Out of total deaths in the state, 10.3 % deaths are due to
communicable diseases, while 38.5% deaths are due to non
communicable diseases. Maximum deaths due to communicable
diseases are reported in the age group of 25 to 44 years age group.
Non-communicable disease burden found to be more than
communicable diseases. Male deaths are observed more than female
deaths.(Table no. 27 & 28)
2.1.7a Leading causes of deaths due to communicable diseases-
Communicable disease burden constitutes about 10.3 % of the
total reported deaths in the state. Respiratory infections tops higher
amongst this group measuring 7.2 % of the total deaths in the state.
Respiratory infections are predominantly more in 25 to 54 years of age
group . Male deaths are remarkably more than female deaths. (Table no.
27 )
2.1.7b Leading causes of deaths from non-communicable diseases-
Non-communicable disease burden constitutes about 38.5%
of the total reported deaths in the state. Coronary heart diseases are
the leading cause of death with 21.7% of the total deaths.
Neoplasms with 5.0% deaths, diabetes mellitus with 2.7% are other
major leading causes of deaths amongst non-communicable diseases.
Male deaths are remarkably higher after 25 years of age group in each
cause group. .(Table no. 28)
2.1.8 Leading causes of Maternal deaths-
Total Maternal deaths reported 324. Maximum Maternal deaths
are reported in 15 to 34 years of age group (Table no.20)
2.1.9 Leading causes of Infant & under five deaths-
Slow foetal growth, fetal malnutrition & prematurity are leading
causes of infant and under five deaths, accounting 22.8% infant deaths
& 18.9% under five deaths. Total 6.5% infant deaths & 7.6% under five
deaths are reported due to pneumonia. (Table no.21)
Percentage of MCCD received from districts is 33.53% to
the total registered deaths & 65.38 % to total urban
deaths respectively in the state.
Mumbai district has reported 100% MCCD certificates
out of the total registered deaths. Amravati (78.9%)
Thane (75.7 %), Ratnagiri(73.9%) Wardha (73.9%),
Sangli (71.2%) shows higher % of Medically certified
deaths against registered urban deaths. (Table no 2)
Hingoli(0.1%), Gadchiroli(1.5%), Washim(4%),
Jalgaon(6.2%), and Jalna(4.2%) districts have reported
less than 10% Medically certified deaths out of the total
registered deaths. (Table no 2)
Hingoli(0.5%), Gadchiroli(12.4%), Washim(14.2%),
Jalgaon(17.4%), and Jalna(17.4%) districts have reported
less than 20% Medically certified deaths out of the urban
registered deaths. (Table no 2)
Medical certification of cause of death scheme is aimed at
drawing mortality profile of the state, by cause, age and
sex annually. The provision in the RBD Act 1969
and Maharashtra births and deaths Rules 2000,
along with MCCD notification has been largely
responsible for this coverage in the State. Still more
efforts are needed to improve in future.
Recent trend shows other form of heart diseases (I30-I51)
as highest principal individual cause followed by acute
myocardial infarction (I21-I22) as per the reported
medically certified deaths for 2009. Till then septicemia
was the highest principal individual cause. Certain risk
factors like smoking, lack of physical exercise, obesity etc.
are responsible for this type of disease burden. Specific
intervention programme for non communicable diseases
can be applied to over come the issue.
From the Major group Diseases of circulatory
system (28.9%) has appeared as a largest killer
disease in Maharashtra State year after year.
This may be either due to specific risk factors (as
mentioned above) leading to more disease burden or
wrong reporting or coding of cause of death in form No. 4.
e.g. Cardiac failure, Cardio Respiratory Arrest etc.
Reduction in risk factors can help to reduce this disease
burden. Now is the time to launch National Health
Programme for Non-communicable diseases like
Diabetes, Heart diseases etc.
Yet many medical personnel mention the cause as mode
of death. Correct filling up of MCCD certificate has
became utmost important now. Repeated MCCD training
programmes must be continued in future for successful
outcome of the scheme in the state.
Except major group pregnancy, childbirth & puerperium
(O00 – O99), male deaths are reported more than female
deaths. It may be due to apathy on the part of co-
inhabitants, to provide the medical facilities to females in
their illness.
Symptoms, Signs & Abnormal Clinical & Laboratory
Finding not Elsewhere Classified major group still shows
maximum deaths. The coding of causes like “PM Done
(R99)”, “Viscera Preserved Opinion Reserved (R99)”,
“Histopathology Exam. (R99)”, “Senility (R54)” are
included in this group. These are not correct causes of
deaths. Hence correct reporting or coding of cause of
death will minimize the bulckyness of data
Deaths reported due to Respiratory tuberculosis
accounts for 6.5% of the total deaths in the state.
These deaths can still be reduced by strict implementation
of RNTCP .
Burns & corrosions are found more in females, indicative
of suicidal or homicidal and to some extent accidental
cause of death.
Senility is stated as a cause of death (though it is not) in
3.6% of all deaths in 2009. It can be still improved with
correct coding of causes of deaths through continuous
reorientation training programmes of Medical Officers.
Slow foetal growth, foetal malnutrition & prematurity are
responsible for 2.1% deaths in 2009, This can be
prevented with better antenatal services, safe delivery
practices & promoting institutional delivery services.
Meaningful developmental programmes & remedial
measures should concentrate a foetal malnutrition.
Diabetes Mellitus is still an increasing public health
problem with 2.7% deaths in 2009. This may be due to
sedentary life style, obesity, lack of physical exercise etc.
Improvement in living life style, physical exercise &
reducing other risk factors will minimize this disease
burden. Intervention strategy alongwith awareness and
IEC activities should be designed to overcome this
burden.
Non Communicable disease burden found significantly
more than communicable diseases in 2009. This data
profile has forced the Public Health Specialists
/Researchers to design intervention strategies for better
health planning in future. Hence, the policy to combat
with these non communicable diseases like Coronary
Heart diseases, cerebro vascular disorders, accidents,
cancer, diabetes mellitus & hypertensive heart diseases,
needs to be designed.
Complications of pregnancy & delivery, indirect obstetric
deaths are major causes of maternal deaths. It has to be
reduced by giving excellent antenatal, intranatal &
postnatal services. Provision of institutional deliveries &
delivery by trained personnel will make a necessary
impact to reduce these deaths.
Perfect planning & strict implementation of Routine
Immunization Programme is necessary to reduce the
death due to Vaccine preventable diseases. High risk
areas such as out skirts, periurban area & area with low
vaccination coverage should be identified & child vent
mother must be immunized.
Strategy to overcome the disease burden due to water
borne & vector borne diseases is to be adopted.
Surveillance activity should be strengthened especially for
newer diseases.
Problems of malnutrition, mental disorders etc. should be
looked upon with due care.
In short, medical certification of cause of death scheme
provides a scientific, authentic and valuable data on
mortality, provided properly diagnosed and correct coding
is mentioned, which enables public health researchers to
design further interventional strategies.
Coverage of MCCD is all urban institutes and
private and Govt. hospitals from rural areas also.
The percentage of MCCD received is hardly 33.5 %
to total registered deaths. This annual report is
totally based on the reported institutions which are
attached in the annexure no 4.
Casual approach of medical personnel to fill up form
no.4 & 4A correctly and to determine the underlying
cause. Trainings are organized for Medical
Personnel about MCCD every year.
In some certificates, causes were not mentioned &
those certificates could have been wrongly coded.
Some data was found incomplete as age & sex are
not mentioned in certificates. Sequence of events to
death and interval of diseases were wrongly
mentioned in some cases. Such certificates are
send to concern registrars for corrections.
Findings of this report cannot be generalised as
deaths occurred mostly in urban area are reported.
Analysis of MCCD is totally based on numerator
only.
Vital rates like Infant mortality rate & maternal
mortality rate cannot be calculated from this data as
the denominator cannot be given.
No statistical tests have been applied for this data.

MEDICAL CERTIFICATE OF CAUSE OF DEATH
Directions for completing the form
Name of deceased : To be given in full. Do not use Initials. If deceased is an infant, not yet named at time of death, write. „Son of (S/O) or „Daughter of (D/O), followed by names of mother and father. Age : If the deceased was over 1 year of age, give age in completed years. If the deceased was below 1 year of age, give age in months and if below 1 month give age in completed number of days, and if below one day, in hours. Cause of Death : This part of the form should always be completed by the attending physician personally.
The certificate of cause of death is divided into two parts, I and II, Part I is again divided into three parts, lines (a) (b) (c) . If a single morbid condition completely explains the deaths, then this will be written on line (a) of Part I, and nothing more is needed to be written in the rest of Part I or Part II, for example gastroentritis, lobar pneumonia, rabies are sufficient cause of death and usually nothing more is needed.
Often, however, a number of morbid conditions will have been present at death, and the doctor must then complete the certificate in the proper manner so that the correct underlying cause will be tabulated. First, enter in Part I (a) the immediate cause of death. This does not mean the mode of dying. e.g. heart failure, respiratory failure, etc. These terms should not appear on the certificate at all since they are modes of dying and not causes of death. Next consider whether the immediate cause is a complication or delayed result of some other cause. If so, enter the antecedent cause in part I, line (b). Sometimes there will be three stages in the course of events leading to death. If so, line (c) will be completed. The underlying cause to be tabulated is always written last in Part I.
Morbid conditions or injuries may be present which were not directly related to the chain of events causing death but which contributed in some way to the fatal outcome. Sometimes the doctor finds it difficult to decide, especially for infant deaths, which of several independent conditions was the primary cause of death; but only one cause can be tabulated, so the doctor must decide. If the other diseases are not effects of the underlying cause, they are entered in Part II.
Do not write two or more conditions on a single line. Please write the names of the diseases (in full ) in the certificates as legible as possible to avoid the risk of their being misread. Onset : Complete the column for interval between onset and death whenever possible, even if very approximately, e.g.” from birth” several years.‟ Accidental or violent deaths : Both the external cause and the nature of the injury are needed and should be stated. The doctor or hospital should always be able to describe the injury, stating the part of the body injured, and should give the external cause in full when this is shown. Example: (a) Hypostatic pneumonia ; (b) Fracture of neck of lemur, (c) fall from ladder at home. Maternal deaths : Be sure to answer the questions on pregnancy and delivery. This information is needed for all women of child-bearing age, even though the pregnancy may have had nothing to do with the death. Old age or senility: Old age (or senility) should be not given as a cause of death if a more specific cause is known. If old age was a contributory factor, it should be entered in Part II. Example: (a) Chronic bronchitis, II old age. Completeness of information : A complete case history is not wanted, but if the information is available enough details should be given to enable the underlying cause to be property classified. Example : Anaemia - Give type of anaemia, if known, Neoplasms- indicate whether benign or malignant, and site with site of primary neoplasm, whenever possible, Heart disease-Describe the condition specifically, if congestive heart failure, chronic on pulmonale, etc are mentioned , give the antecedent conditions. Tetanus -Describe the antecedent injury, if known. Operation-State the condition for which the operation was performed. Dysentry-Specify whether becillary, amoebic, etc, if know. Complications of pregnancy or delivery-Describe the complication specifically Tuberculosis-Give organs affected. Symptomatic statement: Convulsions, diarrhoea, fever, ascites, jaundice, debility etc. are symptoms which may be due to any one of a number of different conditions. Sometimes nothing more is know, but whenever possible, give the disease which caused the symptom. Manner of Death : Deaths not due to external cause should be identified as „Natural.‟ If the cause of death is known, but it is not known whether it was the result of an accident suicide or homicide and is subject to further investigation, the cause of death should invariably be filled in and the manner of death should be shown as „Pending investigation‟.
Summary
MCCD Scheme is implemented under RBD Act, 1969, Section 10(2) (3)
and Maharashtra Registration of Births and Deaths Rules, 2000- Rule 7.
It is obligatory to the attending physician / surgeon to certify death in
form No. 4/4A.
It is the legal responsibility of the head of institutions to submit MCCD
to the Local Registrar regularly.
Fill in form No.4 for institutional deaths and form No. 4A for
domiciliary events but certified by private medical practitioners.
Do not fill in MCCD form 4/4A for still births.
Each and every column of certificate should be filled in.
Cause of Death :-
(I) write the immediate cause, first.
(ii) Antecedent cause on next line.
(iii) Underlying cause, last.
In case of injury and poisoning- Write the type of injury and the part of
the body involved and supplementary cause of injury/poisoning.
In case of deaths due to HIV/AIDS write the seropositive status along
with opportunistic infection or disease conditions responsible for fatal
outcome.
International Classification of Diseases 10th
Revision (ICD-10) is used
for classification of causes of deaths.

In the year 2008, total of 191586 Medically certified deaths are reported, out of 554633 total registered deaths in the state i.e. 34.5% & 279627 total registered urban deaths i.e.68.5% .
Out of 35 districts, reported percentage of medically certified deaths to total registered urban deaths was 75% & above in 5 districts, 50 to 75% in 12 districts, 25 to 50% in 14 districts and < 25% in remaining 3 districts.
