CITY OF LIVONIA STANDARD CONSTRUCTION NOTES

1. ALL WORK AND MATERIALS SHALL BE IN ACCORDANCE WITH THE CURRENT SPECIFICATIONSAND STANDARD DETAILS OF THE CITY OF LIVONIA.

2. NOTIFY THE CITY OF LIVONIA ENGINEERING DIVISION (734) 466-2571 A MINIMUM OF 48 HOURSPRIOR TO THE START OF CONSTRUCTION.

3. CALL MISS DIG (1-800-647-7344/ 1-800-MISS DIG) A MINIMUM OF 72 HOURS PRIOR TO THE STARTOF CONSTRUCTION.

4. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ANY AND ALL DAMAGES TO EXISTINGUNDERGROUND UTILITIES CAUSED BY THE CONTRACTOR'S OPERATIONS.

5. TRENCHES UNDER OR WITHIN 3 FEET OF EXISTING OR PROPOSED PAVEMENTS, DRIVEWAYS,AND/OR SIDEWALKS SHALL BE BACKFILLED AND COMPACTED WITH M.D.O.T. CLASS II SAND.

6. WHERE UTILITIES CROSS, PROVIDE POROUS BACKFILL TAMPED IN 12" LAYERS TO THEUNDERSIDE OF THE HIGHER UTILITY. A 6" MINIMUM SAND CUSHION SHALL BE PROVIDEDBETWEEN UTILITIES.

7. ALL SEWER CONSTRUCTION SHALL HAVE M.D.O.T, CLASS II BEDDING UNLESS OTHERWISESPECIFIED ON THE PROFILES.

8. ALL ELEVATIONS SHOWN ARE BASED ON U.S.G.S. DATUM.9. ALL DISTURBED AREAS BEHIND THE SCHOOLCRAFT ROAD RIGHT-OF-WAY SHALL BE

RESTORED BY PLACING 3" OF TOPSOIL AND SODDING IN ACCORDANCE WITH THE CITY OFLIVONIA STANDARDS.

10. ALL DRIVEWAYS, DRAIN TILES, CULVERTS AND OTHER FACILITIES REMOVED, DESTROYED,DAMAGED OR OTHERWISE DISTURBED BY THE WORK SHALL BE REPLACED AND/OR RESTOREDTO THEIR ORIGINAL CONDITION BY THE CONTRACTOR.

11. ALL SOIL EROSION AND SEDIMENTATION MUST BE CONTROLLED AND CONTAINED ON SITE.12. THE CONTRACTOR MUST MAINTAIN ALL TRAFFIC AT ALL TIMES AS REQUIRED BY THE CURRENT

MICHIGAN MANUAL OF UNIFORM TRAFFIC CONTROL DEVICES (M.M.U.T.C.D.), WAYNE COUNTY,AND/OR MICHIGAN DEPARTMENT OF TRANSPORTATION (M.D.O.T.)

13. THE CONTRACTOR SHALL AT ALL TIMES PROVIDE EMERGENCY ACCESS TO ALL PROPERTIESFOR POLICE, FIRE DEPARTMENT, AMBULANCES AND/OR OTHER EMERGENCY VEHICLES TOPROTECT LIFE, HEALTH AND PROPERTY.

14. PAVED STREETS AND DRIVEWAYS MUST BE MAINTAINED IN A REASONABLE STATE OFCLEANLINESS AND THE CONTRACTOR SHALL REMOVE ACCUMULATIONS OF DEBRIS CAUSEDBY CONSTRUCTION OPERATIONS.

15. THE CONTRACTOR SHALL HAVE AN OPERATING SWEEPER BROOM AND CLEAN THE PAVEMENTAT THE CLOSE OF EACH DAY'S OPERATION AND AS OFTEN AS NECESSARY. FAILURE TOCOMPLY SHALL BE CAUSE TO STOP CONSTRUCTION. CONTRACTOR SHALL COMPLY WITH THESTATE AIR POLLUTION CONTROL REGULATIONS AND CITY OF LIVONIA SOIL EROSION ANDSEDIMENTATION CONTROL (SESC).

16. CONTRACTOR MUST PROVIDE ALL NECESSARY SHEETING, SHORING, DEWATERING, BRACING,TRENCH BOXES, ETC. TO PERFORM WORK SAFELY, PROTECT EXISTING UTILITIES ANDIMPROVEMENTS IN ACCORDANCE TO MICHIGAN OCCUPATIONAL SAFETY AND HEALTHADMINISTRATION (MIOSHA).
