"rnlThe P i t t s b u r g h Supercomputing Center (PSC) is conduct ing a five-day workshop on supercomputing techniques f o r biomedical r e s e a r c h e r s December 4-8, 1989. This workshop is funded by a g ran t from t h e Div is ion of Research Resources' Biomedical Research Technology (BRT) Program o f t h e Nat iona l I n s t i t u t e s o f Health ( N I H ) . The workshop is aimed a t experienced FORTRAN programmers, bu t p r i o r supercomputing exper ience is not necessary . The t o p i c s inc lude an i n t r o d u c t i o n t o VMS (half-day, o p t i o n a l ) , t h e Cray-Vax i n t e r f a c e , t h e UNICOS opera t ing system, opt imiza t ion techniques , an overview o f a v a i l a b l e biomedical sof tware and a d e s c r i p t i o n o f access pa ths t o t h e PSC.

The first program, "Developing and Improving I n s t i t u t i o n a l Animal Resources", RFA 89-RR-02, was announced i n t h e A p r i l 28, 1989 i s s u e of t h e N I H Guide f o r Grants and Cont rac ts , Vol. 18, No. 15, and is intended f o r , bu t is no t r e s t r i c t e d t o , t h o s e i n s t i t u t i o n s t h a t r ece ive more than $500,000 of Publ ic Heal th Se rv ice funds annual ly f o r an imal - re la ted r e sea rch . The t o t a l award must be matched ( d o l l a r - f o r - d o l l a r ) by non-federal funds.

The a s s i s t a n c e mechanism used t o support t h e s e s t u d i e s w i l l be t h e coopera t ive agreement, which is similar t o t h e t r a d i t i o n a l N I H r e sea rch g r a n t . It d i f f e r s from a r e sea rch g ran t i n t h e ex ten t and n a t u r e of N C I s t a f f involvement wi th i n v e s t i g a t o r s . BACKGROUND

Urinary incont inence , t h e involuntary l o s s o f u r i n e so severe as t o have s o c i a l and/or hygienic consequences, is a major c l i n i c a l problem and a s i g n i f i c a n t cause o f d i s a b i l i t y and dependency. Urinary incont inence affects a l l age groups and is p a r t i c u l a r l y common i n o l d e r persons. A t l eas t 10 m i l l i o n a d u l t Americans s u f f e r from u r ina ry incont inence, inc ludingapproximately 15 t o 30 percent o f community-dwelling o l d e r people and a t l eas t one-half of a l l nurs ing home r e s i d e n t s . The monetary c o s t s o f managingur inary incont inence are conse rva t ive ly estimated a t $10 .3 b i l l i o n annual ly ,and t h e psychosocial burden o f u r i n a r y incont inence is g r e a t .

Urinary incont inence is a symptom r a t h e r t han a d i s e a s e . I t appears i n a l i m i t e d number o f c l i n i c a l p a t t e r n s , each having s e v e r a l p o s s i b l e causes . I n some cases, t h e d i s o r d e r is t r a n s i e n t , secondary t o an e a s i l y reversed cause such as a medication or an acu te i l l n e s s l i k e u r i n a r y t r a c t i n f e c t i o n . Many cases are chronic , however, l a s t i n g i n d e f i n i t e l y u n l e s s proper ly diagnosed and t r e a t e d .

An N I H Consensus Development Conference e n t i t l e d "Urinary Incont inence i n Adults" was he ld October 3-5 , 1988. This conference was sponsored by t h e Nat ional I n s t i t u t e on Aging and t h e Office of Medical Appl ica t ions o f Research o f t h e Nat ional I n s t i t u t e s o f Health, i n conjunct ion with t h e Nat ional I n s t i t u t e o f Diabetes and Diges t ive and Kidney Diseases, t h e Nat ional Center f o r Nursing Research, t h e Nat ional I n s t i t u t e o f Neurological and Communicative Disorder and S t roke and t h e Veterans Adminis t ra t ion.

The Consensus Development Conference provided an overview of c u r r e n t knowledge on t h e e t io logy , pathophysiology, sequelae, and management of t h i s p reva len tc l in i ca l problem. Although information on incont inence is inc reas ing , t h e f i e l d has long been neglec ted , and numerous gaps e x i s t i n our knowledge.While many c o n t r o v e r s i e s were addressed, numerous ques t ions were i d e n t i f i e d t h a t await answers and t h u s serve as t h e focus f o r f u t u r e r e sea rch d i r e c t i o n s . ELIGIBILITY REQUIREMENTS

The proposed V i s i t i n g Researcher must be P r i n c i p a l I n v e s t i g a t o r on a t l e a s t one DDEM-supported r e sea rch p r o j e c t g ran t (Rol l o r P r i n c i p a l I n v e s t i g a t o r on a program p r o j e c t g ran t ( P o l ) , o r r e sea rch center g ran t ( P 5 0 ) . Under s p e c i a lcircumstances t h e p r i n c i p a l i n v e s t i g a t o r may sponsor a q u a l i f i e d member of h i s l h e r team, provided t h e member r e t u r n s t o t h e Sponsor 's l abo ra to ry and a p p l i e s t h e acqui red techniques f o r a t l eas t one yea r . Data on t h e a s s o c i a t i o n between s o c i a l support and h e a l t h come from a d i v e r s e set o f i n t e r n a t i o n a l i n v e s t i g a t i o n s . I n t h e Alameda County Study low s o c i a l support w a s p rospec t ive ly r e l a t e d t o a l l - cause m o r t a l i t y and t h i s r e l a t i o n s h i p was maintained af ter c o n t r o l l i n g f o r socioeconomic s t a t u s , i n i t i a l h e a l t h s t a t u s ,and s tandard r i s k f a c t o r s ( e . g . , smoking, o b e s i t y , a l coho l consumption).Furthermore, low support p red ic t ed s p e c i f i c causes o f dea th , inc ludingischemic h e a r t d i s e a s e and cancer . I n a p rospec t ive Tecumseh, Michigan s tudy , a s imilar r e l a t i o n s h i p between s o c i a l support and m o r t a l i t y was found f o r men. Re la t ionsh ips between support and m o r t a l i t y were found i n t h e Evans County,Georgia, s tudy for white men, bu t no t f o r women or b lack men. I n a p r e d i c t i v es tudy conducted on o l d e r men and women i n Durham, North Caro l ina , a v a i l a b i l i t yo f suppor t , frequency o f c o n t a c t s with network members, and pe rcep t ions o f suppor t were a l l r e l a t e d t o m o r t a l i t y a f t e r c o n t r o l l i n g f o r socioeconomic s t a t u s , phys i ca l h e a l t h , stress, and smoking.

Outside t h e United S t a t e s , popula t ion s t u d i e s have r e p l i c a t e d t h i s s o c i a l support and m o r t a l i t y r e l a t i o n s h i p . I n a s tudy o f men i n Gothenburg, Sweden,s o c i a l p a r t i c i p a t i o n w a s nega t ive ly r e l a t e d t o m o r t a l i t y . Th i s r e l a t i o n s h i p w a s maintained after c o n t r o l l i n g f o r age and ca rd iovascu la r r i s k f a c t o r s . S imi l a r ly , a p o s i t i v e effect o f s o c i a l c o n t a c t s and s o c i a l i n t e r a c t i o n s on s u r v i v a l was found f o r a random sample o f t h e Swedish popula t ion aged 29 t o 7 4 . I n t h e F innish North Karelia s tudy, s o c i a l support w a s a s soc ia t ed wi th t o t a l and ca rd iovascu la r m o r t a l i t y i n men.

I n a prospec t ive s tudy of male su rv ivo r s of myocardial i n f a r c t i o n ( M I ) , it w a s found t h a t low suppor t coupled with psychologica l s t r a i n p red ic t ed post-MIm o r t a l i t y a f te r c o n t r o l l i n g f o r medical s t a t u s and t r a d i t i o n a l CVD r i s k f a c t o r s . Recent s t u d i e s have a l s o demonstrated a r e l a t i o n s h i p between s o c i a l support and h e a l t h behaviors . For example, people with adequate support maybe more l i k e l y t o comply wi th medical regimens, and may be more success fu l a t a l t e r i n g r i s k behaviors ( e . g . , smoking c e s s a t i o n ) . Some d a t a a l s o demonstrate ** a p o s s i b l e r o l e f o r p o s i t i v e s o c i a l r e l a t i o n s and heal th-seeking behaviors , short- term recovery ,,from ca rd iovascu la r events , and long-term adjustment t o CVD. For example, access t o suppor t ive o t h e r s has been i d e n t i f i e d as a component i n f a c i l i t a t i n g recovery from bypass surgery .

Support may a l s o b u f f e r t h e effect o f stress on h e a l t h . A s tudy conducted i n t h e Nether lands, f o r example, found t h a t s t r e s s e d employees wi th s o c i a l support had lower blood p res su re than s t r e s s e d employees without suppor t . A Swedish s tudy o f work cond i t ions , p a r t i c u l a r l y degree of c o n t r o l over t h e work day, pe rcep t ions o f autonomy, and sources of s o c i a l suppor t , revea led t h a t t h e h ighes t rates of ca rd iovascu la r d i s e a s e occurred i n persons who perce ived h igh stress i n t h e i r jobs , had a low a b i l i t y t o c o n t r o l t h e i r working environment, and had low s o c i a l suppor t .

To summarize, population-based s t u d i e s have c o n s i s t e n t l y demonstrated a r e l a t i o n s h i p between s o c i a l support and CVD-related h e a l t h and well-being. This a s s o c i a t i o n is p a r t i c u l a r l y robus t when one cons ide r s t h a t t h e empi r i ca l i n v e s t i g a t i o n s have been notab ly d i v e r s e with marked v a r i a t i o n s i n s o c i o c u l t u r a l con tex t s , measures o f s o c i a l support , s tudy endpoints , and confounding v a r i a b l e s . Unfortvnately, very l i t t l e is known beyond t h e s e broad a s s o c i a t i o n s between s o c i a l support and CVD. More s p e c i f i c a l l y , s t u d i e s t o d a t e have no t been designed t o address t h e mechanisms and processes t h a t may account f o r t h e s e broad a s s o c i a t i o n s between s o c i a l support and CVD-related h e a l t h . U n t i l a clearer understanding o f t h e s e processes is achieved, i n t e r v e n t i o n s designed t o a l l e v i a t e t h e nega t ive h e a l t h effects of inadequate s o c i a l suppor t are no t f e a s i b l e .

The purpose o f t h i s announcement is t o s t i m u l a t e r e sea rch designed t o i n v e s t i g a t e t h e processes through which s o c i a l support may in f luence t h e onse t , s e v e r i t y , p rogress ion or adjustment t o ca rd iovascu la r d i s e a s e . S t u t t e r i n g , a speech d i s o r d e r which t y p i c a l l y begins during t h e preschool yea r s , is cha rac t e r i zed by t h e r e p e t i t i o n and pro longat ion of speech sounds. Many i n d i v i d u a l s who begin t o s t u t t e r dur ing childhood experience spontaneousremission of t h e i r symptoms by adolescence. More information is needed concerning t h e causes o f t h i s d i s o r d e r and t h e reasons f o r spontaneous

-LILlfl recovery. Because t h e r e i s a l a c k o f unanimity concerning t h e d e f i n i t i o n of t h i s d i s o r d e r , d i agnos t i c c r i t e r i a are needed f o r t h e i d e n t i f i c a t i o n o f both c h i l d and a d u l t s t u t t e r e r s . Experimental i n v e s t i g a t i o n s t h a t expand our knowledge of s t u t t e r i n g i n t h e s e a r e a s w i l l l e a d t o improved t rea tment methods.
