• An NHSE vanguard site, developing the Multi-specialty Community Provider (MCP) model of care, as described in the NHSE Five Year Forward View.

• In the model an extended team of GPs and specialists work together to provide better access to a wider range of health and care closer to people’s homes.

• Centred around access hubs that support a population based around natural communities of care.

• Better Local Care has grown to cover 829,000 registered population in 13 natural communities of care across five CCGs.

• There is commitment from GP practices, Southern Health NHSFT, Solent NHS Trust, Hampshire County Council, Southampton City Council and commissioners to integrate our teams and work together at scale.

• MH problems are the largest single cause of disability in the UK, with a cost to the economy of £105billion/year

• 1 in 5 mothers have MH problems in the year after birth• People with serious mental illness die 15-20 years earlier,

2/3 of these deaths are from avoidable causes• Long term condition + mental health problem = 45%

increased cost of care • 40% of elderly in care homes are depressed

• 1 in 6 of population has diagnosable mental health problem• Half do not access primary care• Half that do access primary care are not recognised as having

a mental health problem• 90% of those who are receiving care are managed in primary

care• 28% of suicides occur in people who are not in contact with

MH services• People ‘stuck’ in mental health services, who should be

managed in primary care

Integrating mental health into Better Local Care aims to achieve: • Improved early identification and intervention in mental health

problems• Reduce proportion of GP consults about mental health• Improve rates of mental health interventions for people with

long term physical health conditions• Improve physical health care for people with serious mental

illness (close the mortality gap)• Reduce referrals into mental health services, and more rapid

discharge• Improve access to crisis services• Reduce suicide rate

Lessons so far…

• A step change is needed – not playing around the edges

• Creating and developing leadership capability throughout the organisation and system is key

• The challenge to focus all our people on transforming services for the future, despite increasing demand and reducing resource now

• Changing culture – across the whole system

• Organisational form changes are required to successfully deliver the new models of care
