Labette Health and Labette Center for Mental Health Ser-vices, in partnership with the Kansas Foundation for Medi-cal Care (KFMC,) will offer a two free workshop series for people living with chronic conditions and/or diabetes as well as their caregivers. The workshops are highly interactive and promote the de-velopment of skills necessary to lead a healthy life with chronic conditions. The workshop series is designed to complement medical treatments and education provided by healthcare providers. People living with physical and men-tal chronic conditions and their caregivers who attend the workshop series will receive a free copy of the book, Liv-ing a Healthy Life With Chronic Conditions.

Two classes will be starting in October, Chronic Condition and Diabetes. An informational meeting for the Chronic Conditions Lead Your Health workshop will be held on October 6th at 2pm at the Rector Diabetes Education Cen-ter, Building G, at Labette Health. The workshop will then run from October 13th - November 17th. This workshop will be presented by Melissa Lunsford and Trinette Vance.

Over a Year in and Health Homes Showing Success

Labette Center has now been providing health home services for over a year. On August 1st, 2014 the Center embarked on a journey that was completely unchartered for the state of Kansas at that time. The creation of Health Homes was a new and exciting prospect with many un-knowns attached to it. But over a year in and the program has shown remarkable success.

Through the hard work and dedication of the five person team that make up the Health Home, the program has managed to not only successfully charter the ups and downs of creating something new, but accomplish great strides in the process.

Labette Center’s Health Home is considered one of the top health home programs in the State. Input from the coordi-nator of the program, Deb Williams is being used, along with only two other programs, to assist the MCO workgroup in creating a manual which will be adopted by the state. Williams also was honored to speak in regards to health homes at both learning collaborative and the first Annual Health Homes Conference.

At this point, the program has completed five “Lead Your Health” courses, in conjuction with Labette Health. These courses are community based and serve not only individuals who are part of the health home, but others who are not. The “Lead Your Health” course has two versions, one for chronic illness and one for diabetes. Courses are held at the Rector Diabetic Center located at Labette Health and presenters include not only Health Home staff but also Shellie Collins, RN from Labette Health. These courses have been instrumental in help individuals learn how to deal with and adapt to these illnesses.

Health Home staff have shown great success rates with clients that are served, with an approximate 70-75% rate of engagement. The staff assists with doctor’s appointment, gaining various services throughout the community, transi-tional care, hospital care, home care, medication needs and much more.

By collaborating with other entities’ within the area, such as medical personnel, hospitals and home care agencies, the staff of the LCMHS Health Homes has seen the program soar beyond expectations. “It’s wonderful to see individu-als getting services they weren’t receiving before, whether because they were unable or didn’t know about them,” stated Health Home Coordinator Deb Williams. “I’m very proud of the staff. They are the heart and soul of the health home.”

LCMHS looks forward to many more successes with the Health Home in the future, and look forward to helping many more individuals find happy, healthy lives. Through this program, as well as all the services the Center is able to provide, it is a joy to give ‘Help for Today, Hope for Tomorrow’.
