Patient sees GP
Hilda sees her GP with shortness of breath and worsening chronic obstructive pulmonary disease (COPD). Her GP, Dr Allenby, feels that all practice-based treatments have been exhausted and now needs specialist input into Hilda’s care.

GP refers patient for test
Hilda wants to be seen locally. Dr Allenby types “COPD” into the NHS e-Referral Service and is prompted that the local services require breathing tests. He therefore fills in the appropriate electronic referral template.

Patient books appointment on her Smartphone
Hilda asks to have her appointment booking information sent to herSmartphone. She follows the instructions and downloads the NHS e-Referral Service app, which she is then able to use to book her appointment and receive the confirmation.

Patient attends appointmentTim Williams, the respiratory technician, reviews and accepts the referral.Hilda attends the appointment and has the breathing tests. The result is uploaded to the NHS e-Referral Service and Dr Allenby receives a notification.

Dr Allenby telephones Hilda and explains that the results confirmworsening COPD; he advises referral to a respiratory specialist. Hilda says she would like to be seen at the same hospital.

Once again, Hilda wants the information to be sent to herSmartphone and she books the appointment via the NHSe-Referral Service app.
