Mabedlane clinic under Nkonjeni Dis-
trict Hospital as a Clinical Nurse Practi-
tioner. I was trained as NIMART nurse
by FDP, From training I had an insight ,
since protocols and guidelines were
given.
One day I had one incidence that low-
ered my self esteem, the client was hav-
ing co-infection of TB and HIV. He was
started on TB treatment then after two
weeks he came for ART initiation,. He
had a very low CD4 count, one week
passed by the I phoned the Community
Care Giver to go and check him. The
report I got was not good and three days
later he passed away.
I felt so bad asking myself what have I
done I phoned my mentor telling her I
can not continue with this thing, she
came for support and check the file say
“ you have done all you were supposed
to do, anyway you were saving life, un-
fortunately he did not make it”. With
mentoring and clinical audit, I become
more confident and competed , thus I
became a mini expect on NIMART.
With us initiating there was increasing
number of client causing more conges-
tion resulting in long waiting times,
then we implemented Mpilonde Clubs.
(it a chronic club that integrates all
chronic). But Mpilonde Clubs wouldn't
be successful without 3 TIER which has
help us to identify early missed and
late missed the appointment before the
client came to default.
Stable clients who were virally sup-
pressed were moved to Mpilonde
Clubs and there are given two month
supply, we will be upgrading to three
month supply that will make the client
to come four time a year in the clinic.

I am able to manage Paeds and
adult at the PhC Level.
Integrated TB and HIV co-
infection.
Fast patient enrollment on ARIT
(no waiting times of eligible
clients)
Greater acceptability and ac-
cessibility.
Improved patient outcome, we
started on a CD4 count of 200
we upgrade to 350, we are now
at 500 and with the PMTCT we
initiate at diagnosis, the soon we
start the better are the outcomes

PHELOPHEPHA HEALTH TRAIN ARRIVED AT ULUNDI MUNICIPALITY
PHELOPHEPHA TRAINS FROM TRANSNET STATIONED AT ULUNDI TOWN FOR HEALTH SERVICES
The community of Ulundi and Mahlabathini Municipality access Health Services on Othometrist, Dental, Physcologist and many more health services as the train got specialist. Operation Sukuma Sakhe,Local Task Team together with Zuluand Mu-nicipality and Ulundi Municipality supported the programme.
Nkonjeni District Hospital Management visited the site to interact with acting CEO for Phelophepha , on the right picture, Act-ing CEO Mrs T.R Sibisi, Acting CEO Phelephepha train, Mrs T.T Shezi (Deputy Manager Nursing and Mrs Mkhwanazi the Phc Manager on site.
