SummaryThis is a descriptive cephalometric study aimed at contributingto the establishment of Steiner analysis cephalometric norms fora patient population from the Center for Dental Treatment andConsultation (CCTD) at the Ibn Rochd Hospital Center inCasablanca, Morocco. Teleradiography was used to obtainlateral cephalograms of 71 young adults (47 women; 24men) aged on average 22.73W 1.69 years. All subjects pre-sented good facial harmony, an acceptable profile, and dentalclass I with no history of orthodontic treatment. Tracing andanalysis followed. The resulting means and standard deviationswere compared to Steiner’s norms and norms from similarstudies. This revealed certain particularities in our sample,the most notable being a greater skeletal sagittal discrepancy(indicating a class II tendency), labial crown tipping of thelower incisors, and a retruded position of the maxillary andmandibular bony bases. These features should be taken intoaccount when diagnosing and treating patients at the CCTD.However, more exhaustive studies must be performed before theseresults can be generalized to the Casablancan or Moroccanpopulations as a whole.

Ce travail pr�esente une �etude c�ephalom�etrique descriptivedont l’objectif �etait de contribuer a l’�etablissement de normesc�ephalom�etriques de l’analyse de Steiner pour une populationconsultant au Centre de consultation et de traitement dentaire(CCTD) du centre hospitalier Ibn Rochd de Casablanca(Maroc). Soixante et onze t�el�eradiographies de profil de jeunesadultes (47 femmes et 24 hommes) ag�es en moyenne de22,73W 1,69 ans, pr�esentant une harmonie faciale et un profilacceptable, une classe I dentaire et n’ayant aucun ant�ec�edentde traitement orthodontique ont �et�e prises, trac�ees puis ana-lys�ees. Les moyennes et �ecarts-types obtenus ont �et�ecompar�es aux normes de Steiner et a celles d’�etudes simi-laires mettant en �evidence certaines particularit�es au sein denotre �echantillon, notamment un d�ecalage squelettique sagit-tal plus important traduisant une tendance a la classe II, uneversion corono-vestibulaire des incisives inf�erieures et uneposition sagittale recul�ee des bases osseuses maxillaire etmandibulaire. Ces caract�eristiques devraient etre prises enconsid�eration dans le diagnostic et le traitement des patientsconsultant au CCTD. Introduction
Cephalometric analysis is a method for measuring, describing,and studying the relationships between structures in the head.Cephalometric data have numerous applications [1,2]. Theycan be very useful in diagnosis, treatment, teaching, andresearch.
The diagnostic and therapeutic usefulness of cephalometricshas given rise to several hundred analysis types, each involv-ing a large number of measurements and norms [3]. Yet theapplication of these analyses and norms remains relativelycontroversial because, among other reasons, they are basedon certain populations with specific traits [4].
Morocco is a nation of great geographic and demographicdiversity. Its unusual anthropological heterogeneity and mix-ture of different ethnicities confer upon it a unique characterthat cannot be ignored. Nevertheless, Moroccan orthodontistscontinue to treat their patients based on European orAmerican norms.Our study is intended to help establish cephalometric normsspecific to a sample of Moroccan individuals in order todevelop diagnosis and treatment guidelines proper to thispopulation.
Patients and methods
Type of study
We performed a descriptive cross-sectional study on 71 lateralcephalograms of young Moroccan adults, ages 19–27 (bothsexes). All subjects met specific criteria for facial harmonyas well as normal occlusion, and were part of the patientpopulation of the Center for Dental Treatment andConsultation (CCTD) in Casablanca. The resulting data rep-resent linear and angle measurements taken in accordancewith the rules of cephalometric tracing.
Study population
The sample was selected from within the entire patient pop-ulation of the CCTD in Casablanca between January and June2010. Patients from the dentofacial orthopedics departmentwere not included.The inclusion criteria used for sample selection were:
— young adult patients with good facial harmony and anacceptable profile;— no past or present orthodontic treatment;
— no joint or bite problems;— class I occlusion;

Introduction
L’analyse c�ephalom�etrique est une m�ethode de sch�ematisa-tion, de mesure et d’�etude des rapports des structuresc�ephaliques. Les utilisations des donn�ees c�ephalom�etriquessont nombreuses [1,2] et peuvent avoir un int�eret diagnostiqueet th�erapeutique, un int�eret p�edagogique, un int�eret pour larecherche.L’utilisation diagnostique et th�erapeutique de l’analysec�ephalom�etrique a donn�e lieu a l’�elaboration de plusieurs cen-taines d’analyses, chacune comportant un grand nombre demesures et de normes [3]. Toutefois, l’utilisation de ces ana-lyses et de ces normes reste assez controvers�ee du fait, entreautres, des sp�ecificit�es des populations a partir desquelleselles ont �et�e �etablies [4].La populationmarocaine, riche de sa diversit�e g�eographique etd�emographique, se distingue par une h�et�erog�en�eit�e anthropo-logique et un brassage ethnique lui conf�erant une particularit�esinguli�ere devant etre prise en consid�eration. Toutefois, lesorthodontistes marocains continuent de traiter leurs patientsen se r�ef�erant aux normes europ�eennes ou am�ericaines.Notre travail se propose de contribuer a l’�etablissement denormes c�ephalom�etriques sp�ecifiques d’un �echantillon maro-cain et cela dans le but d’orienter le diagnostic et la prise encharge th�erapeutique propres a cette population.
Patients et m�ethodes
Type d’�etude
Nous avons r�ealis�e une �etude descriptive transversale de 71t�el�eradiographies de profil de jeunes adultes marocains, ag�esde 19 a 27 ans, des deux sexes, r�epondant a des crit�erespr�ecis de normalit�e faciale et occlusale et ayant consult�e auCentre de consultation et de traitement dentaire (CCTD) deCasablanca. Les donn�ees recueillies repr�esentent desmesures lin�eaires et des constructions angulaires effectu�eesselon les r�egles des trac�es c�ephalom�etriques.
Population de l’�etude
Elle a �et�e extraite de l’ensemble des patients consultant auCCTD de Casablanca durant la p�eriode de janvier a juin 2010,sans y inclure les patients consultant au service d’orthop�ediedentofaciale.Les crit�eres d’inclusion retenus pour la s�election de notre�echantillon sont :— patients jeunes adultes, pr�esentant une harmonie faciale etun profil acceptable ;— absence de traitement orthodontique pr�ealable ou encours ;— absence de troubles occluso-articulaires ;— occlusion dentaire de classe I ;

— little to no overlap or spaces between teeth.
In total, 47 young women and 24 young men, ages 19–27, wereincluded from the target population. The study thus included71 radiographs.All patients were informed of the study objectives. Full con-sent was obtained from all study participants.
For each subject we created an information sheet with thefollowing data: last name, first name, age in years, sex, tele-phone number, all linear and angle measurements.
Methodology
Cephalometric tracings were performed with a 0.5 mmmechanical pencil on sheets of tracing paper fixed to theradiographs over an illuminator. In accordance with standardtechnique, the patient’s profile faced right and was oriented toplace the SN line horizontally.All tracings were done by the same experienced professor oforthodontics. Tracings were repeated 15 days later to mini-mize any risk of operator error.
Landmarks were then located and lines traced for cephalo-metric analysis.
Statistical analysis
Statistical analysis was performed using the French edition ofEPI INFO version 6 for PC (specifically the “EPI Table”statistics module).Student’s t-test was used to compare our means with those ofother studies.For the purposes of our study (ddl = n � 1, i.e. 71 � 1 = 70),the difference between twomeans is significant if the t-value isgreater than 1.96.
Results
Sample details
Age (in years)
The 71 subjects of our sample are distributed as follows:
— 19 subjects aged 19–21 (26.8%);— 41 subjects aged 22–24 (57.7%);— 11 subjects aged 25–27 (15.5%).Mean age was 22.732 W 1.698 years.

M�ethodologie
Le trac�e c�ephalom�etrique a �et�e r�ealis�e a l’aide d’une mine decrit�erium 0,5 sur un calque solidaris�e au film, pos�e sur unn�egatoscope. Selon la technique standard, le profil du sujetest tourn�e a droite et le clich�e orient�e de telle sorte que la ligne(SN) soit horizontale.Tous les trac�es ont �et�e r�ealis�es par un seul op�erateurexp�eriment�e et professeur en orthodontie. Les trac�es ont �et�erefaits 15 jours apr�es pour minimiser les erreurs li�eesa l’op�erateur.Sur les trac�es ainsi obtenus, nous avons rep�er�e les pointsremarquables puis trac�e les lignes propres a l’analysec�ephalom�etrique.
Analyse statistique
L’analyse statistique de l’�etude a �et�e r�ealis�ee par la versionfrancaise du logiciel statistique EPI INFO 6 implant�e sur PC,sous le module statistique EPI Table.Le test t de Student a �et�e utilis�e pour la comparaison de nosmoyennes avec celles des autres �etudes.Pour notre �etude (ddl = n � 1, soit 71 � 1 = 70), la diff�erenceentre les deux moyennes est significative si la valeur de « t »est sup�erieure a 1,96.

Les r�esultats de l’�etude nous r�ev�elent que les 71 sujets sontr�epartis comme suit :— 19 sujets ag�es de 19 a 21 ans, soit 26,8 % ;— 41 sujets ag�es de 22 a 24 ans, soit 57,7 % ;— 11 sujets ag�es de 25 a 27 ans, soit 15,5 %.La moyenne d’age du groupe �etant de 22,732W 1,698 ans.

Of the 71 subjects that participated in the study, 47 werefemale (66.2%) and 24 male (33.8%).
Sagittal skeletal measurement results
Broken down by sex
The difference in means between the sexes is significant forSNA, SNB, SND and SL (Table I).We can thus conclude that the women from our sample presenta more retrognathic position of the maxilla and mandible whencompared with the men.The difference in means between the sexes is not significantfor any other values (t < 1.96).

La diff�erence entre les moyennes des deux sexes est signifi-cative pour les valeurs de SNA, SNB, SND et SL (Tableau I).Nous pouvons en d�eduire que les femmes de notre �echantillonpr�esentent une r�etrognathie maxillaire et mandibulaire parrapport aux hommes.La diff�erence des moyennes des deux sexes pour toutes lesautres valeurs est non significative (t < 1,96).

Overall, subjects aged 19–24 showed very similar sagittalskeletal relationships (Table II). The third age group (25–27) presents distinctly larger values.

Like the sagittal skeletal measurements, the vertical skeletalmeasurements show different results for the third age group,this time with smaller mean values compared with the othertwo groups (Table V).

La diff�erence entre les moyennes des deux sexes est signifi-cative pour la valeur de Occ/SN, t = 2,63 (Tableau IV).La diff�erence entre les moyennes des deux sexes est nonsignificative pour les valeurs de GoGn/SN et Pog/NB(t < 1,96).

Although the measurements taken for each age group weredifferent, the difference between the means is not statisticallysignificant (Table VIII).

Dentoalveolar measurement results are summarized in TableIX.
Comparison to Steiner’s norms
We used Student’s t-test to set the norms obtained through ourstudy in relation to those used in Steiner’s analysis [2,5](Table X). Our means are significantly different fromSteiner’s for all variables.
Discussion
Discussion of materials and methods
To be valid, a study sample requires to show minimum selec-tion bias. This is crucial to establishing accurate populationnorms [6].

We therefore selected our sample from a population that metvery specific objective criteria, namely no history of orthodon-tic treatment, class I occlusion, etc. The only subjective inclu-sion criterion was facial harmony/symmetry.
The patient population at the CCTD in Casablanca consists ofindividuals from various regions of Morocco. For this reason,we can say that our target population, and therefore our sam-ple, presents a certain degree of ethnic diversity even thoughthis is not statistically proven or quantified in our study.
The tracing and locating techniques for certain cephalometriclandmarks are not universally defined, thus leading to possi-ble projection errors due to the 2-dimensionality of radio-graphs or errors in the identification of various points andstructures [7]. The gonion point, for example, is sometimesfound by determining the lowest posterior point on the angle ofthe mandible. It can also be constructed by bisecting the angleformed between the mandibular and ramus planes and thenlocating the intersection of this bisector with the angle of themandible. Consequently, measurements performed on differ-ently-defined landmarks cannot be compared. Unfortunately,most published studies do not specify how their landmarkswere located.
Discussion of results
Several foreign studies similar to ours have sought to establishnorms for their respective populations using the same refer-ence variables as in our study, namely those from Steiner’sanalysis (Table XI).

C’est pour cela que nous avons choisi notre �echantillon parmiune population r�epondant a des crit�eres objectifs bien pr�ecis,notamment l’absence de traitement orthodontique pr�ealableou encore une occlusion de classe I, etc. Le seul crit�ered’inclusion subjectif de l’�etude a �et�e l’appr�eciation de la sym�e-trie et l’harmonie faciale.La population consultant au CCTD de Casablanca �etaitconstitu�ee d’individus originaires de diff�erentes r�egions duMaroc. De ce fait, nous pourrions dire que notre populationcible et donc notre �echantillon pr�esente une certaine diversit�eethnique bien que cela ne soit pas quantifi�e ou prouv�e statis-tiquement dans notre �etude.Le tracage et le rep�erage de certains points remarquables del’analyse ne sont pas universellement d�efinis et peuvent etresujets a des erreurs de projection dues au caract�ere 2D desclich�es, ou encore a des erreurs d’identification des diff�erentspoints et structures [7]. Nous citons pour exemple le pointGonion dont le rep�erage peut etre estim�e comme �etant le pointle plus post�ero-inf�erieur de l’image de l’angle mandibulaire, oualors construit par l’intersection de la bissectrice de l’anglePlan mandibulaire/Plan ramal avec l’image de l’angle mandi-bulaire. Par cons�equent, les mesures effectu�ees sur despoints rep�er�es diff�eremment ne pourraient etre compar�ees.Or la majorit�e des �etudes publi�ees ne pr�ecisent pas les tech-niques de rep�erage utilis�ees.

In order to determine where our study stands in relation tosome of these others, we performed statistical comparisonsusing Student’s t-test (Table XII).
The results are as follows.
Comparison of the sagittal position of the maxilla: SNAangle
The mean value established by our study for the SNA angle(80.59W 3.8�) is significantly smaller than Ajayi’s Nigeriannorm [8] (85.5W 4.3�), Al Jasser’s Saudi norm [9] (83.6W4.3�) and Basciftci’s Turkish norm [10] (82.57W 3.55�).
This shows that, unlike these African, Arab, and Europeansamples, our Moroccan sample is characterized by a moreretruded position of the maxilla in relation to the cranial base.
Comparison of the sagittal position of the mandible:SNB angle
The mean SNB angle for our sample is 77.68 W 3.55�. As withthe SNA angle, this value is significantly smaller than thenorms established in the other studies referenced forcomparison— Ajayi: 81.2 W 4�;— Al Jasser: 81 W 3.7�;— Basciftci: 79.22 W 3.44�.Our sample therefore shows a more retruded position of themandible as well.

Comparaison de la position sagittale dumaxillaire : angleSNA
La valeur moyenne �etablie par notre �etude pour l’angle SNAqui est de 80,59W 3,8� est significativement inf�erieure a lanorme nig�erienne d’Ajayi [8] : 85,5W 4,3�, a la norme saou-dienne d’Al Jasser [9] : 83,6W 4,3� ainsi qu’a celle de Basciftci[10] concernant une population turque qui est de 82,57W3,55�.Cela signifie que, contrairement a ces �echantillons africain,arabo-asiatique et europ�een, notre �echantillon marocain secaract�erise par une position plus recul�ee du maxillaire parrapport a la base du crane.
Comparaison de la position sagittale de la mandibule :angle SNB
Pour notre �echantillon, l’angle SNB est en moyenne de77,68W 3,55�. Comme pour l’angle SNA, cette valeur estsignificativement inf�erieure aux normes des autres �etudesde comparaison, soit :— Ajayi : 81,2 W 4� ;— Al Jasser : 81W 3,7� ;— Basciftci : 79,22W 3,44�.Ce qui signifie �egalement une position mandibulaire plusrecul�ee sagittalement pour notre �echantillon.

Comparison of the sagittal relationship of the bonybases: ANB angle
In our study, the sagittal discrepancy between the bony basesis 3.11 W 1.68�. This value is significantly greater than thosefound in Basciftci’s study (2.65 W 1.63�) and Al Jasser’s(2.5 W 2�). In a study performed on a Saudi population,Hassan [11] found an ANB angle wider than that shown byEuropean and American norms.
Ajayi also reports a significantly greater sagittal discrepancy(4.3 W 2.5�) for Nigerian children.Similar results were found by Bailey [12], whose study con-cluded that Americans of African descent show a significantlymore prognathic position of the maxilla and mandible thanwhite Americans.On the other hand, Hamdan and Rock [13] found that the ANBangle (as well as the SNA and SNB angles) of a Jordanianpopulation was significantly similar to European andAmerican norms.
Comparison of the sagittal positions of the upper andlower incisors: I/NA and I/NB angles
Our sample’s I/NA norm (23.75W 6.1�) is between thosereported by Basciftci (21.47 W 6�) and Al Jasser(24.8 W 5.6�).On the other hand, our I/NB angle is slightly wider than thoseof Basciftci and Al Jasser. This can be explained by slightlabial crown tipping of the lower incisors in our sample. Thisdifference, however, is not statistically significant.
Comparison of sagittal inter-incisal relationships: I/iangle
Our angle (125.01W 8.45�) is similar to Al Jasser’s 2005angle (124.8 W 6.9�), but significantly wider than the anglereported by Ajayi in that same year (109.1W 8�). It is alsosignificantly more acute than Basciftci’s 2004 angle(128.32 W 10.06�) and Steiner’s, as previously stated. Thisconfirms that our sample exhibits labial crown inclination ofthe upper and lower incisors.
Conclusion
The objective of this cephalometric study was to work towardsestablishing cephalometric norms, based on Steiner’s analy-sis, for young Moroccan adults aged between 19 and 27 fromthe patient population of the Center for Dental Treatment andConsultation in Casablanca, and to compare these norms toSteiner’s as well as to those found in similar studies.Statistical comparison with Steiner’s norms reveals that oursample presents the following traits:

Comparaison des positions sagittales des incisivessup�erieures et inf�erieures : angles I/NA et I/NB
La norme relev�ee au sein de notre �echantillon pour l’angle I/NA (23,75 W 6,1�) est situ�ee aumilieu des normes rapport�eespar Basciftci (21,47 W 6�) et par Al Jasser (24,8W 5,6�).La valeur de I/NB que nous avons relev�ee pr�esente, enrevanche, une l�eg�ere hausse par rapport a ceux de Basciftciet Al Jasser. Cela pourrait etre expliqu�e par une l�eg�ere versioncorono-vestibulaire de l’incisive inf�erieure pour notre�echantillon. Cependant, cette diff�erence n’est pas statistique-ment significative.
Comparaison des rapports inter-incisifs sagittaux : angleI/i
Notre angle de 125,01 W 8,45� se rapproche de celui d’AlJasser (2005) : 124,8 W 6,9�. Il est cependant significative-ment plus important que celui rapport�e par Ajayi (2005)a 109,1 W 8� mais reste significativement inf�erieur a celui deBasciftci (2004) a 128,32W 10,06� et a celui de Steinercomme rapport�e pr�ec�edemment, ce qui confirme la versioncorono-vestibulaire des incisives sup�erieures et inf�erieures denotre �echantillon.
Conclusion
L’objectif de la pr�esente �etude c�ephalom�etrique �etait de contri-buer a l’�etablissement de normes c�ephalom�etriques, selonl’analyse de Steiner, pour des jeunes adultes marocains, ag�esde 19 a 27 ans, consultant au Centre de soin et de traitementdentaire de Casablanca, et de les comparer aux normesadmises de Steiner ainsi qu’a celles d’autres �etudes similaires.La comparaison statistique aux normes admises de Steiner ar�ev�el�e que notre �echantillon pr�esentait :

— maxillary and mandibular bony bases with sagittalpositions;— labial crown tipping for upper and lower incisors;
— more siginficant sagittal skeletal discrepancy.Comparison of our results with those from other studies allowsus to say that our Moroccan sample is characterized by a moreretruded position of the maxilla and mandible, a larger skel-etal sagittal discrepancy, and greater labial crown tipping ofthe lower incisors.
The results of this study clearly reveal the existence of severalcephalometric particularities within our sample, especially ascompared to foreign populations. Nonetheless, these particu-larities cannot be generalized to the entire Casablanca orMoroccan population unless additional, more exhaustive stud-ies are performed on statistically representative samples.
