Medication errors are killing more people than heart attacks or cancer, according to new research.

Pharmacist Ken Lee is lobbying for more communication between doctors and pharmacies after his research found one in three prescriptions featured mistakes.

Mr Lee this week released his book, How Safe is Your Prescription, at the Australian Pharmacy Professional Conference on the Gold Coast.

His study found about 190,000 hospital admissions a year are related to medicines and their side effects while the inappropriate use of medicine costs Australia about $600 million a year.

Mr Lee studied 21,733 consultations in Australian community pharmacies between August 2008 and July 2009 using the Chemconsult prescription safety check system.

He said errors could easily be avoided if patients talked to their pharmacist about the uses of their prescribed medicine.

And Mr Lee blamed dosing errors, allergic responses and drug interactions as other common mistakes.

Mr Lee’s research comes as Australian data on medication errors and adverse drug events suggests up to 3 percent of all hospital admissions are related to problems with medicines and, of those, 43 percent are potentially preventable.

Australian Incident Monitoring System figures show 10 percent of medication incidents occur as a result of dispensing errors.

Pharmacists have reported that two of the most common types of dispensing errors are selecting the incorrect strength and selecting the incorrect product, with major contributing factors being ‘look alike’ or ‘sound alike’ errors, high prescription volume and interruptions.

New figures have revealed Melbourne’s busiest emergency departments turned away ambulances for more than 4300 hours in a six-month period last year.

Victorian health minister David Davis released a report which also shows the city’s 13 biggest hospitals spent a staggering 3052 hours on the Hospital Early Warning System (HEWS) – an unofficial form of bypass.

The Austin, Frankston and Northern hospitals were all on bypass for almost 170 hours each while Monash Medical Centre had 131 hours on bypass and 573 on the early warning system.

The system is used to alert paramedics that a hospital is almost full and should divert non-urgent patients.

The report is the first time the HEWS data has been included with the bypass figures.

“There has been some incentive to classify things as HEWS, which is hidden, as opposed to bypass which is public,” Mr Davis told media.

Mr Davis promised the government would strive to “do better”.

“The first thing is to get these facts on the table and then hospital managements, network managements, can start to do the work to steadily improve.”

The figures come as the Ambulance Employees Australia Victoria union, in its budget submission, stated Victoria needs an extra 266 paramedics, 35 more ambulances and seven new ambulance stations – at a cost of $30 million - to cope with demand.

The union has repeatedly called on the government to invest heavily in the sector, with general secretary Steve McGhie claiming it would reduce pressure on paramedics while bolstering ambulance response times.

The new Victorian Government has promised to spend $151 million on 320 extra paramedics, including 210 in country areas, across the next four years.

Statistics show Victorian ambulances treated 748,834 people in 2009/2010 - up by more than 34,000 people in the previous year.

TRAVEL NURSES FIND HOME AT CONTINENTALContinental Travelnurse is the UK’s premiere travel nurse agency. Founded in 2000 and built with a staff that boasts decades of experience in the field, Continental Travelnurse has established its excellent reputation by placing only the best nurses from Australia, Canada, New Zealand and the US in assignments throughout the UK.

The high standards we have for our nurses allow for successful placements in some of the UK’s top facilities. St. Thomas & Guys Hospital in London, Cheltenham General and Gloucestershire Royal in the Cotswolds, Royal County Sussex Hospital in Brighton and The John Radcliffe in Oxford are among the respected names Continental Travelnurse has done business with over the past decade.

Continental Travelnurse boasts a string of excellent benefits that allow nurses to work in the UK and visit exciting destinations throughout Europe while making money and having the adventure of a lifetime. We offer up-market fully furnished and conveniently located company housing or a housing allowance. You’ll receive travel money, registration assistance, continuing education allowance and RCN membership. You’ll have the security of being rostered full time for each three month assignment and the huge bonus of being in the staff uniform so Continental Travel nurses will never be mistaken for an agency nurse brought in on a day-to-day basis.

What does this mean in practice? It means coming to work and play in Europe has

never been easier for RN’s. Company housing means housemates who are sharing in on the same adventure so you’ll likely have a new travel buddy and a friend for life. The housing is always located in a nice area with easy access to work. Travel nurses enjoy higher pay than the staff they are working with but also receive great respect – they are welcomed and valued because they’re a part of the team contributing to the continuity of care.

Nurses spend months in wonderful locations, exploring all the area has to offer in arts, tourism, shopping and fun. Three months is the ideal time. It’s long enough to get to know a place, but not too long so you’re never bored. Often, travel nurses will head off on short hops to Europe during the week on their scheduled days off. (Air fares are incredibly cheap.) When an assignment is completed, many nurses use some of the money they have earned to explore Europe for a few weeks or even months. Then they happily sign on for another three month stint and get a chance to reconnect with a hospital and location they have grown to love or discover another part of the UK and all it has to offer.

It is no surprise a growing number of nurses from around the world are using their skills to take this amazing opportunity to combine work with living and traveling in the UK and Europe. Continental Travelnurse can make that happen.

Tasmanian aged care workers will consider a revised pay and conditions agreement from OneCare after last week’s four-hour work stoppage.

More than 460 workers across four OneCare nursing homes are lobbying for better pay and to maintain conditions ranging from paid meal breaks to personal leave.

Health and Community Services Union acting secretary Tim Jacobson said last week’s stop work at New Norfolk’s Corumbene nursing home had prompted OneCare to step up to the negotiating table.

Mr Jacobson said other planned stop work actions had been shelved while members consider the agreement.

“Members will decide this at membership meetings this week but no further action will take place should the revised offer be acceptable to members,” he said.

OneCare also operates Bishop Davies Court at Kingston, The Manor in Launceston, Umina Park in Burnie and the Rubicon Grove nursing home at Port Sorell.

Staff at OneCare homes at Kingston and Burnie also stopped work last month.

A OneCare statement said the organisation offered salary packaging, enabling aged care employees to pay their bills from their salary before tax, which also raised the hourly rate to $19.04 for a 20 hour week.

The Australian Nursing Federation has called on the Federal Government to inject $513 million into the aged care system to close the wages gap between aged care and public hospital nurses.

ANF federal secretary Lee Thomas has spoken at the Productivity Commission hearing in Melbourne as a group of more than 50 nurses, assistants in nursing and other union members staged a rally outside.

Ms Lee said the sector is struggling to attract employees due to a wages gap of almost $300.

“A workload management tool is needed to keep track of the hours nurses are working and to provide an indication of staffing needs in each facility,” she said.

“There should be flexibility in rosters to make the sector attractive to nurses coming back into the workforce who are looking for a work/life balance.

“A career structure that offers nurses and assistants in nursing opportunities for progression would also aid recruitment and retention.”

Ms Thomas said one registered nurse should be allocated to care for four residents per day shift, covering assessment, care planning and provision, along with complex and basic care ranging from showering and assistance with two meals to dressing and medications.

“The ANF is seeking a minimum of 4.5 hours of nursing care per resident per day,” she said.

The Federal Government has come under fire for an educational cash splash on aged care nursing despite those nurses being up to $300 a week worse off than their public hospital colleagues.

Federal Health and Ageing Minister Mark Butler has unveiled a new round of postgraduate and continuing professional development scholarships in aged care nursing.

The release of 130 scholarships, valued at up to $30,000 each through the Royal College of Nursing Australia, comes as the Australian Nursing Federation steps up its fight for the government to spend more funds on closing the wages gap between aged care nurses and those at public hospitals.

An ANF spokeswoman said while the union welcomed funding earmarked for improving the skills and education of the aged care workforce, it was no substitute for fixing some of the sector’s systemic problems such as wages and ratios.

“These measures will have an enduring impact on the quality of care we provide to older Australians in the future,” she said.

Figures show the Federal Government spends just 0.8 percent of gross domestic product on aged care, well below that of other developed nations.

The statistics, released by the Combined Pensioners and Superannuants Association of NSW, shows Norway spends 2 percent of GDP on aged care while the Netherlands spends 3.5 percent and Sweden spends 3.6 percent.

In announcing the scholarships, Mr Butler said eligible study areas include clinical gerontology, behavioural management, dementia care, wound care, continence care, palliative care, infection control and leadership and management.

“Having access to further training and career advancement makes aged care a better career option and makes it easier to attract and keep quality staff, which in turn means better care for our increasing population of older Australians,” he said.

Scholarship applications close April 27, 2011. For more information visit www.rcna.org.au

A renewed push for supermarket giants to operate pharmacies is misguided, according to the Pharmaceutical Society of Australia.

PSA national president Warwick Plunkett said any move to allow supermarkets, such as Woolworths and Coles, to run pharmacies would ignore the contribution pharmacists play in delivering better health outcomes.

“Such a leap of logic is entirely baseless and experience overseas shows that it is quite false to believe there are major cost-saving benefits in having pharmacies in supermarkets,” he said.

“The cost of prescription medications is regulated by the government so there can be no savings on these through supermarket channels.”

Mr Plunkett said the onslaught of discount pharmacies had already introduced a cost-competitive retail environment into the community pharmacy sector.

“However, in the discount model, the health care aspect and the provision of professional services are often not as apparent as they might be.”

His comments come after Australian Financial Review economics editor Alan Mitchell this week criticised the current pharmacy model, labelling the profession “highly protected” and “inefficient”.

The statement comes just eight months after Woolworths dropped its two pharmacy trademarks.

Staff and almost 200 families at Sydney’s Liverpool Hospital are being screened for tuberculosis after a hospital health worker was diagnosed with the potentially fatal disease.

The hospital, in the city’s south-west, has revealed a worker in the maternity and neonatal units may have exposed 140 colleagues, 187 families and nine babies to the disease between December last year and March 6.

The NSW health department said the health worker is no longer at work and is receiving treatment and has advised there is no ongoing risk to patients and staff at the hospital.

The hospital has begun contacting parents of babies born in its maternity unit and cared for in the Neonatal Intensive Care Unit (NICU).

Tuberculosis screening and preventative medication will be available for all babies in the NICU and delivery suite who may have been in contact with the health care worker during the infectious period while screening and counselling will also be offered to parents.

Health care workers who worked in the NICU and birthing units during the period of exposure will also be screened.

Liverpool Hospital Chest Clinic Professor Guy Marks said the risk of a baby being infected with tuberculosis was low but it was still important that precautionary measures were taken.

Last year, aged care nurses revealed that mistakes are being made within the aged care sector, revealing all to the media for fear that it was only a matter of time before there is a major incident. So has anything changed in 12 months? Nina Hendy investigates.

Nearly a year ago, the Nursing Federation revealed the extent of the pressure being placed on the aged care system when it told the media about a situation in which a graduate was left in charge of a nursing home in her first week on the job.

It was a troubling scenario that made national headlines, raising questions about what was going to be done to ease the pressure on the system.

That was nearly a year ago now, but it doesn’t appear that anything has changed so far.

The NSW Nurses’ Association (NSWNA) confirms that workloads and wages remain the biggest issues facing aged care nurses. “Wages are part of the issue, but a lot of nurses report to us that they feel they don’t have the time or capacity within their role to do their job properly,” Stella Topaz, professional officer, NSWNA says.

“In fact, there have been satisfaction surveys that cite that workload issues are an even bigger issue than wages.”

Topaz says that the last survey of aged care nurses was completed in 2007. It showed that there was an 18% drop in registered nurses nationally.

“However a lot of nurses really love their work and are committed to what they do, but struggle with it because they feel stretched to get everything done, and therefore feel compromised,” Topaz says.

But perhaps change is on the way. The aged care sector is pinning its hopes on a Federal

Government productivity commission, which, depending on what is adopted, could see change implemented later this year.

The productivity commission comes as the industry grapples with some difficult statistics. Among those is the fact that the number of Australians aged 85 and over is projected to increase from 0.4 million to 1.8 million (5.1 per cent of the population), by 2050.

The statistics also show that while further advances in the management of some diseases are predicted, a greater number of Australians will require complex care for dementia, diabetes and other illnesses associated with longevity, as well as palliative care.

The commission’s draft report, which was released in January this year, found that the aged care system suffers several key weaknesses. These include difficulties in navigating the quantity of services, the fact that quality can be variable, there are gaps in service coverage and only limited choices for care recipients.

The commission’s proposals aim to address these weaknesses and challenges, and promote higher quality care. The proposed reforms would give older Australians a simplified gateway for information on aged care, be able to choose, where feasible and appropriate, to receive care at home or in a

residential facility and contribute in part to the cost of their care, among other recommendations.

Trish Noakes, director and executive officer of privately owned aged care provider Just Better Care Australia made a submission to the productivity commission. She is among those that hope that change is on the way. Just Better Care, which offers 15,000 hours of service a week, can find it difficult to recruit appropriately trained staff.

Noakes says she wants the changes to the system to be realistic. “No-one doubts that wages are too low. People can’t pay their rent or mortgage when they’re earning $20 an hour – it just doesn’t happen. Particularly given our workforce is expected to face one challenging situation after another,” Noakes says.

She hopes that the proposed change that moves the control of care to the individual rather than the care provider comes to fruition. “We find a lot of people don’t like to complain about their care because they worry they will get less care.”

If the productivity commission’s recommendations are adopted, some of the numerous changes could be in place by the end of 2011.

In the meantime, all eyes will be on the outcomes of the productivity commission. Final submissions were due by 21 March, 2011 and the outcomes are slated to be released by the end of June, 2011.

Up to 500 Christchurch aged care workers have lost their jobs in the wake of the region’s devastating earthquake.

Nurses, carers and support workers have been hit with redundancies at five aged care centres following the 6.3 magnitude quake that rocked the city on February 22.

New Zealand’s largest aged care business, Oceania Group, has revealed it will close two condemned sites at Woodchester in Shirley and Windermere in Papanui affecting 250 staff while three other aged care facilities have also been closed by the quake.

New Zealand Nurses Organisation coordinating organiser Lynley Mulrine, who lives and works in Christchurch, said the Oceania Group move was another knock for workers and residents in the embattled area.

“This is a huge blow to the sector and will result in a devastating loss of skills,” she said.

“These ‘beds’ will have to be returned to the region through re-building and the skills (are) an important part of that.”

Ms Mulrine said Oceania Group’s decision, which the organisation states will see some staff redeployed and others made redundant, was “extremely disappointing” given Canterbury District Health Board had guaranteed funding to employers who evacuated residents due to safety concerns.

“For workers to suffer the devastating effects of the quake and then lose their livelihood was a tremendous blow to them and their families and we now learn that this was avoidable,” she said.

“It’s a heartless approach from a large employer whose livelihood comes from government funding.”

Ms Mulrine said employees covered under the Collective Agreement and members of either NZNO or the Service and Food Workers Union were entitled to redundancy payments, however those who had worked for less than 12 months would miss out.

She said many of the workers at the other three aged care facilities had no entitlement to redundancy.
