1. Introduction
This proposal is made for the implementation of a
computerized integrated medical and accounting
management information system for Osuntuyi Medical
Center. It is aimed at improving upon the existing system
in place, for faster and more streamlined operation. The
software proposed here (Medstar) will address known
issues in the existing system as much as possible. It will
utilize the latest technology in the field and will be so
simple to use as to require minimal training.
Medstar could operate either as a stand-alone or on a
network of intelligent computer workstations.
Medstar would be supported for six months during a
warranty period, after which a support contract would be
due. Alternatively, the user may purchase source code for
the application so as to be able to maintain and support it
in-house.

With operations since 1993, and incorporated in 1996, our objective is
to provide state-of-the-art software development and related services
for our clients. We have been creating client/server applications using
PowerBuilder since 1997, working with the following back-ends:
Sybase SQL Anywhere, Microsoft SQL Server, Pervasive.SQL, MySQL.
Client/server programming at its best!
We develop custom (database) software to client specification. We
develop to international standards, by using modern tools and
techniques. Our tools enable us to develop complete user-friendly
applications within a matter of days. Apart from back-ends mentioned
above, other environments of which we have experience include: RDS-
Informix under UNIX, Sybase Adaptive Server Anywhere, Java,
JavaScript, PHP, Visual Objects, Visual Basic, and Visual Foxpro under
Windows; QuickBASIC, C, Clipper, dBASE, Foxpro and FoxBase under
MS-DOS.

2. Technical specification
2.1. Methodology
The procedure to be adopted in implementing the system
is as follows.
���� Systems analysis: We will review input, output
and processing requirements (for all modules) with
the end-user (Osuntuyi) to ensure that all details are
taken care of by the system. We will also discuss
issues with any existing system to be resolved by
the new system. This will take 2 to 5 days.
���� Application development and customization:
The executable Windows application (Medstar) will
be developed in line with the specifications in this
proposal and the systems analysis outputs. Existing
accounting modules will be adapted to the medical
environment as required, i. e. MoneyBooks Inventory
Control module. The development shall be done
using Sybase PowerBuilder, one of the fastest RAD
(rapid application development) tools presently
available.
���� Installation and training: The application
(Medstar) and the database server software (Sybase
SQL Anywhere) will be installed at the end-user site.
Introductory training will be provided for selected
staff. Parameter data will be entered and users will
be guided in setting up the necessary data tables.
���� Free technical support: Free support will be
provided for six months from installation, to resolve
program errors and provide operational assistance.
This will involve follow-up visits or other
communication as may be required.

2.2. Environment
2.2.1. DBMS strategy
In database management using personal computers there
are presently two major approaches for communicating
data across a network: client-server and file-server.
In the file-server scheme, when a workstation needs to
update one record, the entire file is transferred from the
server PC to the workstation before this can be done, then
back to the server. This leads to slow response times and
data corruption problems, among others. Popular software
packages that employ this type of data management
scheme include Foxpro and Access. It is best suited to
single-user systems.
In the client-server approach, the database engine will be
running on a (separate) machine known as the server, and
will send only the required record for modification to the
workstation. This makes the system faster particularly
where there are thousands or millions of records. Such a
system is also more secure and the integrity of the data is
stronger. The software being proposed (Medstar) here will
implement this client-server strategy, to enable it support
many users without compromising speed and security.
We intend to use Sybase SQL Anywhere as the server
database. It is a small-footprint SQL database that offers
the benefits of client-server for departmental LANs,
without the usual high cost and technical support
problems associated with SQL databases. The present
capacity of SQL Anywhere is over 12 terabytes, limited
only by hard disk capacity, and this is more than adequate
for now and years to come.
SQL Anywhere is mainly compatible with Sybase’s SQL
(and Microsoft’s SQL Server) which is a more expensive

enterprise-level database for wide-area networks or
systems requiring higher workloads. So in the distant
future, expansion will not be a problem.
2.2.2. Database server features
The SQL Anywhere database server is available for the
following platforms:
• Windows x86, x64
• Windows Mobile
• Linux x86, x64 and Itanium
• Sun Solaris SPARC and x86 Mac OS X on Intel
• IBM AIX
• HP-UX Itanium
See appendix at the end of this document for a fact sheet
detailing the features. One SQL Anywhere database server
can support multiple clients operating on different
operating systems, communicating via the TCP/IP network
protocol.
2.2.3. Operating system
The software (Medstar) will be a multi-user application
that will run on Windows workstations. The database
server will operate on any platform listed in the previous
section.
2.2.4. Hardware requirement
For a multi-user environment, the database server
computer should be a Pentium PC with 512 MB RAM or
better. The Medstar application requires workstations with
Pentium processors and 16 MB of RAM minimum. In the
Windows environment, the more the RAM and hard disk
space available and the faster the CPU, the better the
response times for applications.

2.3. General software features
2.3.1. Security
Each user of the system will have an ID, a personal
password, and a list of permitted activities (in every
module). Hence no unauthorized user would be able to
use the system without the required ID and password.
2.3.2. Windows features
Being a Windows application Medstar will use Windows
services such as for printing, and will also conform to the
guidelines for the Windows user interface.
2.3.3. Data entry
Entering data in Medstar is through browse lists and data
entry forms, as in the following illustration.
Browse lists are used to manage records with few data
fields, while forms are used for records with many fields.
Browse lists are resizable grids, with movable and
resizable columns.

On each data entry form, buttons and commands are
available for data management, as shown in the previous
illustration.
Ordinarily you would enter data via the keyboard by
typing. For most data items the main restriction would be
in the number of characters you may type. The standard
Windows data entry controls are also used, such as
listboxes, combo lists, checkboxes and radio buttons. All
dates allow 4-digit years, for Year 2000 compliance.
2.3.4. Reporting
Formatted reports when generated may be viewed on
screen or printed. A Report Manager tool is available for
managing reports generated and stored on the hard disk
or the database. The data entry screens also have Print
buttons to produce quick print-outs of data on the screen.
The formatted reports have the following markers:
User id stamp: This identifies the user that generated
the report as well as the software version.
Date stamp: This follows the user id stamp and shows
the date and time the report was generated, from
the system clock.
Page number: This appears at the bottom of the
report and usually includes the current page as well
as the number of pages.
2.3.5. System enquiry
Medstar offers authorized users access to all available
information. Users have a simple query-by-example
facility such as shown on the following illustration to
construct queries.

2.3.6. Auditing
There will be an audit table with restricted access in which
the user ID, date and time are recorded for all entries,
exits and operational activities within Medstar. So data
changes and who made them will be logged. Certain
transactions may also be printed and corrected prior to
posting.
2.3.7. Referential integrity
The RDBMS ensures that data integrity is maintained at all
times. Values that should be defined in look-up tables
must EXIST before records are saved at all. Similarly
before any record could be deleted, it must not have other
records depending on it, unless these will also be deleted
automatically.
2.3.8. Parameterization
Medstar will use a parameter table as well as look-up
tables to hold data that control how it works. The
parameter table will hold important variables most of
which some users will be able to modify. Users will also
be able to maintain the look-up tables. As much as
possible, embedding of data values in program code will
be avoided.

2.3.9. Data Backup
Backing up of the database is one of the functions handled
by a utility of the RDBMS. However it will be possible to
call up the utility to perform this function from within the
Medstar application at any time.
2.3.10. Instant Messaging
This facility allows users of Medstar to exchange
messages. System-generated messages will also be
possible, such as to alert designated users of certain
situations, possibly with audio prompts. So when a patient
visits, the doctor could be alerted with a message. When a
message is sent, it will pop up at the receiver’s
workstation within 5 seconds or less, or whenever the
receiver logs into the system.

3. Functional Specification
3.0. Modular structure
There will be up to nine integrated modules in the system,
two for core medical records and functions. These are
illustrated below. However some modules will be optional.

The lines represent flow of control and information. The
two modules by the left are the core medical modules
while the rest are accounting modules. The same chart of
accounts defined in GL will be available in each of the
connected modules. From the other accounting modules
will flow transactional information to the GL.
From the medical modules, medical stock information will
be exchanged with the Inventory, ordering information will
be exchanged with Accounts Payable, billing information
will be exchanged with Sales, while medical equipment
information will be exchanged with Fixed Assets.
The Sales module will decrease Inventory when drugs are
sold, while the Accounts Payable will increase Inventory
when ordered items are delivered.

3.1. Basic Records
This is the core medical module.
This comprehensive module
handles the management of basic
patient information, from
registration to discharge. When a
patient is registered, the module
sets up the necessary records for
the patient's basic data, admission
details, discharge details,
accounting data as well as drug
administration details. One of its
main functions is maintenance of
daily attendance registers which
could be referred to at any point in
time for reference as well as
statistical reporting. Some of the
important reports from this module
include patient list, attendance
register, attendance history,
admission and discharge lists, registration and drug
history. Key components of the module are listed below.
3.1.1. Patient Billing
This component of the Basic Records module provides
automatic patient billing functions. Once the user sets up
the appropriate standard charges the system is able to
automatically generate bills for such activities as patient
registration and consultation. It also communicates with
the Inventory module where drug charges are maintained.
3.1.2. Case Notes
This component of the Basic Records module is aimed at
meeting the needs of the medical professional in creating
and maintaining notes on patients. Each note is identified
by patient, subject, date and time as well as the author.

These fields are very useful for searching and reporting
purposes. The body of notes may be any number of lines
long and the user is able to easily mark, delete and re-
arrange text.
3.1.3. Appointment Scheduling
This component of the Basic Records module not only
handles scheduling of patients but also provides the
medical professional with a calendar and diary. At a glance
the user can see what days in the month or year are free,
and then inspect engagements for particular days within a
few presses of keys. Engagements may be identified by
date and time, duration, purpose and if necessary patient.
Diary entries are identified by date. Both diary and
engagement entries are classified by author for
identification. Duration of meetings as well as missed
engagements are monitored, and the module can
automatically bill for consultation charges.
3.2. Operations
This module complements the
Basic Records with specialized data
management facilities.
3.3. Inventory Control
This is a comprehensive module which monitors the
movement of drugs from delivery to filling of prescriptions
to expiry. It maintains supplier accounts, monitoring
invoices and payments; tracks drugs issue to patients and

various sections; maintains re-
order levels and monitors use of
dangerous drugs. Different
batches are maintained per drug,
with supplier information, supply
date and expiration date. One of
its key functions is maintenance
of drug profiles of vital technical
details. Some of the reports of the
module include detailed drug
report, drug profile, drug usage
analysis, missing drugs lists,
requisition list, expiry report and
dangerous drugs list.
The module will also handle other non-drug inventory like
general consumables.
3.4. Sales
This module correctly tracks all bills, and payments made
by patients. Company-sponsored
patients are handled as well.
Some of the important reports
from this module are invoice,
statement of account, account
aging, and covering letters for
statements.
3.5. Fixed Assets

as well as a number of payroll analysis reports.

generating purchase orders and monitoring activities on
such orders including deliveries and payments. Reports to
be generated will include:

3.9. General Ledger
Last but not the least, the General
Ledger module will be at the
center of the accounting system.
This facilitates administration of
the hospital or clinic by providing
standard, debit-credit ledger
accounting functions. Other
modules will be able to send
information (transaction journals)
to the GL if desired. Data entry
facilities will be available for the
following:
3.9.1. Chart of accounts
This will allow setting up of the coding structure. Provision
will be made if required for entry of budget amounts
against accounts. Reports will be available to show
budgeted versus actual results. A control option could
prevent posting of journals in GL that would exceed
allotted budgets.
3.9.3. Bank reconciliation
In this component of the GL, cleared and uncleared bank
items are identified in a report which could be used to aid
in reconciling the bank statement.
3.9.4. Transaction entries
Entries will be made for cashbook, bank book and other
ledgers as journals. The type of the journal will be

included, along with the ID of the data entry operator.
Other items that will be included in journals are:

Every journal entered must balance—i.e. debits being
equal to credits—before being posted.
