SupportLinc offers confidential, professional support and the following resources, at no cost to you and your family:* Short-term counseling Receive up to three in-person counseling sessions with a licensed clinician to address issues such as depression, stress, relationship problems, grief, substance abuse, anxiety, work-related pressures, anger management, and other emotional health concerns. Legal services Obtain a 30-minute telephonic or face-to-face consultation with a local attorney. Expert referrals Knowledgeable specialists provide referrals to resources that help address issues such as child and elder care, adoption, pet care, home repair, education, and housing needs. Freestanding ERs (FSER)Sometimes referred to as urgency centers, FSERs can be 2x the cost of an ER and 20x the cost of an Urgent Care Center. FSERs are able to treat similar conditions as an ER but cannot admit patients – they are neither located in nor attached to a hospital.

Tips for Aetna Dental plan membersPlease note that while medical and pharmacy benefits changed to UnitedHealthcare on September 1, 2019, dental benefits are still with Aetna. Additionally, Aetna does not issue physical dental ID cards but would have mailed you a welcome letter when you initially enrolled. If you would like to print a dental ID card, you may do so by logging in to your member account at aetna.com. Aetna also has a handy app compatible with most smartphones and tablets.

If you are enrolled in the DMO plan, please recall this plan requires you to designate a primary care dentist (PCD) to receive care, and each family member

enrolled in the plan can have a different PCD. Aetna can tell you which dentist is designated, or you can look up this information in your member account at aetna.com.

The phone number for Aetna Dental Member Services is 1-877-238-6200 (TTY: 711), and the Conroe ISD group number is 737387. Member Services can help you locate providers, check claims, and more. For information about your plan’s benefits, please refer to the plan documents, which are available on the plan documents page of the CISD Benefits Office website (www.conroeisd.net/department/hr/benefits/).

Wipe out the fluA yearly flu shot is your best way to avoid or weaken the flu. If you are a Conroe ISD medical plan member with UnitedHealthcare, you can receive a free flu shot from a network doctor, convenience care clinic, or retail pharmacy. To find a covered provider, log in to your member account at myuhc.com.

Making changes to your benefitsNormally, you may choose or change benefits only during annual enrollment. However, when certain life events occur (marriage, birth, employment changes, etc.), you can make changes to your benefits.

To make coverage changes, you must contact the CISD Benefits Office no later than 30 calendar days after the event date.

Newborn

To ensure your newborn is covered, you must submit a change request to the CISD Benefits Office by the 31st day of life, even if you already have family or child coverage.

Email and voice messages sent to the CISD Benefits Office are time- and date-stamped, so be sure to leave a message if no one is available or the office is closed.

Eric Munchrath MSN, RN, FNP-C, LP

Eric is a family nurse practitioner serving patients of all ages. He is a graduate of The University of Texas at Arlington and the University of Texas Health Science Center in Houston.

Eric worked as a paramedic and an emergency room nurse before becoming a family nurse practitioner. In every role, Eric has enjoyed taking care of people of all ages. He sees patients for chronic disease management, including hypertension and diabetes, as well as acute illnesses such as sore throats, aches, and sprains, among others.

With his experience in emergency medicine, Eric has unique training in treating a variety of conditions. He enjoys creating an individualized plan of care for his patients and walking with them each step of the way. With a warm and personable demeanor, Eric makes sure his patients are comfortable with their plan of care. He takes the time to listen to each concern and wants to see patients feeling their best. For him, the best part of practicing medicine is getting to help people.

In his spare time, Eric enjoys spending time with his family and exploring technology.

Andrea Scace MSN, RN, FNP-BC

Andrea is a family nurse practitioner who received her Bachelor of Science in Nursing at Indiana-Purdue University Indianapolis, and her Master of Science in Nursing at the University of Cincinnati.

Andrea sees patients of all ages with a variety of primary care needs, from acute illnesses such as colds and the flu, to chronic diseases, and physical exams. Some of her clinical interests include health promotion, disease prevention, and disease management. She strives to establish open communication and trust with her patients, and encourage them throughout their wellness plan.

In her spare time, Andrea enjoys spending time with her family and being outdoors.

Claudia Castillo MSN, RN, FNP-C

Claudia received her Bachelor’s degree and Master of Science in Nursing – Family Nurse Practitioner at The University of Texas Health Science Center at Houston (UTHealth) School of Nursing.

Claudia worked as an EMT and an emergency room and ICU nurse before becoming a family nurse practitioner.

Claudia sees patients 6 months of age and older and is fluent in Spanish. She treats patients for a variety of primary care needs, from acute illnesses, to chronic diseases and wellness exams. Some of her clinical interests include diabetes prevention and management as well as preventive care. Claudia takes a holistic approach to patient care and strives to spend as much time as possible educating her patients on how they can improve their overall well-being.

In her spare time, Claudia enjoys spending time with her family, crafting, party planning, and swimming.

Employee Health & Wellness Center reminders• Cost is $10 for Conroe ISD medical plan members and $50 for all other

Conroe ISD employees.

• Services provided (all ages): physicals, screenings, and acute episodic care for common illnesses and injuries such as seasonal allergies, coughs, colds, flu, fevers, and infections.

• Services provided (ages 7+): primary care, vaccinations, lab work, and chronic disease management for conditions such as diabetes and high blood pressure.

• Appointments are preferred and should be scheduled in advance for physicals and well-person, follow-up, and chronic care visits.

• Walk-in patients for sick visits and acute care needs will be taken up to 45 minutes prior to closing time, or when the clinic reaches maximum capacity. We encourage patients to call ahead for availability to promote a better experience for all.

• Individuals enrolled in a health savings account (HSA) are not eligible to receive services from the Center.

• The Care Manager Program ended August 31, 2019. Contact the CISD Benefits Office for alternate resources available through your medical plan.
