_____ Enlarged Legible Drivers License Copy (Color copy if possible) _____ 3 Months Merchant Statements (Only applicable for accounts that have processed credit cards with other merchant accounts in the past.) _____ Letters of Incorporation or Copy of the Business License **Further Information maybe required for accounts with monthly volume of $100,000.00+ or accounts that fall in the high risk, highly discouraged business/products, or if merchant’s/officer’s/owner’s credit is questionable and/or poor.
