While the importance of competitor hospitals’ volume data for estimating and monitoring the market share of the AKUH cannot be over emphasized, reliable data on competitors’ volume is difficult to capture. The Marketing Department gathers price data on some common tests, procedures and surgeries on telephone by presenting ourselves as patients because this information is generally available with frontline staff of competitors’ hospital. This approach however is not compatible for gathering volume data. Usually information on prices, quality and features of competitors’ services is obtained by using the ‘mystery shopper’ method. Although, this method helps in generating basic information, however, some of the flaws include unreliable information on competitor volumes.
The major reason for scarcity of hospital volume data in Pakistan is that information on revenue and volume is considered confidential and access to this information is restricted to only senior leadership of competitor hospital. The recently published article “The Cost of Falling Ill” confirms that even the correct pricing information is not available on other hospital websites. For this reason, the staff members of AKUH Marketing Department, to some extent, can extract limited volume data by intelligently socializing with faculty and staff of other hospitals.
Based on volumes, Liaquat National Hospital (LNH) and Dr. Ziauddin Hospital can be considered as AKUH’s closest competitors. However, there are 3 to 4 major competitors for each service. For example, Cath Lab services are available at Tabba Heart Institute, Orthopaedic Medical Institute (OMI) and National Medical Center (NMC). Similarly, major competitors for cataract surgery are Al-Ain Institute of Ophthalmology, Shehzad Eye Clinic and Akhtar Eye Hospital. These service-based competitors are identified by the public (self pay and corporate) including general perception among faculty members mainly on the basis of clinical quality. Hence, there is no single hospital in the city which can be considered as major competitor offering all services under one roof.
While there is a high degree of variation in service-based competitors, the competition can nonetheless be classified into five broad categories:
This category includes Liaquat National Hospital (LNH) an 850 bed hospital, Dr. Ziauddin Hospital a 350 bed facility and Baqai University Hospital. No new entrants have emerged in this category in the last few years. LNH and Ziauddin Hospital are primarily considered as competitors and continue to provide best value for money to the lower and middle income groups. The charges are relatively lower as compared to AKUH and the medical treatment and outcome is generally good. These hospitals also hold a major share of the corporate patient market due to a heavily discounted charge structure offered to large employers and insurance companies.
Expected Competitors
Due to its geographical location, Dr. Ziauddin Hospital continues to be a popular choice for patients residing in Karachi - District North. LNH has been focusing on improving its quality and range of services over the last 3-4 years and poses a significant threat to AKUH. Recent developments at LNH includes improved ambiance, introduction of a new wing for executive rooms, well-designed consulting clinics and operating rooms, evening consulting clinics with premium charges, state-of-the art lithotripter, executive clinic and 64 slice CT Scanner capable of performing coronary CT.
1. Creek City Healthcare Complex
A new entrant expected to emerge in this category is Creek City Healthcare Complex, a joint project of Defence Housing Authority, MAS Associates (a local company) and International Hospital Group (a UK based hospital management and consulting group). The Hospital is planned to be a 300 bed tertiary care facility on a 16 acre campus located in D.H.A. Phase-8 catering to the high-income groups and elite class providing a choice for international patients from UAE, UK and Canada who seek comfort and personalized care. According to the Hospital’s management, Phase-1 of the hospital will be operational by the end of year 2008. However, the ground-breaking at the declared site is yet to occur. In the event that plans for establishing this hospital materialize, this hospital may compete for the high income group currently being served by AKUH and a few other specialized facilities. AKUH may face tough competition in retaining this segment of its market in future.

Shaukat Khanum Cancer Hospital is gradually increasing its services in Karachi and intends to build a second Shaukat Khanum Cancer Hospital facility on the under construction Link Road. The hospital facility on Link Road is expected to be operational in the next 5 years. Recent addition to services available in Karachi include a Referral Desk and Outpatient Clinic on Shara-e-Faisal near P.E.C.H.S. The main objective of the clinic is to screen and filter patients suffering from cancer so that the desired category of patients can be referred to the Hospital in Lahore for treatment. Future plans include establishing a state-of-the-art, cancer diagnostic facility offering Laboratory and Radiology services in D.H.A. Phase 7, Karachi which is expected to be operational by 2009 - 2010.

NMC, OMI and Park Lane provide relatively better quality and have an excellent panel of consultants and are able to attract patients from middle to high income groups. The charges of these hospitals are quite comparable to those of AKUH, but these hospitals due to their smaller size and lower overhead expenses are able to provide relatively better personalized care and discounts.
South City Hospital will be a major threat for market share from Clifton and Defence. The scheduled Phase-2 of the Hospital is expected to be completed in 2007 and will result in expansion of inpatient facility to a total of 80 beds. Other services planned in this phase are a fully-equipped ER and Cath Lab.
Other competitors in this category could include new entrants or existing hospitals after improvements and expansion in services.
This category includes hospitals offering a single specialty ranging from Ophthalmology and Dental Services to Cardiac Surgery, Orthopaedics and Urology. Popular hospitals in this category include Akhtar Eye Hospital, Al-Ain Institute of Ophthalmology, Hashmani Hospital, Kidney Centre, A.O. Clinic, Tabba Heart Institute, etc.
Tabba Heart Institute due to its state-of-the-art facility and equipment is a threat to AKUH’s Cardiology and Cardiac surgery services. Charges at Tabba for Cardiac surgery are approx. 30 - 40% lower than AKUH. In addition, 40% of the beds at Tabba Hospital are supported through welfare by the Tabba Group and other Memon welfare agencies. These factors give the Hospital a formidable edge in competing for patients belonging to the lower and middle income groups.

Entry in this category is relatively easy due to lower infrastructure costs and many new entries can be expected in the coming years.
Welfare and community hospitals play a significant role in providing the population with cheaper medical alternatives. Some of these hospitals such as Sindh Institute of Urology and Transplant (S.I.U.T.) also offer excellent quality, while the quality of others depends on the funding resources available to the hospital.
Patel Hospital is a tertiary care, welfare hospital located in Gulshan-e-Iqbal. The hospital provides an excellent option to good quality care with experienced consultants catering to lower and middle income groups. Low-cost emergency and consultation services attract a large number of patients. The Hospital also offers a Burn Unit facility in addition to Cardiology, Cardiac Surgery and Intensive Care services.
Other popular hospitals in this category include Chiniot General Hospital, Kharadar General Hospital, Islamic Mission Hospital, Memon Hospital, and Bantva Memon Hospital etc.
This is a growing category and one of the entries within the next few years will include Memon Medical Institute, a 300 bed tertiary care hospital located in Safura Goth - a new development located approx. 10 kms. from Nipa Chowrangi.
Recent initiatives taken by the Local and Federal Government to improve the Health care sector through aggressive reforms in large public hospitals like Jinnah Postgraduate Medical Center (J.P.M.C.), National Institute of Cardiovascular Diseases (N.I.C.V.D.), National Institute of Child Health (N.I.C.H.) and Civil Hospital could result in improvement of services and quality offered by these facilities. KIRAN is an example of a well-managed Government owned Institute offering cost intensive services like Radiotherapy free of charge to a large number of patients. N.I.C.V.D. is an example of a Government Hospital catering to both affording and non-affording class by offering a differential pricing and service strategy.
A recent entrant in this category is Karachi Institute of Heart Diseases (K.I.H.D.), a 350 bed Cardiac Hospital located in Federal B. Area with 120 operational beds. The hospital is currently offering Cardiology services only without an Operating room at exceptionally subsidized rates to over 250 outpatients daily.
If the recent reforms initiated by the Government are successful, it will have a positive effect on the quality of treatment and many additional patients would opt for cost effective options, hence, higher utilization of such hospitals.
