NOTICE INVITING BIDS FURNISH LABOR AND MATERIALS FOR

Fair Oaks Health Center Renovation Project Interested bidders shall submit three copies of sealed bids prior to 4:00 PM, January 25, 2018 to Francisco Benavides, Chief Financial Officer, Community Health Alliance of Pasadena, 455 W. Montana Street, Pasadena, CA 91103 and will be opened at that time and place. Copies of this bid package may be obtained at Community Health Alliance of Pasadena, 455 W. Montana Street, Pasadena, CA 91103 during normal business hours, Monday through Friday. Bids will be submitted in sealed envelopes and clearly titled: Fair Oaks Health Center Renovation Project. Bidders shall refer to attached project description and architectural drawings for details. Additionally, bidders are informed this is a federally funded project. As such, bidders are directed to attachment C, Federal Requirements and Forms, for a description of such requirements and copies of forms which must be submitted with all bids.

This is a federally-assisted construction project. Federal Labor Standards Provisions outlined in the

HUD-4010 form, including the prevailing wage requirements of the Davis-Bacon and Related Acts

(DBRA) will be enforced. The “current Federal Wage Decision” is the one in effect 10-day prior to

the bid opening date and can be found on-line at http://wdol.gov. The State wage rates are

available online at http://www.dir.ca.gov/DLSR/PWD/index.htm. In the event of a conflict between

Federal and State wages rates, the higher of the two will prevail. Lower State wage rates for work

classifications not specifically included in the Federal wage decision are not acceptable.

Apprenticeship Program: Attention is directed to Sections 1777.5, 1777.6, and 1777.7 of the

California Labor Code and Title 8 of the California Administrative Code, Section 200, et seq., to

ensure compliance and complete understanding of the law regarding apprentices.

Conflict of Interest: in the procurement of supplies, equipment, construction, and services by sub-

recipients, the conflict of interest provisions in 24 CFR 85.36, OMB Circular A-110, and 24 CFR

570.611 shall apply. No employee, officer, or agent of the sub-recipient shall participate in the

selection, award, or administration of a contract supported by federal funds if a conflict of interest,

real or apparent, would be involved.

Section 3 Bid Preferences: This HUD Section 3 covered construction project is intended to create

economic opportunities for low-income residents and qualified businesses in the project area. First

preference will be given to a bidder who is a qualified Section 3 Business Concern and provides a

reasonable bid. Second preference will be given to a bidder who commits to employing income

qualified residents or to subcontracting with qualified business concerns; identifies the positions

and/or subcontractors to be hired; and provides a reasonable bid. A bid is considered reasonable

for Section 3 if the bid is found to be within the Zone of Consideration, as outlined in 24 CFR 135,

detailed in the project specifications, and explained at the pre-bid meeting.

Attachment A: Project Description

This project will convert office and storage space at the 1855 N. Fair Oaks health center into two (2) additional behavioral health counseling rooms, for a total of four (4). Further, project consists of demolition, including minor wall removal and doors to be relocated. The erection of new, non-bearing partition walls, per City of Pasadena requirements. Per the plan, supply and install new T bar ceiling and relocated existing lights. Relocate existing power, signal and HVAC ducting in the two (2) additional offices and as required. Paint all walls and replace all flooring.

14. THE MAXIMUM FORCE REQUIRED TO PUSH OR PULL OPEN A DOOR SHALL COMPLY WITH THE FOLLOWING.PUSH OR PULL FORCE FOR A HINGED DOOR SHALL BE MEASURED PERPENDICULAR TO THE DOOR FACE AT THE DOOROPENING HARDWARE OR 30 INCHES FROM THE HINGED SIDE, WHICHEVER IS FARTHER FROM THE HINGE. A. REQUIRED FIRE DOORS SHALL HAVE THE MINIMUM OPENING FORCE ALLOWABLE BY THEAPPROPRIATE ADMINISTRATIVE AUTHORITY, NOT TO EXCEED 15 LBF.

B. OTHER THAN REQUIRED FIRE DOORS, INTERIOR DOORS SHALL HAVE A MINIMUM OPENING FORCEOF 5 LBF.

15. WHEN THE DOOR HAS A CLOSER, THEN THE SWEEP PERIOD OF THE CLOSER SHALL BE ADJUSTED SO THATFROM AN OPEN POSITION OF 70 DEGREES THE DOOR WILL TAKE AT LEAST 3 SECONDS TO MOVE TO A POINT 3INCHES FROM THE LATCH, MEASURED TO THE LEADING EDGE OF THE DOOR. (1133B.2.5.1)

30. THE CONTRACTOR MAY BE REQUIRED TO EXECUTE PORTIONS OF THE WORK AFTER NORMAL BUSINESS HOURSOR ON WEEKENDS. ALL WORK SHALL BE SCHEDULED TO PREVENT INTERRUPTION OF THE REGULAR ROUTINE OF THETENANT AND/OR THE BUILDING OWNER.

31. ALL EXISTING DAMAGED, IRREGULAR, OR MARRED SURFACES SHALL BE REPAIRED AND BROUGHT UP TO THEREQUIRED QUALITY OF THE NEW WORK.

32. WHEREVER EXISTING EQUIPMENT, PIPING, DUCTS ETC. ARE REQUIRED TO BE REMOVED, SUCH REMOVALSHALL INCLUDE ALL ANCHORS, HANGER, FOUNDATION, ETC AFTER REMOVAL, FLOORS, WALLS AND CEILING SHALL BEFINISHED TO MATCH ADJOINING SURFACES.

Attachment C: Federal Requirements and Forms This is a federally-assisted construction project. Federal Labor Standards Provisions outlined in the HUD-4010 form, including the prevailing wage requirements of the Davis-Bacon and Related Acts (DBRA) will be enforced. The “current Federal Wage Decision” is the one in effect 10-day prior to the bid opening date and can be found on-line at http://wdol.gov. The State wage rates are available online at http://www.dir.ca.gov/DLSR/PWD/index.htm. In the event of a conflict between Federal and State wages rates, the higher of the two will prevail. Lower State wage rates for work classifications not specifically included in the Federal wage decision are not acceptable. Apprenticeship Program: Attention is directed to Sections 1777.5, 1777.6, and 1777.7 of the California Labor Code and Title 8 of the California Administrative Code, Section 200, et seq., to ensure compliance and complete understanding of the law regarding apprentices. Conflict of Interest: in the procurement of supplies, equipment, construction, and services by sub-recipients, the conflict of interest provisions in 24 CFR 85.36, OMB Circular A-110, and 24 CFR 570.611 shall apply. No employee, officer, or agent of the sub-recipient shall participate in the selection, award, or administration of a contract supported by federal funds if a conflict of interest, real or apparent, would be involved. Section 3 Bid Preferences: This HUD Section 3 covered construction project is intended to create economic opportunities for low-income residents and qualified businesses in the project area. First preference will be given to a bidder who is a qualified Section 3 Business Concern and provides a reasonable bid. Second preference will be given to a bidder who commits to employing income qualified residents or to subcontracting with qualified business concerns; identifies the positions and/or subcontractors to be hired; and provides a reasonable bid. A bid is considered reasonable for Section 3 if the bid is found to be within the Zone of Consideration, as outlined in 24 CFR 135, detailed in the project specifications, and explained at the pre-bid meeting.
