How long have you worked @ NAHC? 22 years.

Tell us a little about your job: I take care of kids and pregnant ladies and new/young families. I supervise the most amazing team on the planet. Cathy Scott is the midwife we have once a week, Shamika Dokes, Regina Roman, Suzy Kim Tran, and Laura McLively make me look good every day. We work hard to support the next generations and those parents trying so hard to raise them right.

What is the most enjoyable part about your job? I enjoy caring for families and welcoming new babies into the world. I love my team mates: laughing and crying with them and working it together.

What do you do to relax on the weekends? Cook, read, walk/exercise with Rose our black lab each day, church activities, hang out with hubby Dan and precious daughter Leah.

What is your ideal vacation? travel the world with extended stay in Ivory Coast, West Africa where I used to live and work.

What three things would you take on a deserted Island? Bible/Dan/Leah

Jerri (top Left) and her OB Team

Unusual Occurrence Forms:

What are they?

They are Forms found:

S:\_Shared Bulletin Board\Forms and Instructions used by Employees to report incidents. Employees are required to immediately report any

unsafe condition or hazard that they discover in the workplace to their

supervisor via the NAHC Unusual Occurrence/HIPAA Incident Report and the supervisor will then forward a copy of this report to the either your

supervisor, a witness, or the Executive Director. No employee will be disciplined or discharged for reporting any workplace hazard or unsafe

condition.

Employees who wish to remain anonymous may report unsafe conditions or hazards by submitting Incident/Accident report to the Executive

Director without identifying themselves.

Why are they used?

Documentation is a very key aspect to healthcare for liability purposes.

Who stores & maintains the records?

The Executive Director maintains an Unusual Occurrence log as well as

corporate files in which all incidents are documented.
