Presentation Notes
Metastases are distributed along the spinal column in approximate proportion to the bulk of vertebrae. The lumbar region is actually the most commonly involved but symptomatic metastases most frequently affect the thoracic region.

Presenter
Presentation Notes
In patients with limited life expectancies from their underlying disease, high surgical complication rates may make palliation of the underlying symptoms impossible. In such patients, radiation often has been used as an initial treatment modality in place of open surgery because of its perceived diminished risk of morbidity.
Page 27: Management of Metastatic Spinal Neoplasms · 2018-11-20 · Radiation resistant tumors Low tolerance of spinal cord to XRT Mets progress or recur Radiation Induced Myelitis Tolerance

Radiotherapy Limitations

Harmful Side Effects to local tissue/skinRadiation resistant tumorsLow tolerance of spinal cord to XRTMets progress or recurRadiation Induced MyelitisTolerance dose 5/5 is 5 GyTolerance dose 50/5 is 7 Gy

Presenter
Presentation Notes
A primary factor that limits radiation dose for tumor control with conventional radiotherapy is the low tolerance of the spinal cord to radiation, which may decrease the treatment dose to a level far below the optimal therapeutic dose. Unfortunately, spinal metastases often progress or recur because insufficient doses of radiation were given due to the dose constraints imposed by the spinal cord’s ability to tolerate radiation.

The Dark Ages

Prior to 2003, only one Class I study was published in the peer-reviewed literatureYoung et al. Journal of Neurosurgery, 53: 741-748, 1980.Randomized prospective comparison:

16 pts underwent laminectomy/radiation13 pts underwent radiation alone

Mean followup: 4 months

Page 29: Management of Metastatic Spinal Neoplasms · 2018-11-20 · Radiation resistant tumors Low tolerance of spinal cord to XRT Mets progress or recur Radiation Induced Myelitis Tolerance

Young et al.

No significant difference was found in the effectiveness of the two treatment methods in regard to pain relief, improved ambulation, or improved sphincter function.

Page 30: Management of Metastatic Spinal Neoplasms · 2018-11-20 · Radiation resistant tumors Low tolerance of spinal cord to XRT Mets progress or recur Radiation Induced Myelitis Tolerance

The Dark Ages: Laminectomy

Spine mets are most often located anteriorly in the VBPoor for resection/decompressionMay predispose patient to spinal instability

Presenter
Presentation Notes
the primary site of spinal cord compression, which is the ventral vertebral body. Tumor in this area often causes intrinsic instability of the anterior column of the spine. Laminectomy procedures, which are destructive to the posterior spinal elements, may then further destabilize the spine and worsen a patient’s pain and neurologic status.41,42 As a result, surgery was historically reserved for patients who had failed radiation.
Page 31: Management of Metastatic Spinal Neoplasms · 2018-11-20 · Radiation resistant tumors Low tolerance of spinal cord to XRT Mets progress or recur Radiation Induced Myelitis Tolerance

In many centers patients were referred for surgery:

After Chemotx and XRT had failedEmergency decompression with acute and rapid neurologic failureConsiderable morbidity

Spine Metastasis: Summary

Spine Mets are not uncommon in patients with cancerSurgery and radiation therapy is superior to radiation therapy alone in selected patientsManagement of patients with spine metastases requires a multidisciplinary approach
