In contrast to other surgeries, Natural hair trans-plant procedures rely on a physician to perform surgery, a designer to design ahairline and technicians to prepare and transplant hair grafts. Natural Look Hair Transplant Technology

The Natural Hair Transplant procedure is nearly painless. For those patientswho want an almost completely painless procedure, Dr. Chang has invented aPainless Natural Hair Transplant procedure called Twilight Anesthesia. Thisprocedure has been presented to a major hair restoration conference and acceptedas a unique and painless procedure.

Natural Grafts Procedure

The unique Natural Grafts procedure transplants hair in their natural follicularunits available in each patient’s donor area. We do not graft randomly.

Safety is our main concern. Most of the tools we used are disposable. Non-disposable equipment is sterilized twice, first in a chemical solution and then in apressured steam heater autoclave. During the surgery, pulse, oxygen saturationand blood pressure are constantly monitored. Our clinic is also equipped withEKG, and a respiratory monitor.

In 1939, Dr. Okuda (aJapanese dermatologist)founded the principle ofhair transplantation. Fordecades, his technique forhair transplants did notchange until 1990. At thattime, during each proce-dure, hair transplant sur-geons were only able totransplant between eightyto one hundred 4 mm sizedpunch grafts. The resultwas unacceptable, resultingin what people call a "cornrow" appearance. This wasthe First Generation ofhair transplantation.

In the early 1990s,it was discovered that hairtransplant surgeries withsmaller grafts producedmore natural looking hairgrowth. With smallergrafts, each hair receives anincreased blood supply, incomparison to those withthe larger grafts. The re-sults were more natural,and the regrowth rate aftersurgery improved greatlyfrom the previous style ofsurgery. This discoverywas soon followed by im-provements in the instru-ments, techniques, and an-esthesia used. Within oneto two years, some sur-geons were even able totransplant thousands ofgrafts in one session, farmore than the 80-100 grafts

done before in previoussingle surgeries. This isthe megasession anddense packing hair trans-plantation. We call it theSecond Generation ofhair transplantation; itis the generation ofQuantity.

In that world,some surgeons onlyraced to achieve themaximum number ofgrafts in one session.One day, someone mighttransplant 1000 grafts;the next week, anotherdoctor would manage totransplant 1500 grafts;the next month, some-body else would trans-plant 2000 grafts, oreven up to 4000 grafts inone session. Nowadays,most of us can do asmany grafts as we want.We are only limited bythe maximum amount ofdonor hair that we cansuccessfully removefrom the permanent hairat the back of the scalpwithout leaving a visiblescar. (The size of thescar is dependent on theamount of tension at thesuture site.)

But we must con-sider several things.What is in the patient'sbest interest? During themega session , how canwe decrease the possible

hair follicle damage? Asyou know, a hair trans-plant only relocates exist-ing hair; it does not regen-erate any new hair. There-fore, if we damage a folli-cle, we lose one hair. It isextremely important toprevent any damage to fol-licles; however, the moregrafts you cut, the moreopportunities there are todamage them.

In 1995, Dr. JamesArnold started to teachphysicians how to cut highquality strips with a multi-ple-bladed knife. In 1996,Dr. Limmer and Dr. DavidSiegel began to use astereoscope to cut stripsfor better grafts. Thistechnique helped preparethe way for a new genera-tion of hair transplant sur-gery. At this point, theThird Generation of hairtransplantation began -- itis the generation of Qual-ity and Quantity.

We at the NaturalHair Transplant MedicalCenter realized that, in ad-dition to mega sessions,there needed to be a wayto decrease the damagerate of the grafts, and wesearched for many differ-ent ways to minimize fol-licular damage and im-prove hair transplant yield.

hair. In addition, wetake digital picturesfor each patient andinput them into thecomputer. With theMirror Image Soft-ware, the total areaof coverage can beprecisely calculated.

• We use an adjustablemulti-bladed knife toharvest the donorstrips. The firstmulti-bladed knifeused was straight.Because the donorsite has some degreeof curvature, the useof a straight-bladedknife resulted in toodeep of a cut at thelower strips. Then,th e f i xed -an glemulti-bladed knifewas invented. How-ever, everybody'shair grows at differ-ent angles. There-fore, the fixed angledmulti-bladed knife isnot suitable for eve-ryone. In 1996, theadjustable bladedknife came out onthe market. We cannow adjust theblade's angle in ac-cordance with the pa-tient's hair angle.This results in muchbetter strips, and pre-vents the possibility

of cutting too deeply,which can damagenerves and vessels.

• To cut better grafts,we use the Backlight, stereoscope,and video visualizer.The video visualizercan enlarge an imageof the strips up totwelve times onto atelevision screen.The technician canthen easily observethe screen and cutexcellent qualitygrafts. This screen isespecially helpfulwhen cutting whitecolored hair grafts,which are normallyvery difficult to see.Without the videovisualizer, we wouldhave to rely on in-stinct or simplyguess when cuttingthe grafts; with thevisualizer, however,we can even reversethe color of the im-age, making thewhite hair more visi-ble.

We can now suc-cessfully conduct hairtransplants without over-harvesting too much hairand without causingmuch damage to thedonor site, so that the do-nor area looks as full as

The preparation of hairgrafts is identical in bothconventional and laserhair transplantation. Thedifference is that laser hairtransplantation uses a ma-chine that shoots a beamof high energy light burn-ing slits or holes in thescalp, while cold steel isused to make slits conven-tionally. The hair graftsare then transplanted intothe slits manually. The la-ser beam cannot be usedin removing or cutting thepatient's own permanenthairs with roots from the"donor" areas. It burnspermanent hairs and va-porizes tissues.

it did prior to surgery. Ad-ditionally, the third gen-eration method reducesthe size of the scar in theback of the scalp, creatinga natural-looking hairlineinstead of the "corn row"appearance. We train ourtechnicians better, usingmodern equipment. Andwe create smaller grafts toproduce more natural hairgrowth.

Laser Hair Transplantation Experiences in Germany

Laser hair transplantation has been performed widely in Germany. Sev-eral research reports have indicated that there are no advantages of laser hairtransplantation over the conventional cold steel technique and more and morestories are surfacing in the German media about unsuccessful laser treatments,casting a bad light on the whole hair transplant industry. The only advantage oflaser hair transplantation is in marketing.*Disadvantages of laser hair transplantation:

• The laser beam burns tissues, damages existing hairs and seals bloodvessels around the transplant areas.

• The yield of growing hairs on the laser hair transplanted side is visiblylower.

• Longer post-op crusting time.

• Delay in hair growth.

For laser hair transplantation, there are two concerns:

1. High energy laser beams may damage hairs existing near the implantsite. Are you aware that one of the latest laser applications is for perma-nent hair removal? Unlike electrolysis which removes hairs one by one,laser removes hair in a certain area all at once. Some doctors claim thatthere are special laser torches specifically designed for hair transplantand that no thermal damage is caused. But as a prospective client, youshould demand to see the results of laser hair transplant for yourself.

2. A laser seals blood vessels. This is helpful in other types of cosmeticsurgery, where wounds need to be closed up and cauterized. But forhair transplantation, continuous blood flow is important for hairgrowth; blood brings necessary nutrients and oxygen to the implantedhairs. The heat of the laser stops this blood flow, resulting in a lowergrowth rate and less successful transplant.

Hair Transplantation Process FAQ

What steps should Itake before surgery?

Maintain an adequatelength of hair on theback of your head. Thisshould be at least oneinch long, or you mayhave a problem coveringup the sutures after thesurgery. Your donorgrafts will be removedfrom here and youshould expect coveragefollowing your proce-dure.

(acetylsalicylic acid -A.S.A.) or anti-inflammatory medi-cations containingAspirin. Note: thesemedications are in-cluded in many over-the counter medica-tions.

Is the hair transplantsurgery painful?

You may haveheard that the process isvery painful. But with theproper technique in theapplication of anesthesia,it does not have to be apainful process at all. Itall depends on how thesurgeon applies the anes-thesia, and our officeuses the least painfulprocess available for thecomfort of our patients.For a hair transplantationprocess, every surgeonuses either a nerve block-ing process or a local an-esthesia; general anesthe-sia is never used in thiscase. Any pain at all dur-

ing our procedure would beduring the initial needle in-jection. Our office usesseveral techniques to re-duce even this small pain.The anesthesia is warmedto reduce reaction time, theskin is pre-numbed beforeeach localized shot and thefinest gauge needle (30gauge) is used to minimizethis pain. Initially, the pa-tient is given a sedativethrough an IV medication.This sedative puts the pa-tient into a relaxed,"twilight zone" state tominimize any pain theremight be from the subse-quent injections. The tu-mescent technique is usedfor prolonging anesthesiatime.

Most patients haveinformed me that the proc-ess was less painful than aroutine trip to the dentist.In addition, the day afterthe hair transplant process,50% of our patients do notrequire any pain relieversat all, and the other 50%will take Tylenol for only afew days until the discom-fort is gone. Few of our pa-tients request a prescriptionpain reliever. We feel thatour procedures are muchless painful in comparisonto many other offices. Theonly disadvantage to this"twilight zone" anesthesiais that the patient will not

be able to drive on theday of the surgery. How-ever, should the patientforego the "twilightzone" anesthesia and optinstead for a local anes-thesia, he will be able todrive on that day.

How will the surgeryaffect me when I re-turn to work the fol-lowing day?

For the firsttwenty-four hours, therewill be a strip of bandagearound the head. Afterthat, you will be able toremove the bandage andshampoo your hair asusual. If you have thesurgery done on the hair-line, the small scabs willremain for seven to tendays. Some patients maywant to cover the areawith a cap or even a hair-piece at work if they de-sire. If you have partialhair, it is much easier tocover up the area, or youcan use a camouflageproduct.

When the smallscabs fall off after sevento ten days, the freshlyimplanted grafts will bea pink, slightly shiny,skin which is usually asimilar color to the sur-rounding normal skinand is minimally obvious

at first. After approxi-mately six weeks, the ar-eas where the grafts weretransplanted are barelyvisible.

Pre-existing hairadjacent to the trans-planted grafts may shed,giving a thinner look, buthair will begin to growback within a fewmonths. This temporarythinning of pre-existinghair is called Telogen Ef-fluvia. Unfortunatelythere is a time lag be-tween this increasedthinning of pre-existinghair and re-growth oftransplanted hair.

How many sessionsof surgery will Ineed?

If you have acompletely bald area, itmay take two to threesurgeries to cover yourentire head. If you havean area partially coveredwith hair, it may takeonly one surgery to fillin the area. Our goal is torestore about twenty-fivepercent of the donor'soriginal density in eachsurgery. After two sur-geries, the density is atabout fifty percent whichis often adequate cover-age to satisfy the patient.The time between each

session is usually threeto four months in orderto see how the hair isgrowing out and to pro-vide more equal distribu-tion.

How long does ittake the transplantedhair to grow outenough to fill in thebalding area?

Usually it takesthree to four months forthe hair to grow out, andafter that it will growabout half an inch permonth which is the samerate as the donor hair.Initial hair quality is usu-ally very thin, like babyhair, and then it becomescoarser over time. Some-times the initial hair iscurlier, then straightens,in about one year. Thecolor of the initial hairmay also be darker andlater will be lightened bythe sun.

How long will thetransplanted haircontinue to grow?

Basically, thetransplanted hair is thehair from the back of thehead, so even though it istransplanted, it will stillhave the same life span.

Bleeding Some bleed-ing is normal and willstop with simple pres-sure. Persistentbleeding occurs inabout one in a fewhundred cases. Addi-tional stitching israrely required.

Normally a hair folliclesheds every three to sixyears and then growsback from the same root.The transplanted haircontinues the same lifecircle.

Can hair transplan-tation affect the restof my hair?

Hair roots arevery tough and can toler-ate surgery or any irrita-tion on the scalp. Weknow this because wecan see how difficult it isfor people to remove un-wanted hair. A hair canbe plucked out hundredsof times and just keepgrowing back. The actualphysical trauma of cut-ting the scalp when in-serting the donor graftscan cut the shafts and/ordamage some hair folli-cles, but more significantis the interruption to thepre-existing hair's bloodsupply which is enoughto cause the shedding ofthis pre-existing hair. Itis temporary and mostlost hair will grow backafter a few months.

5%. It usually lasts several hours to several days. If left untreated, the hic-cups may interfere when you eat and sleep, but there is a medication that thedoctor can prescribe to ease the hiccups.

Itching Some itching commonly occurs but is rarely troublesome and lastsonly a few days. Shampooing the hair daily will help the discomfort.

Swelling There is some swelling in nearly all cases. It affects the foreheadand the area around the eyes and lasts two to five days, being maximal onthe fourth day. In about one in fifty cases, a “black eye” develops. With par-ticular patients, swelling can be considerable and temporarily disfiguring,especially after the first session. However, it does little harm and lasts nomore than a week. A drug called Cortisone can be given to reduce thesesymptoms for those who are concerned about swelling, but Cortisone can beharmful if used often; it has numerous rare but potentially serious side ef-fects.

Infection This happens in one in several thousand cases and is easily curedwith antibiotics.

Scarring Keloid scarring occurs only in pre-disposed individuals, and evenmore rarely (1/1000 cases) has this keloid scarring been hypertrophy to thepoint of “ridging.”

Cysts One or more cysts may occur in the recipient area when manymini-grafts have been inserted. They usually disappear by themselves after afew weeks or immediately with various simple treatments. They are not usu-ally more than 2 or 3 mm in diameter, i.e., the size of small pimples.

Neuralgia When a medium-sized nerve, such as the occipital nerve, is eithercut or bruised, patients can get either numbness, tingling or “pins and nee-dles” sensations, sometimes even “shooting pains” and hypersensitivity ofskin in that part of the scalp. Once in a while someone reports varying com-binations of the above, usually lasting for one to four weeks. Very rarelyhave patients had neuralgia last a longer time period. Prolonged or severesymptoms are exceedingly rare and virtually always disappear within a fewmonths.

• The patient is given amild sedative intra-venously.

• Hair is taped up inthe back to allow ac-cess to the selectedarea of hair-bearingdonor scalp. A thinstrip of the perma-

nent hair along thelower back of thescalp is used. After-ward, this thin scarwill be easy hiddenunderneath the sur-rounding permanenthair.

• Using a very fine 30gauge needle, thescalp is anesthetized.Once the scalp isnumbed, the surgeryis virtually painlessand the nerves areblocked.

• After the fine stripsof hair are removedfrom the back of thescalp, the techniciansdivide them into thepredetermined sizesof grafts using BackLight, stereoscopeand the video visual-izer

• The back of the scalpis sutured by the sur-geon. This scar willbe barely detectablein a few months andwill be completelyhidden by the hairgrowth in back.

• The grafts are nowready to transplantinto the bald or"recipient" area ontop of the scalp.Mini-slits are madefor the mini-grafts (2

to 4 hairs) while 19gauge needle holesare made for the mi-cro grafts (1 to 2hairs.) We are able toaccomplish severalthousand hair graftsin one session.

• The patient is thenserved lunch and isoffered a selection ofmovies to watch afterthe slits are made ac-cording to the design.

• After lunch 2 to 4technicians begin im-planting the grafts.This part of the pro-cedure can last be-tween 2 to 4 hours.

• After all grafts havebeen completed andchecked, the donorand recipient areasare cleaned and thedonor area only isbandaged. The pa-tient is given a list ofpost-operative in-structions and medi-cations. Because an-esthesia is used, thepatient must notdrive for severalhours so it is neces-sary to arrange fortransportation homeprior to arrival at theclinic.

I understand that a non-refundable $500 Booking Fee which willbe credited towards the cost of surgery (non-refundable unless Igive a one week notice) is required at the time of booking a pro-cedure to cover administrative costs and to secure the time allot-ted for the procedure.
