Use the addendum and the provider directory for the complete list of providers available at each location. To get the most up-to-date information about Security Health Plan’s Ally Rx HMO SNP network providers in your area, you can visit www.securityhealth.org/allyproviders or call our Customer Service Department at 1-877-998-0998 or 715-221-9897.
