Being a minus two (-2.00) myope, one for the first time enjoys a benefit: Build in reading glasses! One may, however get fed up lifting the glasses more and more often. Another target group are hyperopes, who may not warm up to the idea of having to wear glasses, even for distance or to relief them from headaches. Their dislike is similar to that of first time myope teenagers. Last not least, emmetropes or almost emmetropes do not want to show their age, by having to use half readers or alike. They will also end up putting and removing the glasses in too high a frequency. So they get fed up and look for alternatives. Multifocal contact lenses offer a lot of benefits, while may be not as equally good as multifocal or single vision (near) glasses, especially in challenging light conditions. This presentation witll review these various points of difference and and give tips for the fitting. Some wearers may use glasses for heavy near work, but will still enjoy the multifocal contact lenses in the spare time, when doing sports, travel etc. The option of a daily disposable version is then very appealing to them.

Abstract The central European population is aging and the median age will still go up over the next decades. With high penetration rates among the not yet but soon to be presbyopic contact lens wearers, it is clearly time to consider the fitting of multifocalsoft contact lenses as a standard procedure. Why? Just because the wearers do not want to give up on their contact lenses, just because they are now presbyope. The higher the distance correction, the more insisting they may become. The Opportunity

A big part of the Latvian population have just become or will become presbyope in the next 15 years (app. 500.000!)

Some of them are already contact lens wearers, most are not.

Some of them are emmetropes

The Opportunity

The ‘low-hanging’ fruit is:

- Keeping existing contact lens wearers in contact lenses when they become presbyope, certainly the spherical ones.

=> need to talk to them about the existence of multifocal lenses

But, also offer to all, especially emmetropes (opportunity of the occasional use):

- During the weekend, in the aerobic class, when doing sport ... The emmetropes are not ‘entrenched’ in using glasses, as e.g. myopes.=> They behave more like the first time teenage glasses wearers, most do not like them

The benefits of multifocal contact lenses Against the believe of many optometrists, especially the non presbopic ones, Multifocal glasses aren’t as perfect as these manufacturers say.

Today’s are much better than the older ones, but the are still not perfect

There is no perfect solution for presbyopia

Examples when not perfect:

- Doing near work that is above the head

- Walking down on steep stairs

- Working on a wide or several screens

- ...

The benefits of multifocal contact lenses

Less head movement (vertical and horizontal) for near work, but more importantly for intermediate (computer screen!), compared to multifocal glasses.

Can put computer screen at ergonomic heights, not the one ‘dictated’ by the glasses

No ‘bending’ of the floor, no ‘motion of the earth’ when moving the head

No blurry stairs when going down

Potential downsides of multifocal contact lenses

A bit less contrast, due to simultaneous vision

Somewhat lower visual acuity in case of uncorrected (residual) astigmatism

Deteriorating visual acuity when not blinking enough (surface of the lens dehydrates, deforms)

Somewhat lower visual acuity in low illumination

Fitting Tips

Understand the potential wearers

- Expectations (what do you expect from the lenses as such)

- Wants (where, when, how long, how often intend to use the lenses in detail, describe the tasks ...)

- Any specially close up work at certain times => may need support

- Motivation to wear lenses instead of glasses

- Key performance points (will not be happy if not fulfilled)

- Willingness to may have to go through iterations

When presenting multifocal / presbyopic CLs

• Be prepared to tell how simultaneous vision works / to answer the question of: How does this work?

• Do not believe or assume that multifocal spectacles do always work and have no weaknesses

– (think walking down steep stairs, lots of head movement at work or during spare time activities (dynamic vision), need for wide field of vision in intermediate and / or near

• Believe in multifocal contact lenses as a business!

Fitting TipsManage the Expectations! Communicate!

- They will work well at least 80 % of the time (like your multifocal glasses also do not work well 100% of the time)

- DO NOT USE THE WORD COMPROMISE!

- You will not be 20 again, sorry, but you will enjoy theses lenses

- You may benefit from some extra light once in while (nothing new to them)

- May need some help (half readers over the lenses) once in a while for very small or bad contrast print, close work (thread a needle ...)

- The first pair of trial lenses may already be a success. If not modifications, adjustments can and will be tried.

- Very small changes in the powers can make it a success, you and the patient need to be willing to try those steps

Fitting Tips

- Always make a new refraction.

- Never accept that they not need any correction for distance to not do a distance refraction. Small correction needs does not mean they need glasses for distance, but needs to be included in the determination of the multifocal lenses

- Make directional (hole) and sensitivity / sensory (+1.5) dominance tests

- When doing the eye exam with the bio-microscope, check if the pupil is centered to the cornea

Fitting Tips- Let the lenses settle (more important for new fits versus refits), ~ 10 min

- Use one or better two lines less than the distance VA for a start.

- Do not use a near acuity card with all the small print, use real world near materials. They will look a their watch, their mobile ... That is OK

Fitting Tips- Test under binocular conditions all the time, normal room light, not

dimmed - If distance VA is above drivers license and the wearer is OK with the

near, ask for a one week trial. - In that week wearer shall note where / when the lenses made her/him

really happy and where / when wants some improvement (what needs to be improved?)

- Computer use is more or less always OK, use this to re-motivate for next week of trial

Fitting Tips- If improvements are needed, follow fitting guide!

- Always work under binocular conditions, do not occlude an eye

- Do not work with a phoropter, work with single trial lenses or flippers

- Start with the distance (near or far) that requires improvement

- Do not lock yourself in by attempting to make your knowledge of dominant eye work, let the patients guide you. You offer the choices.

- Start with binocular changes first.

- Start with spherical changes first, touch add only at a later stage, if needed

Fitting Tips

- When aiming to improve distance / far vision, always start with binocular plus!Do not expect this to be a success all the times, but be surprised it can be!

- If improved in the required distance, try the opposite one

- If no improvements, test eye by eye, again start with the needed (far or near), and then test the effect on the opposite one

- Once you found a power change that works, delivers the desired improvements, show it again and get the ‘approval’ from the wearer. The apply the new trial lenses, let them settle and again ask for a one week trial.

Fitting Tips

- Do only one change per visit

- After three to four visits, ask for a decision between the two best options so far (usually one is bit better in distance, the other one in near)

- Remember the ‘Key Performance Point’. Which option delivers best on that?

Summary and conclusions

The European and also the Latvian population is ageing.

Existing contact lens users are becoming presbypic. They do want to carry on wearing lenses, if informed that multifocal contact lenses exist.

More or less emmetrope presbyopes are open for alternative corrections of presbyopia (they also ask about refractive surgery!). => need to tell that MF CL exist.

DD MF CL availability is a great tool! Occasional use / wear, different corrections for different times of the week, different activities.
