Case Definition for Multisystem Inflammatory Syndrome in Children (MIS-C) [CDC, 5/14/20]

• An individual aged < 21 years, presenting with fever*, laboratory evidence of inflammation**, and evidence of clinically severe illness requiring hospitalization, with multisystem (≥ 2) organ involvement (cardiac, renal, respiratory, hematologic, gastrointestinal, dermatologic or neurological); AND

• No alternative plausible diagnoses; AND

• Positive for current or recent SARS-CoV-2 infection by RT-PCR, serology, and antigen test; or COVID-19 exposure within the 4 weeks prior to the onset of symptoms.

*Fever > 38.0°C for ≥ 24 hours, or report of subjective fever lasting ≥ 24 hours

**Including, but not limited to, one or more of the following: an elevated C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), fibrinogen, procalcitonin, d-dimer, ferritin, lactic acid dehydrogenase (LDH), or interleukin 6 (IL-6), elevated neutrophils, reduced lymphocytes and low albumin

Multisystem Inflammatory Syndrome in Children (MIS-C)

• Temporal association of MIS-C cases occurring 2-4 weeks after initiation of SARS-CoV-2 transmission in a community

• Apparent post-viral syndrome

• MIS-C appears to be distinct syndrome, although some similarities with other recognized conditions, including:

• Kawasaki disease

• Kawasaki disease shock syndrome

• Staphylococcal or streptococcal toxic shock syndrome

• Macrophage activation syndrome/hemophagocytic lymphohistiocytosis

• Cytokine release syndrome

• Tremor, shock, and encephalopathy syndromeTomisaku Kawasaki (1925-2020)

Multisystem Inflammatory Syndrome in Children (MIS-C)

• Symptoms at presentation (among hospitalized children):• Fever

• Gastrointestinal symptoms (abdominal pain, diarrhea)

• Most (65%+) progress to shock

• All develop 1+ organ dysfunction:• Most frequently - myocardial impairment, myocarditis-like injury

• Renal/acute kidney injury

• Gastrointestinal/hepatitis

• Hematologic

• Central nervous system/neurologic

• Few patients with respiratory symptoms (mechanical ventilation, when used, more often to support cardiac function)

Multisystem Inflammatory Syndrome in Children (MIS-C)

• Laboratory studies:

• Neutrophils

• Lymphocytes, platelets

• Inflammatory markers (CRP, PCT, d-dimer, ferritin, IL-6)

• Markers of cardiac injury: troponin, BNP abnormal

• Most with detection of SARS-CoV-2 antibodies (some are SARS-CoV-2 PCR+)

Multisystem Inflammatory Syndrome in Children (MIS-C)

• Echocardiography:

• Impaired left ventricular function of variable severity

• Coronary artery dilations, rarely coronary artery aneurysms

• Anticipate need for serial echocardiography/follow-up with cardiology

Multisystem Inflammatory Syndrome in Children (MIS-C)

• Optimal management (beyond ICU care) = unknown• Best practices evolving;

• Some centers have used:• IVIG

• Immunomodulators (corticosteroids, IL-6 blockers such as tocilizumab)

• Important to collaborate to develop best management plans• Intensive care

• Rheumatology

• Cardiology

• Hematology

• Other sub-specialties

• NEEDED: further systematic studies; collaborative, multidisciplinary research

REQUESTED ACTIONS:

1. Report any patient who meets this case definition to the Vermont

Department of Health in order to increase understanding of risks factors,

pathogenesis, clinical course, and management of MIS-C. Some patients may

fulfill all or some criteria for Kawasaki disease, but they should still be reported if

they meet the case definition for MIS-C. Call (802) 863-7240, ext. 3 to report.

2. Consider MIS-C in any pediatric death with evidence of SARS-CoV-2 infection.

Childcare and Summer Programs: A few of the lessons learned

• Keeping children and staff home when sick is important and hard• Do NOT need medical clearance letters for inclusion of children with allergy or mild

asthma but..

• Do need medical assessment, conversation, confirmation with family

• Children of all ages and staff are figuring out the facial coverings

• Physical distancing is hard with children-do the best you can

• Adequate and available supplies are essential

• Childcare and summer programs need reassurance that if staff or child become ill with COVID19, there is no blame or finger pointing

• Frequent communication through email, phone calls and webinars creates shared understanding and sense of commitment to children/families

Health Guidance: Cloth Facial Coverings for Children

Cloth facial coverings and children:

No cloth facial coverings for children under 2-years-old

No cloth facial coverings while sleeping, eating or swimming

Help children to understand the importance of wearing cloth facial coverings to prevent the spread of germs

Cloth facial coverings are developmentally appropriate for children when they can properly put on, take off, and not touch or suck on the coverage

Cloth facial coverings can be removed during outdoor activities where children and staff can maintain physical distancing and have ready access to put them back as needed when activity stops

Children who have a medical reason for not wearing a cloth facial covering must not be required to wear one

School Health Guidance: Pediatricians Needed

• COVID Task Force with school nurse leader- need pediatrician involvement

• Children with special health and education needs: Team-based care with medical home

• Social emotional health in general also in age of COVID, supporting students and staff

• Food service guidance- advocacy for universal meals since many kids will be eating in classrooms. Let’s decrease stigma and feed everyone

• Ventilation-environment health leaders, including pediatricians needed

• Communication (staff, students, families, community): 8 times, 8 ways-please help
