Supporting documents are required for Utility & Municipal Agreements to be completed by RUERelocation of water main owned by Suffolk County Water will be required by DB teamLIPA to provide a cost estimate for their work to the RUE, once all supporting documents are provided by the DB Team. The RUE will complete the AgreementNYSDOT will not incur added costs for undergrounding aerial utilities

NY Route 347, Design-Build Phase 2 Quality Control (QC)
QC is Design-Builder’s Responsibility
Design-Builder shall appoint a QC Manager responsible for developing QC Plan for both Design and Construction
Design QC shall be by the Designer in accordance with the QC Plan
Construction Inspection shall be by an Independent Engineering Firm (Hired by the Design-Builder)
Materials Testing shall be by an Independent Testing Firm or Laboratory (Hired by the Design-Builder)
The Materials Testing Firm shall report to the Construction Inspection Firm
Offsite Plant Quality Control will be performed by the Design-Builder

NY Route 347 – Design Build Quality Control (QC) Cont..
If the Design-Builder becomes aware of an apparent error, omission or ambiguity in any of the Contract Documents or of an apparent conflict, the Design-Builder shall notify the Department’s Project Manager in writing of any such error, omission, ambiguity or conflict.

Design and Construction QA is Department’s ResponsibilityThe Department may perform QA on any of the Design-Builder’s activities, including Offsite Plant QA The Department may utilize an independent Consultant to perform QA of the Design-Builder’s DesignsThe Construction QAE will be Department staff.

D/B Team to prepare SPDES Storm Water Pollution Prevention Plan (SWPPP) and an acceptable Notice Of Intent (NOI) consistent with NYSDEC and NYSDOT procedures.
NYSDOT will obtain the NYSDEC Fresh Water Wetlands permit based on the conceptual 30% plans.

NY Route 347, Design-Build Phase 2 SPDES General Permit
D/B Team to prepare SPDES Storm Water Pollution Prevention Plan (SWPPP) consistent with NYSDEC and NYSDOT procedures.D/B Team shall design drainage system to treat Storm Water Quality and Quantity as per NYSDEC and NYSDOT guidance documents.D/B Team to prepare an acceptable Notice of Intent (NOI) for NYSDOT signature/submission to NYSDEC.The DR/FEIS drainage information can be used as a design “starting point” but shall not be used to calculate final design flow volumes and storm water treatment practices.

NY Route 347, Design-Build Phase 2 NYSDEC/ USACE Fresh Water Wetlands Permits
NYSDOT will obtain the permits based on the conceptual 30% plans prepared by the Department. The permits may include conditions requiring the submission of final detailed plans after preparation by the D/B team.If the final plans are not consistent with the DEC permit conditions, the D/B team shall submit the plans to NYSDOT/ NYSDEC for review. NYSDOT/NYSDEC reserves the right to reject any plans that are inconsistent with the approved permits.
