Activate your MyChart account using emailed Activation Code
The e-mailed letter will contain your required information including your Legal Name, your MyChart Activation Code, and your Medical Record Number (MRN).
From the MyChart Log In screen: 1. Click Sign Up Now.
2. Enter your MyChart Activation Code in the top line.
3. Enter the Medical Record Number from your Activation Letter, or listed on the After-Visit Summary from your last appointment. (########)
4. Enter your Date of Birth as shown (MM/DD/YYYY).

On the Security screen: 6. Create a MyChart ID that is secure
and easy to remember. Use the prompts shown to include any of the following: letters (it is NOT case sensitive), numbers, a period (.), an underscore (_), or the @ symbol.
7. Enter your Password twice to confirm it. Passwords ARE case sensitive and must be 6 characters or more and must contain at least 1 number and at least 1 letter.
8. Select a Security Question from the drop-down list provided.
9. Type in the Secret Answer to your security Question.
10. Click Next.
On the E-mail Options screen: 11. Select whether or not you wish to
receive E-mail Notifications. 12. If you select Yes, ensure your email is
correct in the fields. 13. Click Sign In.
On the Terms and Conditions screen: 14. Read the Terms and Conditions and
mark the box to not show the page next time.
15. Click Accept.

Add Personal Insurance
NOTE: If you do not have personal insurance, you can sign up for Student Insurance under the SHIPS program. Please contact the Insurance Coordinator at 319-335-9506.
To complete the Insurance Information: From the Home Screen: 1. Click Billing and Insurance. 2. Select Insurance Summary.

3. Click the Plus sign next to Add a coverage to enter new insurance information.
4. Enter the Insurance policy Name (i.e. Blue Cross, Aetna, etc.)
5. Enter the name of the Policy Holder NOTE: This may be your parent’s name if you are covered under a parent’s policy. 6. Enter the Policy Number from the
insurance card. 7. Enter the Group Number from the
insurance card. 8. Enter the Effective Date of the policy
from the insurance card. 9. When all fields are complete, click the
