It is now the Olympic season. A time for all of us to watch the world’s best athletes compete for the gold medal. In perianesthesia nursing we are in the season of competition, though our prize is not a gold medal, but the gold leaf award. This is a time for our component to show our best and strive for the ultimate goal. The goal for ASPAN is to have components work towards the gold leaf award in all they do. The gold leaf award is designed to recognize excellence in component leadership and member development, communication, education services and community relations. The award drives us to encourage quality component management and recognize, with distinction and visibility, the efforts and results of meaningful activities that build a strong component. There are seven different categories on the gold leaf application: governance, membership/customer service, financial health/stability, participation at the national level, certification, community/information to the public, and research/EBP/celebrate successful practices. The application requires documentation for a minimum of sixteen of the thirty-six criteria. Each area is allotted points which a committee uses to determine the winner. The gold leaf is a vast accomplishment for a component to win of which MNDAKSPAN has not yet done. The board of directors worked very hard to create a strategic plan to help us get a little closer to the ultimate gold leaf award. You will notice this year we have included some initiatives to increase our membership, involve us in the community and promote evidence based practice. By using our strategic plan to help guide us to fulfill some of the requirements of the gold leaf we will get a little closer each year. Mission Statement: MNDAKSPAN serves to

promote the education and professional growth of nurses practicing in perianesthesia

areas such as ambulatory surgery, pre-anesthesia, and

post-anesthesia care.

One of the initiatives to increase our evidence based practice and promote involvement is a poster display area at our winter conference. There will be a designated area for those interested in presenting their projects of research or even process improvements. By presenting at the conference this gives our local membership the ability to see the good work that some of you are doing. If you or someone you know would be interested, please reach out to me to reserve your spot.

Another one of our initiatives is to be more involved in the community. We have always had a community service project available for those attending the conferences with the opportunity to donate. Sometimes it has been the local food shelf or other healthcare mission work. This year we are looking for opportunities for us as a group to actually put our hands and feet to work. We are looking for ideas that would allow us to be a part of and involved in improving the health of our communities. If you know of a local cause, please provide us with your idea.

These are just two examples of the new ideas coming through a board that is excited to see us reach for the gold and improve our component. Achieving the gold leaf is a big undertaking however just like some of the Olympic athletes we can all work together to reach the gold.

Family Visitation of Pediatrict Patients in the PACUPurpose and rationale: The purpose of this evidence-based practice project was to explore the benefits of family visitation of pediatric patients in the Post Anesthesia Care Unit (PACU) and to develop a visitation policy based on best practices. An evidence-based approach was utilized to promote family visitation of pediatric patients in the post-operative period in order to decrease patient and family anxiety levels, improve pain scores, facilitate communication with parents, and improve overall satisfaction.

Synthesis of the evidence: Family visitation in the PACU is a controversial topic and visitation practices vary widely across the country. Historically, PACUs have been closed units. More recently, the restrictive practice of excluding visitation in the PACU has been challenged. Evidence has confirmed the development of a parental visitation program in the PACU is beneficial in providing family-centered care (Junge, 1987). Reuniting children with their parents provides reassurance and comfort to both the child and parent, in addition to promoting increased communication and collaboration between family and staff (Kamerling et al., 2008).

A Staff Assessment Survey was created to examine staff attitudes and beliefs regarding visitation in the PACU. An 11-item questionnaire was distributed and collected prior to the implementation of the proposed practice change. 33 RNs responded, yielding an 89% response rate. Results showed despite 73% of staff strongly agreeing providing psychosocial and/or emotional support to family members is part of their practice, only 45% strongly agreed they feel comfortable providing that support. The survey also revealed 33% of staff strongly agreed parents/guardians should have the option to be present during their child’s stay in the recovery room, however 90% responded they would want the option to visit their own child in the same situation. Overall, the results indicated staff was hesitant of any change in practice related to family visitation in the PACU. The perceived barriers to family visitation identified by staff were overwhelmingly spacial and privacy issues, along with staff biases, the comfort level of staff with family at the bedside, and concerns of family member disruptions to the delivery of care.

Implementation Strategies

~To explore the benefits of family visitation of pediatric patients in the PACU and to develop a visitation policy based on best practices. ~To promote family visitation of pediatric patients in the post-operative period in order to decrease patient and family anxiety levels, improve pain scores, facilitate communication with parents and improve overall satisfaction.

Additional benefits: Staff satisfaction, positions hospital more effectively in marketplace, and enhanced employee pride.

~Survey results shared with unit. ~Staff education regarding practice change/new policy. ~Ongoing, informal communication with staff to further disseminate the practice change. ~Presentations at unit meetings by Child Life Specialist on age-appropriate communication techniques and strategies for dealing with anxious children and/or family to support staff acceptance and understanding of pediatric family visitation. ~Renovation of PACU to include dedicated pediatric recovery areas.

~The development of a parental visitation program in the PACU is beneficial in providing family-centered care – Junge (1987)

~By allowing visitation in the PACU, it appears to not only increase patient and family satisfaction, but also staff satisfaction when they realize that they are truly providing the best care to their patients – Walls (2009)

~Family visitation in the PACU decreases patient anxiety scores when compared with no visitation – Vogelsang (1987)

~Reuniting children with their parents provides reassurance and comfort to both the child and parent, in addition to promoting increased communication and collaboration between family and staff – Kamerling et al. (2008)

Practice change: A practice change to allow consistent and timely family visitation of pediatric patients in the PACU was incorporated into a Family Visitation of Pediatric Patients policy and corresponding staff and family guidelines.

Implementation strategies: Staff education was conducted on the pediatric family visitation policy. Survey results were also shared with the staff. Ongoing, informal communication was used to further disseminate the practice change. Presentations at unit meetings by a certified Child Life Specialist on age-appropriate communication techniques and strategies for dealing with anxious children and/or family members were given to support staff acceptance and understanding of pediatric family visitation.

Evaluation: The PACU was renovated to include dedicated pediatric recovery areas. Following completion of the renovation, staff was re-surveyed to determine the level of acceptance with the practice change and compliance with the new policy.

As summer is escaping us all at a phenomenal rate I feel like it would be a great time to talk about politics as the election year approaches us. We see the news and the challenges the politicians are facing today and many of us are at the point that you just don't know what to do or who supports our profession. Does this sound like conversations in your home? We as nurses are a large and extremely effective group. As a group we need to vote and elect legislators that support related nursing issues. Whether we like it or not, politicians play a large role in the future of healthcare and our profession as a whole. The Minnesota Nurses Association website has a list of legislators they support and some general information to guide you. Take a look and be a part of the future (www.mnnurses.org)

In the news, the American Society of Anesthesiologists (ASA) is working on many campaigns. One of the current concepts they are promoting is to keep patients safe in the perioperative area and at home. They are supporting educational material for patients on narcotics, signs of overdosing, how to perform CPR, and the future possibility of sending family members home with narcan for narcotic overdosing in the home. I will continue to follow this campaign as I find it very interesting and will continue to inform you of future developments (here is the link: https://www.asahq.org -when seconds count)

Another tidbit, The National Surgical Patient Safety Summit event recently was held in August and included representatives from medical professional associations, insurers, health care systems, payers and government agencies with a goal of developing surgical care and surgical education. Work groups including surgeons, anesthesiologists, and nurses gathered. The

recommendations from these work groups will be used to finalize National Surgical Patient Safety Standards, develop surgical safety education curriculum proposals, and to identify surgical safety knowledge gaps and research priorities. This campaign is working to optimize surgical patient outcomes (here is the link: https://www.asahq.org -ASA in the news).

We are living in an ever changing environment where our patients have an increased knowledge base and want to be satisfied with their care and outcomes. As perianesthesia nurses we have a large impact on this satisfaction score and the general trend of patients being more involved in their care. I foresee a future of teamwork between the nurse, the anesthesia team, the surgeon, the patient and the family with a greater emphasis on education. As we monitor the election process we need to support those that support our ability to take the time and cherish every minute we have to teach our patients and keep them safe (here is the link to the Minnesota State Legislature: https:// www.leg.state.mn.us).

NEWS FROM ASPAN: REGION 3/CALENDAR OF EVENTS

I send you warm, summer greetings from your Region 3 Director. I hope that you are able to enjoy the warmth of the summer months and not have to suffer as you deal with humidity (as we experience in Northern Illinois)!

I must say that things have been rather quiet since returning home from National Conference. Of course, with the fall (all too quickly approaching) activity will ramp up. The RDs have been busy with phone conferences as we make plans for a stimulating, fun LDI. This year, LDI will be held in Cincinnati, OH. If you have not had the opportunity to attend LDI, I’d recommend that you contact your Component Leadership to see what you (and they) can do to support your attendance at this informational/educational session.

Region 3 is making a major impact with ASPAN activities: LDI this fall and National Conference next April/May. I am hearing from many ASPAN members that they are excited to visit this venue.

I am looking forward to visiting with you all during your Annual Meeting this upcoming September.

I encourage each of you to maintain (or begin) your activity with your Component. Contact your Leadership Team and volunteer to help. You don’t have to do anything big… Your leaders are looking for help with little things. Step up to the plate and take a swing at hitting a home run for yourself and your patients through your involvement with your premier PeriAnesthesia Organization!

Hello fellow members,

With the spring and fall conference, the MNDAKSPAN Board meets to conduct the business of our component. Below are the highlights from the meeting held on May 10th, 2016.

Strategic plan and goals for 2016-2017 reviewed:

A review of the MNDAKSPAN strategic goals and plans were reviewed. All Board members will be assigned to a goals that are aligned to their position with measurable criteria and due dates.

LDI (Leadership Development Institute):

ASPAN hosts an LDI every fall for new Board members to gain knowledge and insight on how to serve on a professional board. The current Board felt that sending any newly elected treasurer position should always be a priority. For this reason, Andrea will attend this year. The 2nd newly elected Board member voted to go should be aligned with the strategic goals our component is going to focus most heavily on that year. For that reason, Kristin will attend as newly elected Director of Certification.

Future Conferences:

With the closure of our current spring venue, the group reviewed options for future spring conferences with a focus of reducing costs while maintaining our members expectations of quality education. The partnership of having vendors at our conferences has been identified as valuable for our component. Vendors are able to support some of the cost of conferences and attendees are able to see new products available for their patients and network with peers. The Board would like to continue to expand this

partnership by offering a vendor reception following our education on Fridays.

A new benefit that the component would like to offer at conferences is an area for research and evidence based practice project poster presentations. The experience of being able to interact with the authors is second to none—what an opportunity for our members and ultimately the patients we care for.

A motion passed to sign a 1 time contract with Crown Plaza in Plymouth for the Spring 2017 MNDAKSPAN conference to be held in Feb.

$5 Friday Spring Conference Offering

The Board reviewed the feedback about the $5 Friday pre-conference event as well as the costs associated with hosting this part of the conference. A motion was made to increase the non-member fee to $25.00 to help off set the cost.

Executive Board Meeting: Summary of Minutes

August 2016

Hello fellow members,

Deb Moengen opened the meeting at the Crown Plaza Hotel in Plymouth, MN at 4:18 pm after a brief tour of the facilities of the hotel. This will be the site of the 2017 winter conference in February. All members present agreed that the location and facilities have all the amentieis to best suite the needs of our members with a decreased cost per person from previous conferences as well.

Progress on 2016-2017 Strategic Goals Up-date:The Executive Board reviewed the 2016-2017 Directors goals and the progress the individual directors present are making towards the goals.

ASPAN LDI Up-date:A decision was made to send the newly elected Treasure, Andrea Neumann, and newly elected Director of Certification to the ASPAN sponsored LDI (Leadership Development Institute) to be held in September. Increased certification is our focus goal for the year and the financial health of the organization is always a priority.

Budget Review:The Executive Board reviewed the 2016 budget with comparisons to the 2015 actual financial statements. Progress is being made with respects to decreasing conference costs, increasing revenue through the vendor fair at the winter conference and slight fee increase for the Friday conference event for non-members.This discussion also included a review of costs associated with the National Conference. No motions were made but each executive board member is asked to bring forward a proposal for the next full board meeting.

Bemidji Conference:The Executive Board reviewed the details for the Bemidji conference. We needed to increase the number of rooms blocked for the event which should translate to a higher attendance number.

February Conference:

The Executive Board reviewed the details for the February Conference. The Crown Plaza conference room can hold up to 250 attendee’s and has a good space for a vendor fair.

The meeting adjourned at 5:30 pm.

Contributions and Feedback to PAR Excellence

The key aspect of better representing MNDAKSPAN activities throughout the year is through member submissions to the newsletter. Even if it is just a short paragraph, an entire article, or a single photo, we warmly welcome all contributed submissions to the newsletter.

Ideas to consider:

• submitting an article or editorial • sharing photos from conferences or other member activities • debuting an excerpt from your research or evidence based practice project • sharing a successful program, strategy, or initiative Please feel free to contact the Director of Publication, at [email protected], with submissions, questions, comments or suggestions. Submission deadlines are the two

weeks preceding the earliest publication date.

At times, PAR Excellence will feature themes and submissions may be held for publication in those issues. MNDAKSPAN does not guarantee that articles submitted will be used in the publication. Authors will be contacted regarding whether the article will or will not be used. Thank you!
