The Amateur Boxing Association of England Limited Page 1 ME1 proforma for boxer ABA OF ENGLAND: INITIAL MEDICAL EXAM: ME1 IMPORTANT NOTE FOR ALL POTENTIAL BOXERS: The following are the most common disorders that will automatically result in failure to pass the initial medical examination: Epilepsy • Eye surgery, including Laser, except squint correction (also very poor eyesight) Head injury requiring surgery Hepatitis B / C or HIV infection Insulin dependent diabetes Severe asthma Sickle cell disease (not Sickle Cell Trait) (Females: Breast Implants) If you have any of these, you should NOT proceed with the expense of a medical examination but you may seek clarification from ABAE Medical Commission by writing to ABAE HQ before proceeding further. Please answer the following questions regarding your general health. If you are under 18 years of age, this section should be filled in by a parent or guardian. If you are unsure of any question, please ask the doctor performing your examination to explain. IMPORTANT NOTE FOR ALL POTENTIAL BOXERS: The following are the most common disordersthat will automatically result in failure to pass the initial medical examination:
