Concerned DDE (District) shall be responsible for all kinds of arrangements e.g. engagement of experts & doctors, rooms for assessment & measurement, assembly of CWSN of the Zone (S) given below and filling of Record Profile & ADIP forms, seating arrangements for CWSN and their escorts, procurement and distribution of refreshment to CWSN and arrangement of lunch etc. to staff, availability of drinking water, toilets facility and other necessities. Adequate staff may be deployed to ensure proper discipline and smooth functioning of the camps. The District Coordinators (IEDC) will make all the efforts for ensuring the attendance of out of school CWSN at the camps as per the list prepared by them at UEEM office. In this regard they are directed to invite the parents of out of school CWSN through letters (sample is enclosed at Annexure-I) persuading and encouraging them to bring their wards to the Assessment Camps. Questionnaire for parents of out of school CWSN is also enclosed at Annexure-II. The dates for these Assessment Camps were finalized in consultation with ALIMCO (Artificial Limbs Manufacturing Corporation) and venues as suggested by DDE’s (Districts) through District Coordinator (IEDC) are scheduled below:

Necessary amount shall be drawn out of the funds being made available for making these arrangements.Expenditure may be incurred as per norms enclosed (Annexure-III) and expenditure statement alongwith requiredinformations in Annexures IV to XI may also be submitted within 07 days of the Assessment Camp is over.
Following officials from IEDC Cell shall monitor and supervise the camps indicated against their names inAnnexure XII and will submit the report to the undersigned:
