    How Do I Get Medicare Part A and Part B?

    Many People Automatically Get Medicare Part A and Part BAlready get benets rom Social Security or the Railroad Retirement Board (RRB)?I you get benets rom Social Security or the Railroad Retirement Board (RRB), you willautomatically get Medicare Part A (Hospital Insurance) and Medicare Part B (MedicalInsurance). Medicare Part B is a voluntary program that will normally require you to pay amonthly premium. Medicare will mail you a Medicare card and general inormation beore thedate you become eligible. I you dont want to keep Part B, you must ollow the directions whenyou get your Medicare card to let Medicare know you dont want it. Otherwise, you will becharged the Part B premium.

    You get retirement benets rom SocialSecurity or the RRB.

    Starting the rst day o the month you turnage 65.

    You get disability benets rom SocialSecurity or certain disability benets romthe RRB.

    Starting the rst day o the 25th month aeryour Social Security or RRB benets begin.

    Your disability is ALS (Amyotrophic LateralSclerosis, also called Lou Gehrigs disease).

    Starting the rst day o the month your SocialSecurity or RRB disability benets begin.

    Live in Puerto Rico?I you live in Puerto Rico and get benets rom Social Security or the RRB, you willautomatically get Part A. I you want Part B, you need to sign up or it. Contact your localSocial Security ofce or more inormation. Visit www.socialsecurity.govand select Find aSocial Security Ofce. Or, call 1-800-772-1213 to nd your local ofce. Y users should call1-800-325-0778. I you get benets rom the RRB, you can contact your local RRB ofce or call1-877-772-5772.

    Note about Part A PremiumsYou usually dont pay a monthly premium or Part A coverage i you or your spouse paidMedicare taxes while working. Tis is sometimes called premium-ree Part A.

    Some people get premium-ree Part A when they turn 65 because theyre already gettingretirement benets rom Social Security or the RRB. You can also get premium-ree Part A whenyou turn 65 i one o the ollowing is true:

    Youre eligible to get Social Security or Railroad benets but havent yet led or them.

    You or your spouse had Medicare-covered government employment.

    I youre under 65, you can get premium-ree Part A i one o the ollowing is true:

    You got Social Security or RRB disability benets or 24 months.

    You have End-Stage Renal Disease (ESRD) (permanent kidney ailure requiring dialysis or akidney transplant) and meet certain requirements.

    People who arent eligible or premium-ree Part A must sign up or Part A and pay a monthlypremium. In most cases, i you want to buy Part A, you must also have Part B, which meansyoull have to pay the monthly Part B premium, too. See the next section or more inormationabout signing up or Part A and Part B.

    Some People Need to Sign Up for Part A and Part B

    Not getting Social Security or RRB benets?I youre 65 or older and you arent getting Social Security or RRB benets yet (or instance,because youre still working), you wont get Part A and Part B automatically. People o any agediagnosed with ESRD and who meet certain requirements are also eligible or Medicare Part Aand Part B, but must sign up or them.

    You need to sign up by contacting Social Security at 1-800-772-1213. Y users should call1-800-325-0778. I you worked or a railroad, call your local RRB ofce or 1-877-772-5772 to

    learn more about signing up.

    When Should I Contact Social Security to Sign Up for

    Part A and Part B?

    Initial Enrollment PeriodYou can sign up when youre rst eligible or Part A and/or Part B (or which you pay monthly

    premiums) during your Initial Enrollment Period. For example, i youre eligible when you turn 65,you can sign up during the 7-month period that begins 3 months beore the month you turn 65,includes the month you turn 65, and ends 3 months aer the month you turn 65.

    Sign up early to avoid a delay incoverage. o get Part A and/or Part Bthe month you turn 65, you mustsign up during the rst 3 monthsbeore the month you turn 65.

    I you wait until the last 4 months o your InitialEnrollment Period to sign up or Part A and/orPart B, your coverage will be delayed. See chartbelow.

    I you sign up or Part A and/or Part B during the rst 3 months o your Initial EnrollmentPeriod, your coverage start date will depend on your birthday:

    I your birthdayisnt on the rst day o the month, your Part B coverage starts the rst dayo your birthday month. For example, Mr. Greens 65th birthday is July 20. I he enrolls in

    April, May, or June, his coverage will start on July 1. I your birthdayis on the rst day o the month, your coverage will start the rst day o theprior month. For example, Mr. Kims 65th birthday is July 1. I he enrolls in March, April,or May, his coverage will start on June 1. o read the chart above correctly, use the monthbeore your birthday as the month you turn 65.

    I you enroll in Part A and/or Part B the month you turn 65 or during the last 3 months o yourInitial Enrollment Period, your start date will be delayed:

    will begin July 1. You may have to pay a higher premium or late enrollment.

    Special Enrollment PeriodI you didnt sign up or Part A and/or Part B (or which you pay monthly premiums) during yourInitial Enrollment Period because youre covered under a group health plan based on currentemployment, you qualiy or a Special Enrollment Period (SEP). You can sign up or Part A and/orPart B as ollows:

    Anytime that you or your spouse (or

    amily member i youre disabled) areworking, and youre covered by a grouphealth plan through the employer orunion based on that work

    Or

    During the 8-month period

    that begins the month aer theemployment ends or the grouphealth plan coverage ends,whichever happens rst

    Tis SEP doesnt apply to you i you have ESRD. You may also qualiy or a SEP i youre a volunteerserving in a oreign country.

    Note: COBRA and retiree health plans arent considered coverage based on current employment.Youre not eligible or a SEP when that coverage ends. o avoid paying a higher premium, make sureyou sign up or Medicare when youre rst eligible.

    Note About Premium Penalties: Part AI you dont sign up or Part A when youre rst eligible, you may have to pay a penalty equal to 10% othe Part A premium, unless youre eligible or a SEP. Te 10% premium penalty applies no matter howlong you delay Part A enrollment. You will have to pay the premium penalty or twice the number oyears you could have had Part A, but didnt sign up. For example, i you delay enrollment or 2 years,you must pay the 10% premium penalty or 4 years.

    Note About Premium Penalties: Part B

    I you dont sign up or Part B when youre rst eligible, you may have to pay a penalty to get it later.For each 12-month period you delay enrollment in Part B, you will have to pay an extra 10% o thePart B premium, unless you qualiy or a SEP. In most cases, you will have to pay that penalty everymonth or as long as you have Part B. I youre enrolled in Part B because youre disabled and yourepaying a premium penalty, you no longer have to pay this penalty once you turn 65. Note: You usuallydont pay this late enrollment penalty i you sign up during a SEP.

    Example: Mr. Smiths Initial Enrollment Period ended September 30, 2008. He waited to sign up orPart B until the General Enrollment Period in March 2011. His Part B premium penalty is 20%. (WhileMr. Smith waited a total o 30 months to sign up, this included only two ull 12-month periods.)

    Medicare and TRICARE CoverageI you have Medicare Part A and RICARE (coverage or active-duty military or retirees and theiramilies), you must have Part B to keep your RICARE coverage. However, i youre an active-dutyservice member, or the spouse or dependent child o an active-duty service member, the ollowingapplies to you:

    You dont have to enroll in Part B to keep your RICARE coverage while the service memberis on active duty.

    When the active-duty service member retires, you must enroll in Part B to keep yourRICARE coverage.

    You can get Part B during a SEP i you have Medicare because youre 65 or older, or youre

    disabled.

    Get More Information about Signing Up for Parts A and BVisit www.socialsecurity.gov, or call Social Security at 1-800-772-1213 or more detailedinormation about signing up or Medicare Part A or Part B. Y users should call1-800-325-0778. People who get benets rom the Railroad Retirement Board (RRB) should calltheir local RRB ofce or 1-877-772-5772.

    Medicare Supplement Insurance (Medigap) Open Enrollment PeriodMedicare Supplement Insurance (Medigap) policies, sold by private insurance companies,help pay some o the health care costs that Medicare doesnt cover. You have a 6-monthMedigap Open Enrollment Period which starts the rst month youre 65 and enrolled inPart B. Tis period gives you a guaranteed right to buy any Medigap policy sold in your stateregardless o your health status. Once this period starts, it cant be delayed or replaced.

    When Can I Sign Up for Medicare Advantage

    (Part C) or Medicare Prescription Drug Coverage

    (Part D)?Tere are specic times when you can sign up or Medicare Advantage (Part C) and Medicareprescription drug coverage (Part D), or make changes to coverage you already have:

    1. When you rst become eligible or Medicare or when you turn 65, during your InitialEnrollment Period. See page 6.

    2. During certain open enrollment periods that happen every year. See page 7.3. Under certain circumstances that qualiy you or a Special Enrollment Period (SEP),

    such as the ollowing:

    You move.

    Youre eligible or Medicaid.

    You qualiy or Extra Help with Medicare prescription drug costs.

    Youre getting care in an institution, such as a skilled nursing acility or long-termcare hospital.

    See the charts beginning on page 8 or a list o dierent SEPs, including rules about how

    to qualiy.

    Note about Joining a Medicare Advantage PlanYou must have Medicare Part A and Part B to join a Medicare Advantage Plan. In most cases, iyou have End-Stage Renal Disease (ESRD), you cant join a Medicare Advantage Plan.

    Note about Extra HelpI you have limited income and resources, you may qualiy or Extra Help rom Medicare thatcould pay or all or most o your monthly premiums, yearly deductible, and prescription drug costsharing.     Note about the Part D PenaltyI you decide not to join a Medicare drug plan when youre rst eligible, and you dont haveother creditable prescription drug coverage, or you dont get Extra Help, you will likely pay alate enrollment penalty. Creditable prescription drug coverage is coverage (or example, rom anemployer or union) thats expected to pay, on average, at least as much as Medicares standardprescription drug coverage. I youre subject to the penalty, you may have to pay it each month

    or as long as you have Medicare drug coverage. For more inormation about the late enrollmentpenalty, call 1-800-MEDICARE (1-800-633-4227). Y users should call 1-877-486-2048.

    Initial Enrollment Periods for Part C and Part D

    Youre newly eligible orMedicare because you turn 65.

    Sign up or a MedicareAdvantage and/or MedicarePrescription Drug Plan.

    Special Enrollment Periods for Part C and Part DYou can make changes to your Medicare Advantage and Medicare prescription drug coveragewhen certain events happen in your lie, such as i you move or you lose other insurance coverage.Tese chances to make changes are called Special Enrollment Periods (SEPs). Rules about whenyou can make changes and the type o changes you can make are dierent or each SEP. Tese

    chances to make changes are in addition to the regular enrollment periods that happen each year.
