Masons Supply has a Safety Committee made up of both management and employee representatives which meets the requirements of OR OSHA 437-001-0765. A minimum of 4 individuals are assigned to the Safety Committee with an equal number of employee and management representatives. Additional employee representatives may be added if agreed upon by the committee. A Committee Chairman is elected by the committee annually. Individuals generally serve on the safety committee for a 2 year period. Half of the members are rotated annually. Employee membership is on a volunteer basis. Safety Committee meetings are held on company time. Safety Committee members are trained on hazard identification in the workplace and principles regarding effective accident and incident investigations. Responsibilities of the safety committee include: • Holding monthly safety committee meetings. • Developing a written agenda for conducting safety committee meetings. • Preparing written minutes of each meeting which are posted for all employees to

review and are sent to each member of the committee. • Conducting and documenting quarterly workplace safety inspections. Safety Committee meetings are used to: • Make recommendations to employer on hazard elimination. • Establish a system for obtaining suggestions, reports of hazards, etc., from all persons

involved in workplace operations. • Establish procedures for investigating all safety-related incidents. • Evaluate employer’s safety/health policies and procedures. • Make written recommendations for changes in safety/health policies and

procedures. Copies of safety committee meeting minutes are kept for 3 years by the Human Resource Department.

Listed below are rules that all employees must follow to ensure safety in the workplace. Failure to comply with these rules may be cause for a written warning, suspension without pay, or discharge.

1. All accidents or injuries are to be reported immediately to a Branch Manager.

2. Chemical containers must be labeled with MSDS information.

3. Shoes that completely enclose the foot are required in the warehouse area.

4. Only Masons Supply employees or service vendors are allowed in the warehouse area.

Approval from a Masons Supply Branch Manager is required before a visitor is allowed into

the warehouse area. All Visitors are to be escorted by a Masons Supply employee at all

times while in the warehouse area.

5. Do not lift more then 100 pounds without help. Always ask for help to complete a lift if you

believe weight is greater than you can lift safely.

6. All spills are to be cleaned up immediately.

7. Keep aisles and walkways free of obstructions.

8. Do not run in facilities; always walk.

9. Individuals must follow operational processes and use any additional personal protective

equipment requirements outlined for any process.

10. Alcoholic beverages or illegal drugs are not permitted on the premises at any time, nor is

an individual allowed to work when under the influence of alcohol or any

drugs/medication that can impair physical or mental processes.

11. Horseplay or practical jokes that could present a safety hazard are prohibited.

12. Lock-out, tag-out procedures must be followed before repairing any equipment. Running

equipment without a lock out tag is prohibited.

13. Smoking and tobacco are not allowed inside any building.

14. Individuals who operate fork trucks must be licensed in their operation.

15. Seatbelts are to be used when operating forklift.

16. Forklift operators are to maintain a safe speed at all times.

17. Forklifts must have daily inspections.

18. Forklift operators will not exceed the rated load capacity of the forklift.

19. Pallets loaded onto pallet racking in receiving are to be wrapped in plastic stretch wrap or

banded when loaded above the ground floor level.

20. Do not tamper with or attempt any unauthorized repairs to any equipment.

21. Extension cords are for short term use. If required to run equipment long term, equipment

must be wired to power source.

22. Floor space in front electrical panels is marked with black and yellow tape. Marked area must be kept clear of material, equipment, or debris.

Keeping the work area clean and well organized is not only safer, but improves work efficiency. Remember these rules: • Keep floors and workbenches clear of all debris. Trash around the work area could

cause a passerby to trip and creates a fire hazard. • Spills should be cleaned up immediately. Slippery substances on the floor could

cause an accident. • Tools should be returned to the proper storage areas after use. They should be

cleaned and any tool not in good working order should be repaired or replaced. • Loose cords, hoses, etc. should not extend into aisles as they create a tripping

hazard. • Keep aisles and walkways free from obstruction. • Round or Rolling Objects left on floor can cause an accident and serious injury. • Keep all areas free of broken pallets, scrap woods, etc. To avoid trip hazards and

possible puncture injuries from exposed protruding nails.

In accordance with OSHA requirements, Masons Supply Co. has developed a hazardous communications program. The purpose of this program is to ensure that employees are aware of hazardous materials used and stored in the work place and are trained in the proper handling and use of these materials. The components of and responsibilities for Masons hazardous communication program are described below. Hazardous Material Inventory A list of hazardous materials known to be in the work place is prepared and maintained by the Branch Manager and is available to any Masons employee. A “hazardous material” is defined as any element, chemical compound, or mixture of elements and/or compounds which is a physical or health hazard as defined by OSHA standard in 29CFR section 1910.1200(c) or 1910.1200(d). Masons relies on the manufacturer of the material to provide information on the hazardous nature of any materials provided. Material Safety Data Sheets (MSDS) A copy of the Material Safety Data Sheet of any hazardous material used or stored in the facility is kept on Masons website and is available to customers and employees. MSDS sheets for materials no longer used in the facility are kept for a minimum of 7 years. Introduction of new hazardous materials Any individual who introduces a new hazardous material into the facility is required to provide a MSDS for the material and obtain approval from the Branch Manager before the material is stored or used at any facility. The Branch Manager ensures that any employees who will use or may be exposed to a hazardous material are trained in the proper use and handling of the material prior to use by reviewing the appropriate MSDS with the employee. Masons employees are not required to use any hazardous material if they have not been trained on its proper use and handling. Material Labeling Materials entering any Masons facility must retain the warning label affixed by the manufacturer. If material is transferred to a different container, the container is labeled with the contents and information on flammability, reactivity, health, and required personal protective equipment. If it is not practical to label the container (label does not adhere or the container is too small), the container is marked to distinguish the contents and the safety information for the material is posted in the area.

It is the responsibility of the Branch Manager to ensure that all containers in the area are appropriately labeled. Masons employees are not required to use any hazardous material that is not properly labeled. Training Masons provides training to all employees on hazardous materials. Training includes: how to read a MSDS, how to read a warning label and labeling requirements, company safety rules, and general practices for proper use and handling of hazardous materials. All truck drivers for Masons have been certified to transfer Hazard materials to different locations. Hazcom and Emergency Response Training is provided to new employees before they use any hazardous materials. It is the responsibility of HR to ensure this training occurs. Training records are maintained by Human Resources. Audits of the Hazardous Communication Program An audit of the Hazcom program is performed annually by the Safety Committee. A report of the audit findings is published and is available for any Masons employee to review. The audit report includes:

• Names of individuals performing the audit • Area audited • Date performed • Status of MSDS files, container labeling, training records, training content • List of discrepancies found • Corrective action plans and responsibilities

Program Administration It is the responsibility of the Branch Manager to ensure compliance to Masons hazardous communication program and to update the program to align with local and federal regulations.

Protective Clothing Protective clothing is important when handling hazardous materials. • Safety glasses must always be worn. • Goggles and face shield must be worn when transferring corrosive materials. • Protective gloves must be worn when handling materials that can be

absorbed through the skin. Remove gloves anytime you are not at your work station. Do not use the telephone or touch a doorknob while wearing contaminated gloves.

Gloves do not protect against all materials. Extreme care should be taken when handling all hazardous substances. Never intentionally touch hazardous materials.

After working with hazardous materials wash off or wipe off any protective clothing that may have been contaminated.

Handling Hazardous Materials When handling hazardous materials, the following rules should be observed: • Do not rub your eyes or touch other parts of your body. • Never intentionally sniff or taste an unknown substance. • All mixing of volatile or reactive chemicals should be performed under a

hood. Never place your head inside a ventilation hood. • Never mix acids and solvents – it could result in an explosion. • Never puncture a container of a hazardous material. Label all containers with material contents before using.

Use these general procedures to treat exposure to hazardous materials. Reference the Material Safety Data Sheet (MSDS) for the material for specific treatment information. If an individual requires medical attention, provide a copy of the MSDS to medical personnel. Skin: Remove any contaminated clothing. IMMEDIATELY flush the affected area with water for 15 minutes. The water may be cold, but it is important that all efforts are made to ensure that the victim receives a full 15 minutes of rinsing! Use soap if available. Get immediate medical attention. Eyes: Do not rub! Flush eyes with eye washing kit. Lift lower and upper eyelids frequently to ensure complete washing. Get immediate medical attention. Ingestion: Victim conscious: Do not induce vomiting. Check the MSDS and copy for the doctor. Wash out mouth with large amount of water. Get immediate medical attention. Victim unconscious: Do not give liquids. Place victim on side to avoid strangulation. Get immediate medical attention.

Report all accidents to your BRANCH MANAGER!

Hazardous Material Storage • Any container should be clearly labeled. Hazardous materials should only be stored

in designated areas and containers. Containers should be wiped clean and capped.

• Acids should not be stored with caustics or solvents. • Flammable materials should be stored in the designated area.

Hazardous Material Waste Dispose of all hazardous materials as instructed.

Hazardous Material Spills In case of a spill, notify your Branch Manager immediately for special equipment to clean the spill and proper disposal.

Hazardous materials labels provide information about the hazards and safe handling of the material. Always read the label before using a new material. Materials are labeled with the name of the item. Materials are also labeled with ratings of health, flammability, and reactivity hazards on a scale of 0 to 4. A rating of 4 indicates extreme danger, while 0 indicates that the level of hazard is low. A guideline to interpreting hazard ratings is provided below:

The National Fire Protection Association hazard identification systems identifies the type of hazard in a diamond shaped label: Special hazards include information such as unusual reactivity with water (W), an oxidizer (OXY), or a radioactive hazard. There are three signal words on labels which indicate the degree of hazard:

• CAUTION! Means the material is hazardous • WARNING! Means the material is more hazardous

• DANGER! Means the material is very hazardous.

Finally, the material should be labeled with the personal protective equipment required for handling the material. If a container is too small to label or the label is easily worn off, the container will be marked with a code to identify the contents and the safety information will be posted in the area where the item is used.

Eye Protection - Approved safety glasses must be worn at all times when in the form yard, warehouse and shop areas. Safety glasses will have side shields. Prescription glasses with side shields are approved for use in the form yard area. All Face shields and goggles must be worn if welding or sanding. Footwear – Boots are required in the warehouse and all form yard areas to protect against spills and dropped objects. High heels (elevated more than 3”) and shoes with slick soles are not recommended. No sandals or open toed, open heeled shoes are allowed in the warehouse. Steel toed boots are recommended for all form yard and productions area employees. Hearing Protection – Hearing protection is required for any operation where noise levels exceed 85dBA. The company provides earplugs which are provided by your manager. Hearing protection is mandatory in posted areas. If the noise level is uncomfortable for you, you should wear hearing protection whether required or not. Respiratory equipment – Filter masks should be worn when working in dusty environments. But remember, filter masks protect against particulates only, not against vapors or gasses. Protective clothing – The type of protective clothing required will vary from operation to operation. Generally, gloves should be worn when handling chemicals, welding, sanding or handling or loading materials that could cause skin sensitization. All PPE must be in good working condition. Free of hole, scratches, cuts, rips etc. Contact your Branch Manger to replace any PPE.

The specific personal protective equipment (PPE) requirements for a process will be determined by your Manager.

Using good lifting techniques is important in preventing back injuries and muscle strains. Lifting: Follow these rules when lifting a load: • Position yourself as close to the load as possible. • Stand with your feet spread to shoulder width. • Bend your knees and lower your body to a position where the load may be grasped

with two hands. • Keep your back straight and lift the load up smoothly, using your leg muscles. • If the load is too heavy, get assistance. Use mechanical lifting devices or hoisting

equipment whenever possible. Know the load capacity of the lifting device and check to ensure you are not overloading it. Inspect all cables, tackles and hooks before lifting loads.

Carrying a Load: A load should be carried in a centered position, close to the body. Don’t lean or twist to one side. Long loads should be carried horizontally by two people or held vertically if one person carries the load. When two people carry a load, it should be carried at waist height. When a load is passed from one person to another, the motion should be smooth. A load should never be thrown from one person to another. Never carry heavy items up or down ladders. Use hoisting equipment. Depositing a Load: A load should be deposited using the same procedure as for lifting. Lower the load by bending the knees and keeping the back straight. Never drop a load suddenly -- this could cause the spine to recoil at the sudden loss of weight. Never attempt to catch or stop falling material unless you are positive there is no danger to yourself or others.

Ladder Safety and Training: Employees of Masons Supply are required to be trained in the proper use of a ladder. All employees must follow these rules when using a ladder.

• Ladders shall be capable of supporting up to four times the maximum intend load.

• When accessing upper landing surface, the ladder side rails will extend 3 feet

above the upper landing surface, to assist mounting & dismounting the ladder. • Ladders must be free of oil, grease and other slipping hazards. • Use a ladder only for its designed purpose. A stepladder shall not be used as a

straight ladder.

• Make sure the ladder is on a stable and level surface.

• Ladders shall not be used on slippery surfaces unless secured or provided with slip resistant feet to prevent accidental displacement. Slip resistant feet shall not be used as a substitute for care in placing, lashing, or holding the ladders.

• If a ladder is put in a place where it can be knocked over, it must be secured or a

barricade put up to keep traffic away form the area.

• Do not use the top or the top step of a stepladder as a step.

• Periodically, ladders must be inspected by a competent person for visible defects.

• Defective ladders will be tagged out or removed from service until repaired.

• When going up/down a ladder, face the ladder.

• When going up/down a ladder don’t carry tools that would interfere with the free

use of both hands.

• When working from a ladder, it shall be secure at the top and bottom.

Forklift Operations Anyone operating a fork truck must be trained on its safe operation and receive an internally issued license to operate the fork truck. The ability of an individual to operate a fork truck is evaluated every 3 years through retraining. Refresher training is required if: • the operator is involved in an accident or near miss incident • the operator has been observed operating the vehicle in an unsafe manner • the operator has been determined to require additional training • the operator is assigned to operate a different type of truck • there are changes in the workplace that could affect safe operation of the truck. Under no circumstances should a fork truck operator lift or carry another individual on the fork truck. • Periodically read the manual and review the rules. • At the beginning of your shift, check your hoist • Examine the load chain for damage or twists, or the wire rope for kinks or fraying. • Check the hook. If it’s out of shape, don’t use it. This may indicate internal damage. • Don’t try to lift more than the hoist rating. If you don’t know the hoist’s rating, find out. • Avoid snagging a load while lifting. • Avoid jogging a load. • Balance a load carefully. Use the right sized sling. • Be sure your load is secure so that nothing can slip out and cause damage or injury. • On two-part reeved hoists, keep the lower block from capsizing to avoid chain or wire

rope damage. • Never use the load chain wire or wire rope for a sling and make sure the load chain or

rope is straight…no kinks, bends or breaks. • Don’t’ bend the rope or chain over sharp edges. • When welding near a hoist, avoid heating the chain and making it weak; keep weld

spatter off the chain, and never use the hoist as a ground. • Never get help lifting something with a chain block. If it can’t handle it alone, don’t

lift it. • Don’t use a pipe cheater on a lever hoist. • When using a wire rope hoist, check the wire on the drum. Don’t let it get out of the

grooves and pile upon itself. • Side pulls with a wire rope hoist may fray the rope and make it unsafe and/or

damage the hoist. • Never leave a suspended load unattended. That loads is your responsibility. • Never carry a load over another person…or get under a load yourself. • Never lift people with a hoist. • When moving a load, look where you are going. Push, don’t pull.

Three out of four injuries on the job caused by electricity involve one or more unsafe conditions. The most common unsafe conditions have been identified as damaged, defective, burned, or wet insulation; lack of proper guards or shields; poor or inadequate maintenance; the absence of proper grounding; and loose connections. Learn to respect electricity regardless of the voltage. Follow these guidelines: • Never work with, adjust, or touch electrical equipment unless you understand it, and

it is part of your job. • Do not use electrical equipment or activate circuits if your hands are wet, or if you

are standing on a wet surface. • Keep flammable liquids away from electrical equipment. • Avoid the permanent use of long extension cords. Do not allow extension cords to

be draped or dragged over sharp edged furniture or equipment, or used in aisles or doorways where they can be damaged or present a trip hazard.

• Notify your supervisor immediately if you notice any frayed electrical cords. • Yanking on electrical cords breaks the insulation and loosens the terminals. When

removing a plug from a socket, pull grasping the plug, not the cord. • Floor space in front of electrical panels must be kept free of storage to provide

access at all times. • All cases of electrical shock or burn should be reported to your Branch Manager at

once. Treating Electrical Shock When electrical shock strikes, muscle spasm occurs and frequently the victim is thrown clear of contact. If, however, (s)he remains in contact with live electrical equipment, try to disconnect the power by pulling a plug or turning off an appropriate switch or circuit breaker. If the power cannot be shut off immediately, you may have to free him/her by using a dry rope, an unpainted wooden pole, an article of clothing, or even a roll of newspaper. DO NOT touch the victim or their clothing, or you may also be shocked! When the victim is clear of the power source, follow these guidelines: • If the victim’s heart is not beating, a trained individual should start CPR immediately.

Masons Supply Co. procedure describing the specifics of our Energy Control (Lock out / Tag out) program: Any electrical equipment undergoing maintenance is disconnected from the electrical supply and tagged with a warning label against starting the machine. A physical lock is installed wherever practical to prevent accidental connection. The location of the breaker or electrical source is documented on a placard affixed to major electrical equipment used in production. The individual performing the maintenance is required to lock out the machine and tag it prior to beginning maintenance work. The individual must sign the tag. It is the responsibility of the individual performing the maintenance procedure to determine when power may be safely restored and unlock and remove the tag from the equipment. No one else is authorized to restore power to a machine that has a lock out tag affixed. If it is impossible to find the individual who has tagged the machine, the tag may be removed only by the Branch Manager or Supervisor for the area. All Branch Managers and Supervisors are trained on the lock-out tag out program. All machine operators are trained in their responsibilities for ensuring safety during lock-out/tag out conditions.

Small fires may be treated using fire extinguishers located around the each building. Large fires should always be left to fire fighting professionals. Emergency Response procedures in this manual are for specific instructions for responding to a fire. Fire is a chemical reaction that involves four elements: fuel, oxygen, heat, and a chemical reaction. Each of those four elements is essential to maintaining a fire. If any one of the components is removed, the fire will go out. Fire extinguishers are located throughout each facility. All extinguishers are inspected monthly to make sure they are in good working order. All extinguishers should have the locking pin in place and the zip tie adhered properly. Using Fire Extinguishers The extinguisher cylinder contains gas under pressure. The extinguisher is operated by: 1. Removing the locking pin. 2. Squeezing the operating lever 3. Directing the escaping gas with the discharge horn. The gas should be aimed at the

base of the flames, using a side-to-side sweeping motion. Follow these basic guidelines when using a fire extinguisher: • Approach the fire from upwind. • Test the extinguisher before getting close to the fire. • Back away from the fire before the extinguisher is exhausted. • Never turn your back on a fire. Use caution when operating a fire extinguisher. As the pressurized gas exits the cylinder, it expands and gets very cold. It could freeze any skin directly exposed.

Identifying Heat Related Illness Heat-related illness can occur when combinations of external heat sources and metabolic heat production overwhelm the body’s ability to regulate its temperature. If someone’s heart begins to pound, they find it difficult to breath, become lightheaded, confused, weak or faint, heat-related illness should be assumed. They should stop all activity and be provided with a means to quickly cool themselves, and be monitored during recovery. Working in a hot environment can cause heat to build up in the body. Heat stress occurs when the body’s cooling system cannot get rid of the heat fast enough. Heat stress can lead to heat exhaustion, which is serious, or heat stroke, which can kill you. Call for Medical Assistance when in doubt. Recognize Common Symptoms of Heat Stress: • Heavy sweating • Cramps or chills • Heavy thirst • Panting or rapid breathing • Rapid pulse • Get help immediately if anyone has the following symptoms: • Headache or blurred vision • Exhaustion, weakness • Clumsiness or confusion • Dizziness or fainting • Convulsions What to Do if Someone is Suffering from Heat Stress: • Move the victim to a cool area. • Remove excess clothing. • Offer sips of water if conscious. • Fan and spray with cool water.

Accidents should be reported immediately to a Branch Manager. Branch Managers will fill out an 801 form and an accident investigation report with the individual involved in the accident. The company’s Workers Compensation carrier requires an 801 form. The individual involved in the accident must fill out the top of the form. Information should be as specific as possible and each box should be completed. The form requires the employee’s signature. Forms are available in the HR office and will be given to the Branch Manager then who will return them to HR. HR will complete the form, send a copy to the insurance carrier, and provide the individual with a copy for his/her records. An Accident Investigation Report is also completed that provides more detail about the accident or injury. This is for the company’s records to better understand what happened and how it can be prevented in the future. The Branch Manager will complete this form with the employee. The forms are located on the Company’s intranet. If immediate medical treatment is required, a Masons Supply representative will drive the accident victim to the nearest Medical Care Facility. Each location has the nearest medical facility posted on the bulletin board. If any injury requires an employee to miss time from work, the employee must provide information from the doctor as to how long he/she will be out of work, an expected return date and any work restrictions upon return. Each February, HR publishes the OSHA 200 log which contains information about all injuries that occurred during the prior year.

To call the Fire Department, dial 9-1-1 Provide your name, a description of the fire, and branch location:

Masons Supply Co. (Your specific store location) If the fire alarm sounds, you must evacuate the building: • All personnel are to evacuate immediately and assemble at the muster point in the

front of your buliding. Check with your Branch Manager for your specific location muster point.

• Branch Manager will check the building to ensure the shutdown of equipment and evacuation of people.

• Branch Manager will take a roll-call at the muster point to ensure all personnel have evacuated.

In case of Earthquake:

Security In case of theft or break in comply with any demands of the intruder. YOUR PERSONAL SAFETY COMES FIRST!

In case of fire: If the fire is small locate the nearest fire extinguisher and follow the directions to put the fire out. Contact your Manager immediately. If any doubt about the fire call the fire department immediately.

Remain in the building. To avoid falling objects move to the interior of the building, away from windows, shelves cabinets or bookcases. Individuals should be familiar with evacuation routes. Maps indicating evacuation routes are posted throughout each location. Do not use the telephone except to call for emergency services. After the initial shocks, do not leave the area unless there is immediate danger to yourself or others. Wait for instructions. Be prepared for aftershocks.
