The key difference between Honor Roll and National Honor Society is that Honor Roll is met by grades alone…..National Honor Society membership includes scholastic, character, leadership and service components.You have met the scholastic and character requirements for NHS membership….now you must show how you fulfill the leadership and service requirements.

•Pay $10 dues yearly•Turn in a signed Service Contract form yearly•Attend 6 out of 10 monthly NHS meetings and stay informed with the agendas and postings on the EVHS/NHS website•Complete and turn in a Service Hour Form per semester (5 Individual Hours & 5 Group NHS Service Hours are required per semester)•Maintain your GPA

Make a copy of your Student Information PacketDue to overwhelming demands, NHS Sponsors will not make copies of Student Information Packets for scholarship applications, service hour cords, etc. regardless of selection or non-selection

Please make sure you retain a copy of the packet for yourself before you turn it in to the NHS Sponsors.

No late forms will be considered.Completion of this form does not guarantee selection. No late forms will be considered. Should it be determined that information on this form is incorrect or has been falsified in any way, the faculty council may rescind their invitation to apply or to join EVHS NHS, or they may dismiss a member without a probationary period.

Student Information Packets are due to one of the NHS Sponsors by 4:00PM on Tuesday, April 17th. No late forms will be accepted.

Part IPART I: All activities require a signature of a sponsor, director or supervisor. A parent signature is not sufficient unless proof is provided that the parent is the sponsor, director or supervisor. Photocopies of certificates, letters or other forms of verification may be provided for proof of participation in lieu of adult sponsor signature. If a sponsor is no longer associated with the district or on the high school or 9th grade campus, see the NHS sponsors before the due date. Refer to pg. 9 for point designation. If you need more space, please attach an additional sheet, make sure to put your Form # in the top left corner and follow the table format.

Part IIPaid employment during the school year (not to include summer).

Part IIIPART III-Community Service-This includes volunteer work performed during the school year and the summer. Service to consider includes church and civic organizations, Eagle Scout/Silver Award projects, tutoring (not a sibling), coaching non-school sports, participation in non-school drama/musical presentations, internships (not paid), voluntary service projects and other community involvement. Community Service hours must take place OUT OF SCHOOL day (i.e. HST hours at R.O.C.K. or PALS hours at various schools during school time MAY NOT be included in Part III).

Part III ContinuedIf you participated in volunteer work that did not occur regularly during the 32 week school year, estimate the number of hours you actually worked and spread them over a 32-week school year to assign points. The formula for this calculation would be the number of hours divided by 32. For these activities, please show that you divided the number of hours worked by 32. (Note-some point values may be in fractions -.5, .75, .33)
